School fees are based on your student’s previous year’s lunch
status, unless you apply for a waiver at the beginning of the year.
Students whose families meet the income guidelines for free and
reduced price lunch, the Family Investment Program (FIP), or
transportation assistance under open enrollment, or who are in
foster care are eligible to have their student fees waived or
partially waived. Students whose families are experiencing a
temporary financial difficulty may be eligible for a temporary
waiver of student fees. Parents or students who believe they may
qualify for temporary financial hardship should contact the main
office at 684-6511, ext. 2150. This waiver does not carry over
from year to year and must be completed annually.
If you would like to order a yearbook for the 2016-2017 school
year, please include $18.00 with your fees.
Please return fee payment with your student’s registration
packet.
ACTIVITY CARDS
If students would like to purchase a 2016-2017 Activity Card, the
cost is $10.00. An Evans Middle School Activity Card grants
middle school students access to all Evans athletic home events.
You can pay for the Activity Card when you pay for registration.
Cards will be distributed to students in August. There will be a
$5.00 replacement fee for an Activity card.
NEW VACCINATION REQUIREMENT FOR INCOMING

7 GRADE STUDENTS
Starting with the 2013-2014 school year the Iowa Department of
Public Health, Bureau of Immunization, requires a one-time
booster dose of tetanus, diphtheria, and acellular pertussisth
containing vaccine (Tdap), for students entering 7 grade and
above, if born on or after September 15, 2000; regardless of
the interval since the last tetanus/diphtheria containing
vaccine was given.
Please contact Wapello County Public Health Nurses, or your
practitioner to set up an appointment for your student to receive
this vaccination before school starts in August 2016. This vaccine

can also be given during the 7 grade physical. Please provide
proof of vaccination to Evans Middle School, as your student may
be excluded from school without it.

7 & 8 GRADERS
GET YOUR PHYSICALS NOW!
Under the Ottumwa Community Schools Policy, seventh graders
must have a physical before they can begin school. Both seventh
and eighth graders must have a physical if they plan to participate
in sports. Students must have a physical on file with the school
nurse by their first practice.

If you are a 7 or 8 grader, there is a green physical form
included in your enrollment packet. You must take your green
physical form with you to your physical. Be sure to complete the
front side of the physical examination form and don’t forget to sign
on the back after your student receives their physical. Once your
physical is complete, please return the green physical form to
your coach or the school nurse.
Your physical may be obtained from your health practitioner or
the Teen Health Center at OHS at 683-4444, ext. 2215.

SCHOOL LUNCHES
Free and reduced lunch applications for the 2016-2017 school
year will be mailed home to parents at the end of July. The free
and reduced lunch application will also be available on-line in
August. If you would like your student to start the year off with a
credit balance, feel free to deposit money in your student’s
account prior to the end of the school year. If your student has a
negative balance, please make payment before the end of the
year. School lunch account balances do transfer with students
and carry over from year to year. Lunch prices at the middle
school are different than at the elementary level. The 2016-2017
lunch prices will be on the free/reduced lunch application as well
as on the August lunch menu.

Las cuotas escolares son basadas en el estado de almuerzos del
año anterior, a menos que aplique por otra forma al inicio del año
escolar
A los estudiantes que sus familias llenen las guías de ingreso
para almuerzos gratuitos y reducidos, el Programa de Inversión
Familiar (FIP), o asistencia de transporte bajo registración
abierta, ó quienes están al cuidado de familias de crianza son

Padres o estudiantes que crean que pueden calificar por una
etapa financiera difícil pueden comunicarse a la oficina principal

debe ser completado anualmente.

ALMUERZOS ESCOLARES
Las aplicaciones para almuerzos reducidos y gratuitos para el
año escolar 2016-207 serán enviados a los padres por correo al
final del mes de julio. Las aplicaciones para las comidas
reducidas y gratuitas estarán también disponibles en el internet
en agosto. Si quieren que su estudiante comience el año sin
ningún balance, por favor asegurarse de pagar antes del fin del
año. Los balances de la cuenta de almuerzos se transfieren con
los estudiantes y se llevan de año en año. Los precios para el
año 2016-207 estarán disponibles en las aplicaciones de
almuerzos reducidos/gratuitos junto con el menú de almuerzos
para el mes de agosto.
TARJETAS DE ACTIVIDADES
Si los estudiantes quieren comprar una Tarjeta de Actividades el
costo es de $10.00. Una Tarjeta de Actividades de la Escuela

NUEVOS REQUICITOS PARA LA VACUNACIÓN PARA LOS
ESTUDIANTES QUE ENTRAN A 7° GRADO
Comenzando con el año escolar 2013-2014, el Departamento de
Salud Pública de Iowa, Oficina de Inmunización, requiere una vez
la dosis de refuerzo para el tétanos, difteria, y tos ferina (Tdap),
para los estudiantes entrando al 7° grado y arriba, si nació en o
antes del 15 de septiembre del 2000; sin tener en cuanta el

primera práctica.
Si usted es de 7° u 8° grado, hay una forma verde para el físico
adjunto a este boletín. Usted debe de llevar esta forma verde a
su médico. Asegúrese de completar la parte del frente de la
forma del examen físico y no olvide firmarlo en la parte de atrás
luego de que su estudiante sea revisado por su médico. Una vez
que su físico sea completado, por favor regrese la forma verde
del físico a su entrenador o enfermera de la escuela
