JOIN THE BAND!
Finally, you get a chance to shine by being in the band!
If you would like to be part of the band, return this form to your classroom teacher or the band director, Mrs. Jenson,
Worthington Middle School. We will contact you soon with all of the information and how you can choose your very
own instrument.
Parents: Please sign this form giving your permission for your student to be part of the band! If you have any
questions, please contact me at Worthington Middle School 376-4174 or [email protected].
The instrument display will be held at Worthington Middle School, Tuesday, April 26 from 2-8 PM.
Representatives from a local music store will be on-site to assist you with purchases and rentals. Please bring check
and credit card info and your driver’s license.
All students need their instrument in class by August 1, 2016 for the summer band camp at Worthington Middle
School. Summer camp runs from August 1 to August 12 with lessons every forty-five minutes.

Si quieres se parte de la Banda, regresa esta forma a tu maestro(a) o la directora de Banda, la maestra Jenson en
Worthington Middle School. Nos comunicaremos contigo con toda la información, incluyendo cómo escoger tu
propio instrumento.

La exhibición de instrumentos se llevará a cabo en Worthington Middle School el martes 26 de abril de 2-8 PM.
En este evento estarán algunos representantes de una tienda local de instrumentos musicales para ayudarle con la
compra o renta del instrumento. Traiga su chequera o su tarjeta de crédito y su licencia de conducir.
Todos los estudiantes necesitan su instrumento en clase antes del 1ro de agosto del 2016 para el programa de verano
de Worthington Middle School. El programa de verano será del 1ro al 12 de agosto con lecciones de 45 minutos.
