El Sistema Inmunitario tiene como función principal proteger al organismo contra diversas
enfermedades; sin embargo, éste podría tornarse en su contra y atacar a las células sanas del
cuerpo. Esto se conoce como autoinmunidad, la cual se puede presentar en dos tipos de

enfermedades: las órgano-específicas y las sistémicas.
Órgano-específicas
Sistemáticas
Enfermedad de Graves (causa más común de hipertiroidismo)
Miastenia gravis
(debilidad muscular grave)
Tiroiditis de Hashimoto (tiroiditis crónica)
Esclerosis múltiple
Enfermedad inflamatoria intestinal
Lupus eritematoso sistémico (tipo más común de lupus)
Artritis reumatoide (inflamación de las articulaciones)
Síndrome de Sjögren (trastorno en las glándulas salivales y lacrimales)
Esclerodermia (esclerosis sistémica progresiva)

Dermatomiositis (inflamación de los músculos esqueléticos)
Entre estas entidades clínicas las más frecuentes son tiroiditis de Hashimoto, en las
órgano-específicas, y la artritis reumatoide, en las sistémicas, según asegura el Dr. Velmar
Quintero, inmunólogo clínico.
En Venezuela, no existen estadísticas confiables sobre la prevalencia de las enfermedades
autoinmunes. “La variedad de estas patologías, que comprometen diferentes órganos y
sistemas, hace difícil agruparlas y determinar su real incidencia y prevalencia. Sin embargo,
aunque no se conozcan cifras exactas, es una realidad que las enfermedades autoinmunes
representan un gran impacto en la calidad de vida y riesgo de complicaciones graves para
quienes las padecen”, enfatiza el Dr. Quintero, Jefe de la Unidad de Inmunología del Hospital
“Dr. Rafael González Plaza” en Valencia.
A nivel global, el Dr. Quintero indica que la prevalencia de lupus eritematoso sistémico
–enfermedad más representativa de la autoinmunidad– se encuentra entre 4 y 250 casos por
cada 100.000 habitantes, dependiendo de la situación geográfica, con una mayor incidencia
entre la población hispana y afroamericana.
Asimismo, se conoce que las más propensas a sufrir enfermedades autoinmunes son las
mujeres, lo que podría estar relacionado con la actividad hormonal durante la etapa
reproductiva. Otros factores que permiten explicar la aparición de estos trastornos son ciertas
infecciones virales; situaciones de estrés; factores ambientales, como exposición a luz
ultravioleta (solar); predisposición genética y consumo de fármacos como penicilinas,
anticonvulsivantes y anticonceptivos.
En cuanto al diagnóstico y tratamiento de estas enfermedades, el Dr. Quintero, también
coordinador de Inmunología del posgrado de Neumología Pediátrica de la Universidad de
Carabobo, vislumbra un panorama positivo. “Hay muchísimas novedades que se convierten en
buenas noticias en cuanto a diagnóstico y tratamiento de las enfermedades autoinmunes.

Vivimos la era de oro en cuanto a la aplicabilidad del conocimiento inmunológico de cómo se
originan, cómo se produce el daño y la determinación de blancos terapéuticos para la creación
de anticuerpos monoclonales (terapias biológicas), que bloquean el proceso inflamatorio y
mejoran la condición del paciente”.
Señales de alarma
Cada una de las enfermedades autoinmunes tiene sus síntomas característicos. Por ejemplo,
el lupus eritematoso sistémico se reconoce por alteraciones en piel y mucosas como
fotosensibilidad, lesiones en el rostro con apariencia de “alas de mariposa” (eritema malar),
orinas espumosas, convulsiones, dolor en las articulaciones, alteraciones hematológicas
(anemia y disminución de las plaquetas), dolor en las articulaciones, fiebre prolongada, urticaria
y angioedema crónicos; la
dermatomiositis se presenta con
enrojecimiento en área de nudillos y en parpado superior, fotosensibilidad, dificultad para
sentarse, pararse e imposibilidad para peinarse; la
artritis reumatoide
debe sospecharse ante dolores e inflamaciones articulares, rigidez matutina (manos
engarrotadas, dificultad para levantarse de la cama), que amerita más de una hora para
mejorar; la
esclerodermia
se caracteriza por alteración en la contextura y color de la piel (manchas oscuras o claras),
hinchazón en los dedos de las manos; mientras que, en el
Síndrome de Sjögren
es relevante evidenciar resequedad en boca y ojos.
Si presenta algunos de estos síntomas, debe acudir a un especialista: médico inmunólogo o
reumatólogo. “Siempre es mejor tener un diagnóstico precoz para actuar y obtener mayor
posibilidad de éxito”, concluyó el Dr. Quintero.
