To enroll in LAYC’s Career Academy, the following documents are
required:
1.
ONE PROOF OF AGE:







A birth certificate
A valid driver’s license showing the student’s name
A valid government or military issued ID showing the student’s name
A passport
A valid INS card showing the student’s name
A valid work permit
PROOF OF DC RESIDENCY:
One of the following:
 A pay stub or tax withholding statement within the last 45 days showing the
student’s, parent’s or legal guardian’s name; or
 Official proof of financial assistance (TANF) from the DC government showing the
student’s, parent’s or legal guardian’s name; or
 Supplemental Security Income annual benefits notification issued to the person
enrolling the student within the past (12) months and indication his/her current
address;or
 Current valid court order indicating that the student is a ward of the District of
Columbia
Or two of the following:
 A valid unexpired DC motor vehicle registration showing the student’s, parent’s or
legal guardian’s name
 A valid unexpired lease or rental agreement with paid receipts or cancelled checks
for payment of rent showing the student’s, parent’s or legal guardian’s name
 A valid DC driver’s license showing the student’s, parent’s or legal guardian’s name
 A DC utility bill (gas, water or electric) and paid receipts or cancelled checks for
payment of rent showing the student’s, parent’s or legal guardian’s name
(cable, cell or telephone bills are not accepted)

VACCINATION RECORDS (including Tuberculosis/PPD test results)
4.
WITHDRAWAL OR TRANSFER FROM FORMER SCHOOL IF APPLICABLE

Credencial militar o del gobierno mostrando el nombre del estudiante.
Permiso de trabajo.
PRUEBA DE RESIDENCIA EN WASHINGTON, DC:
Uno de los siguientes
 Copia de cheque de los últimos 45 días mostrando el nombre del estudiante, padres
o guardián legal.
 Prueba oficial de asistencia publica del gobierno del distrito mostrando el nombre del
estudiante, padres o guardián legal.
 Prueba que el estudiante esta bajo la supervisión de la corte.
 Notificación de beneficios de seguro social al nombre del estudiante, padre o
guardián de los últimos (12) meses y con la dirección comprobando residencia en
DC
O dos de los siguientes
 Registro vehicular en DC mostrando el nombre del estudiante, padres o guardián
legal.
 Contrato de renta valido de DC mostrando el nombre del estudiante, padres o
guardián legal con un cheque cancelado o money order constando que la cuenta fue
pagada.
 Licencia de conducir o identificación valida del gobierno de DC mostrando el nombre
del estudiante, padres o guardián legal.
 Cobros de utilidades (agua, luz o gas) con un cheque cancelado o money order
constando que la cuenta fue pagada. (Cobros de teléfono y cable no son validos)

RECORD DE VACUNAS INCLUYENDO LA PRUEBA DE TUBERCULOSIS

LA RETIRADA O TRANSFIERE DE LA ESCUELA ANTERIOR SI SE APLICA
