Students mayreceiveafeewaiverforoneofthefollowingreasons: • Studentiseligiblebasedonincomeverification. • StudentreceivesSupplementalSecurityIncome(qualifiedchildwithdisabilities) • FamilyreceivesTANF(currentlyqualifiedforfinancialassistanceorfoodstamps) • StudentisinFosterCare(underUtahorlocalgovernmentalsupervision) • StudentisinStateCustody Applicationmustbemadeinperson,andoneofthefollowingmustbeprovidedforverification: • 2015IncomeTaxReturn • Threemostrecentpaystubs • SSI:CurrentverificationofreceivingSSIpayments • TANF:LetterofdecisioncoveringthecurrentperiodfromWorkforceServices • State custody or foster care: Provide the Youth in Custody Required Intake Form and/or School Enrollment Letter provided by the case worker from DCFS or Juvenile Justice Department. EXENCIÓNDECUOTAS:2DEAGOSTO,3,8Y9,2016 Solicitudesparaexencióndecuotasseránrevisadoslosdías2,3,8y9deagostode8:00a15:00.Es necesario que cada familia solicite este servicio anualmente; la aplicación del año pasado no será válida.Losestudiantespuedenrecibirunaexenciónparaunadelasrazonessiguientes: • Estudianteeselegiblebasadoenverificacióndeingresos. • Elestudianterecibe(SSI)Ingresossupleméntalesdeseguridad(estudiantesquecalificanpor discapacidades) • LafamiliarecibeTANF(aquellosquecalificanparaobtenerestampillasparaalimentos) • Elestudianteeshuérfano(bajosupervisiónlocaloestatal). • Estudiantesencustodiaestatal. Solicitud deberá presentarse en persona, y uno de los siguientes debe proporcionarse para su verificación: • Declaracióndeimpuestos2015 • Tresúltimostalonesdesunomina • SSI:VerificaciónactualderecibirpagosdeSSI • TANF:cartadeladecisiónqueabarcaelperíodoactualdeWorkforceServices • Custodia o cuidado de crianza temporal del estado: el formulario de “Jóvenes en Custodia Necesaria” y/o carta de inscripción de la escuela proporcionada por el trabajador social a cargocasodeDCFSodepartamentodejusticiajuvenil.
