ATTENTION PARENTS OF 6TH GRADERS!

 A T T E N T I O N P A R E N T S O F 6 T H G R A D E R S ! The Florida Department of Health and the Department of Education requires all students entering
7th grade to have a Tdap booster (adult strength tetanus, diphtheria and pertussis) vaccine. An
updated Florida Certification of Immunization DH 680 form must be presented to the school before
your child can receive a schedule or enter school on the first day of classes, August 11, 2014.
To assist you with this requirement, the Florida Department of Health in Seminole County will be
hosting a special Tdap clinic for 6th grade students, with extended hours for your convenience.
Note: All children may receive the required vaccines. Children with Medicaid or without insurance are
eligible for the recommended vaccines Hepatitis A, HPV, and Meningococcal.

If you can’t attend the clinic above or see your private provider, your child’s Immunizations and the
DH 680 form can be obtained at the Florida Department of Health in Seminole County, no
appointment needed. Hours are: Monday through Friday, 8:00 a.m. – 3:30 p.m., except for the
2nd Thursday of each month when we are open 8:00 – 11:30 a.m. only. For more information,

IMPORTANT NOTICE
To receive vaccine and/or the required DH 680 form, the child must be accompanied
by a parent or legal guardian (identification required). If the child’s most recent shots
were not given at the Florida Department of Health in Seminole County, parents must
bring the most recent (complete) shot record. We will not be able to administer
vaccines or issue a DH 680 form without a complete shot record.

¡Atención Padres de Estudiantes de Sexto Grado! El Departamento de Salud y el Departamento de Educación requieren a todos los estudiantes que entran a
cursar el 7mo grado, tener el refuerzo para la vacuna de Tdap (Tétano, Difteria y Tosferina). El
certificado de Vacunación de la Florida (Formulario DH 680) debe ser presentado a la escuela antes de
que el niño(a) pueda recibir su calendario o asistir a su primer día de escuela, el 12 de Agosto de 2013.
Para asistirle con éstos requisitos, el Departamento de Salud del Condado de Seminole estará ofreciendo
una clínica especial de vacunación de Tdap para estudiantes de 6to grado, con horario extendido para su
conveniencia.
Nota: Todos los niños(as) tendrán acceso a las vacunas requeridas. Aquellos niños(as) que tengan Medicaid o no
tengan seguro medico, son elegibles para las vacunas recomendadas (Hepatitis A, HPV, y la vacuna contra
el meningococo).

Si no puede asistir a esta clínica o no tiene proveedor médico, podrá obtener las vacunas y/o el
formulario DH680 en la clínica del Departamento de Salud de Seminole, sin cita previa. El horario
es Lunes a Viernes, de 8am-3:30pm, excepto el segundo Jueves de cada mes cuando nuestro
horario es de 8-11:30am solamente. Para mas detalles, llame al 407-665-3281.
AVISO IMPORTANTE
Para recibir las vacunas y/o el formulario DH680 requerido, el niño(a) debe estar
acompañado de su padre/madre o custodio legal (se requiere identificación). Si las
vacunas más recientes del niño(a) no fueron dadas en el Departamento de Salud de la
Florida en Seminole, el padre o custodio legal debe traer el registro de vacunación
más reciente. No podremos administrar vacunas o proveer el formulario DH680 sin un
registro de vacunación actualizado.
No se olvide de apuntar esta fecha y hora en su calendario!
