to be completed.
A Home Language Survey is used determine the language(s) spoken in a student’s home. This is done in
order to identify if there are specific language needs. This information is essential in order for our school
to provide meaningful instruction to all students. If a language other than English is spoken in the home,
there will be further assessment to determine if your child has any specific language needs. Please help
