ITEMS NEEDED FOR REGISTRATION
(CANNOT REGISTER WITHOUT THE FOLLOWING)
• Parent must be present for student under 18
• Valid Identification for Parent (Parent’s name must be listed on birth certificate)
OR
Valid Identification for legal guardian (must provide proof of guardianship signed by a
judge or original “determination of residency of a minor” form issued by HISD 4400
West 18th st, Houston TX 77092)
• Proof of address:
o Gas, Electricity or water bill with parent’s name listed (Parent’s name must be
listed on birth certificate)
Statement of residency may be used if parent/ legal guardian is not listed on utility bill (must
bring driver’s license of person listed on utility bill and a notarized letter from that person
indicating the parent and student is residing at that home address)
OR
o Lease agreement (Parent and student’s name must be listed on the lease)
Statement of residency may be used if parent/ legal guardian is not listed as lease
holder but listed as occupant along with the student (must bring driver’s license of
person listed as lease holder and a notarized letter from that person indicating the parent and
student is residing at that home address)

School record:
o Last Middle School Report Card – first time 9th graders (Must indicate child was
PROMOTED)
o Transcript – Grade 10, 11 & 12
o Summer School Grades (if applicable)
A CONFERENCE WITH THE PRINCIPAL IS REQUIRED IF:
• Student is returning from CEP or Juvenile Detention Center
• Had excessive absences/poor conduct
THE ABOVE NAMED DOCUMENTS ARE REQUIRED FOR REGISTRATION. ATTEMPT
TO ENROLL WITHOUT COMPLETE SET OF DOCUMENTS WILL NOT BE HONORED.

(NO SE PUEDE REGISTRAR SIN LOS SIGUIENTES)

Los padres deben estar presentes para que estudiantes menores de 18 años sean inscritos
Identificación válida para Padres (Nombre del padre debe aparecer en la acta de nacimiento)
O
Identificación válida para tutor legal (debe presentar prueba de tutela firmada por un juez o original
"determinación de la residencia de un menor" formulario expedido por HISD 4400 West 18th St,
Houston TX 77092)

Comprobante de domicilio:
o Factura de gas, electricidad o agua, con el nombre del padre en la lista (nombre de los padres
deben estar inscritos en el acta de nacimiento)
Estado de residencia puede ser usado si el padre / tutor legal no aparece en la factura
de servicios públicos (debe traer su licencia de conducir de la persona que aparece en la
factura de servicios públicos)

o Contrato de Renta (Padres y estudiantes deben ambos ser incluido en el contrato)
Estado de residencia puede ser usado si el padre / tutor legal no figura como arrendatario, pero
aparece como ocupante junto con el estudiante (debe traer su licencia de conducir de la persona que
figura como titular del contrato de arrendamiento) y un carta certificada ante notarizada de esa persona
que indica al padre y al estudiante está residiendo en el domecilio)
Acta de Nacimiento
Tarjeta de Seguro Social (si tiene)
Los registros actuales de vacunación
Registro escolar:
o Última Calificaccion de Escuela Media - primera vez 9 º grado (debe indicar niño fue ascendido)
o Transcripción - Grado 10, 11 y 12
o Grados de Escuela de Verano (si procede)
UNA CONFERENCIA CON LA DIRECTORA SE REQUIERE SI:

El estudiante está regresando de CEP o Centro de Detención Juvenil
Tenía un exceso de ausencias / conducta pobre
LOS DOCUMENTOS ANTERIORES MENCIONADOS SON
NECESARIOS PARA EL REGISTRO. TRATE DE INSCRIBIR SIN JUEGO
COMPLETO DE DOCUMENTOS NO serán honrados.
