Verification of residency dated within the last 45 days, with
the name of the parent or legal guardian. (Electric or
telephone bill.)
If legal custody of a child is split between two parents, in
addition to the documents listed above, you must also attach
a certified copy of the court order identifying each parent’s
respective award of physical custody. You must immediately
inform the school of any changes to the court order.

Si la custodia legal de un menor esta dividida entre ambos
padres, además de los documentos mencionados, debe
adjuntar una copia certificada de la orden judicial que
identifica la asignación respectiva de custodia física de cada
uno de los padres. Usted tiene el deber de informar
inmediatamente a la escuela de cualquier cambio en la orden
judicial.
