Welcome to Moorpark High School, Home of the Musketeers!
Moorpark High School is very excited about your prospective enrollment to our campus! We are confident that
you and your student will find your experience here at MHS to be filled with many educational and
extracurricular opportunities.
In order to become a Moorpark High School student you must enroll through our MHS Admissions Office.
Below is a list of the required documents needed prior to enrolling at Moorpark High School.
PLEASE PROVIDE COPIES OF THE REQUIRED DOCUMENTS BELOW:

Completed Student Registration Form
Immunization Records
Birth Certificate
7th and 8th Grade Report Cards for Students Entering Grade 9
Transcript for Students Entering Grades 10-12
Withdrawal Grades from Previous School if transferring during school year
TWO Proof of Residency Items:
 Gas, Electric or Water Utility Bills (These are the only acceptable utility bills)
 Mortgage or Rental Receipts with the residential address listed
 Lease Agreements with the residential address listed
 If the above items are paid in the name of another adult in your household, other
than your spouse, you must supply two Proof of Residency items in their name,
along with a “Verification of Residency” form available from MHS
Proof of Inter-District transfer (if applicable)
Custody Paperwork (if applicable)
Caregiver Authorization Affidavit Form (if student is living with someone other than parent)
State Testing results, example: CAT 6 scores, High School Exit Exam, CELDT scores
Students Most Recent IEP (if applicable)
California Student Identification Number if student is coming from a school in California

Lewis, in the Admissions Office at 517-2111 to make an appointment for
registration.
Please note, that your student will not be attending school on the day of
registration.

El gas, Cuentas de Utilidad Eléctricas o Acuáticas (Éstos son las únicas cuentas de utilidad
aceptables)
La hipoteca o los Recibos de Alquiler con la dirección residencial puestos en una lista
Arriende Acuerdos con la dirección residencial puesta en una lista
Si los susodichos artículos son pagados en nombre de otro adulto en su casa, además de su
cónyuge, usted debe suministrar dos Prueba de artículos de Implantación de su nombre,
junto “con una Verificación de la Implantación” forma disponible de MHS
Prueba de transferencia de Interdistrito (si aplicable)
Trabajo de escribir de custodia (si aplicable)
La Forma de Declaración jurada de Autorización de Caregiver (si el estudiante vive con alguien
además del padre)
Resultados de Pruebas de estado, ejemplo: GATO 6 tanteos, Examen de Salida de Escuela
secundaria, tanteos de CELDT
Estudiantes IEP más reciente (si aplicable)
El Número de identificación Estudiantil de California si el estudiante viene de una escuela en
California
Cuando usted tiene todos los documentos requeridos, por favor llame a Sra Nancy
