Items Needed for Enrollment 2015-2016 1. Last report card showing student promotion/retention. 2. Parent/Guardian ID (Guardian must provide legal documents regarding guardianship.) 3. Proof of Residency (Gas, Water, Electric bill, mortgage or a lease agreement in the parent or guardians name. If you are sharing an apartment, the lease must include parent and guardians name. If you are sharing a home and no bills are under your name, you will have to fill out an oath that can be picked up at the attendance office. The oath must be notarized.) 4. Birth Certificate 5. ID del padre o tutor (tutores tendran que presenter documentos legales especificando la tutorial del alumno.) 3. Comprobante de domicilio (Gas, Agua, Electricidad, hipoteca o contrato de renta de apartamento con el nombre del padre o tutor. Si comparte apartamento, el contrato devera presentar el nombre del padre y alumno. Si comparte una casa y no tiene facturas en su nombre, tendrá que llenar un juramento que podra recoger de la oficina de asistencia y tendra que notarizar.) 4. Certificado de nascimiento 5.
