Dear Parents, / Estimados Padres de Familia:
Please choose the color of your son/daughter’s ID for the school year 2015 – 2016. You must fill out an application for
each child.
Les pedimos sean tan amables de confirmar el color de la credencial para su hijo(a) en el ciclo escolar 2015– 2016.
Deberán llenar una solicitud para cada hijo(a).
Elementary
_____ Green / Verde:
They leave by themselves at 2:30 p.m., Sabinos exit / Salida libre a las 2:30 p.m. por Sabinos.
_____ Yellow / Amarilla: By loudspeaker, Pinos exit. / Por voceo, salida por Pinos.
_____ Red / Roja:
You have to come in and get your child., Sabinos entrance / Tiene que entrar a recoger
a su hijo(a) por Sabinos.
Middle School y High School (6° - 12°)
_____ Green / Verde:
They leave by themselves at 2:30 p.m. / Salida libre a las 2:30 p.m.
High School (10° - 12°)
_____ Purple / Morada: They can leave from 11:25 a.m. to 11:55 a.m. and at 2:20 p.m. /
Pueden salir de 11:25 a.m. a 11:55 a.m. y a las 2:20 p.m.
“I hereby request that my son / daughter __________________________________________, who is in _____ grade, is
given an ID of the color checked above.
“Solicito autorización para que mi hijo(a) ____________________________________________, quien está en _____
grado, se le dé una credencial del color elegido arriba.
I understand that The John F. Kennedy School will not accept any responsibility whatsoever for students while they are
not in the school premises and that there is no insurance coverage available in this circumstances.”

I understand that if a student has a purple credential, and they misuse it or are not using it correctly, their credential
will be exchanged from purple to green for the rest of the semester.
Comprendo que en caso de que el alumno tenga credencial morada, y éste haga mal uso de ella o realiza alguna acción
indebida, se le cambiará de color morado a verde por el resto del semestre.
