Infection prevention continues to be at the forefront of health care.
Improvements in infection control and treatment strategies have led to
significant decreases in communicable illnesses, such as seasonal flu,
c.difficile, and methicillin-resistant staphylococcus aureus (MRSA). However,
several challenges to successful elimination of infection in the physician office
setting still remain, especially since physician practices are not governed by
the same guidelines as hospitals that are required to have Infection
Preventionists (IP) on staff to raise awareness and ensure compliance.
Our goal in publishing InControl is to support your efforts in working
toward maintaining the total health of your patients and staff by providing
you with the necessary tools to ensure your practice has what it needs to
prevent the spread of disease. Each quarterly publication outlines critical
aspects of infection prevention, as we help to promote better patient
outcomes in a health care facility. We hope you find the content to be a
comprehensive educational resource that helps you identify and reduce risk
of infection within your practice.
In this issue of InControl, we raise awareness of the importance of hand
hygiene — the single most critical measure to avoid the transmission of
disease in our communities. As part of this initiative, the Henry Schein
Cares Healthy Lifestyles, Healthy Communities Program works with
Community Health Centers in at-risk and underserved communities to
provide free health screening events to over 7,500 children and their
caregivers. Hand soaps and sanitizers are provided to patients to help
educate and demonstrate proper hand hygiene techniques which help stop

When using agents containing alcohol, such as
wipes, the entire surface of the hand must be
covered by the product and allowed to dry
prior to donning gloves. In order to maintain
adequate moisture in the hands, it may be
necessary to apply a health care-grade lotion,
especially during the winter season.
Selection of hand hygiene agents
Hand hygiene agents must be carefully
selected in order to ensure compliance,
efficacy, and safety for both the health care
worker and patient. Several factors must be
taken into consideration:
• Ease of use for health care workers
• Potential for dermal irritation and skin
reactions.
• Efficacy of the agent.
• Product availability and accessibility within
the health care facility
• Cost of the agent
• Drying time necessary after applying
the agent
• Staff support for the selected product.

Studies have demonstrated that proper hand hygiene can decrease the transmission of cold viruses and other
germs. The Centers for Disease Control and Prevention (CDC) promote and encourage routine hand washing,
especially during cold and influenza season. Because soap and water are not always readily available, hand
sanitizers are a popular alternative routinely used by millions of individuals. These products are clearly
convenient, but users should keep in mind that they are only effective for infection prevention when used

There are two layers of bacteria. The outer layer
found on your hands is termed “Transient Flora”.
This layer is potentially the most dangerous for
transmitting disease from one person to another.
Fortunately, it is easily eliminated by hand
washing. The deeper layer is called “Resident
Flora”. This bacteria, found in deeper layers
of skin cells, is more likely to be innocuous
bacteria such as Staphylococcus epidermidis
and Corynebacteria spp. (diptheroids); and
is more resistant to washing.
CDC studies show that the number of
bacteria per square centimeter on the
human body are as follows:
Scalp: 1,000,000 | Forearm:10,000
Arm Pit: 500,000 | Abdomen: 40,000

Guidelines &
Recommendations
The effectiveness of hand hygiene is
improved when: skin is intact, nails are
natural, short, and unvarnished; hands and
forearms are free of jewelry; and sleeves are
above the elbow. Health care staff having
direct contact with patients or a patient’s
environment should comply with the steps
outlined below:
• Bracelets, wrist watches, and rings with
stones or ridges are not to be worn when
providing clinical care. A single flat
ring/band may be worn but should not
interfere with effective hand hygiene
practice.
• Long sleeved clothing should be rolled
up above the elbow so as to not interfere
with effective hand hygiene compliance.

• Nails should be kept short and clean.
Nail polish should not be worn. Artificial
nails (gel or acrylic) are prohibited by any
health care worker with direct patient
contact.
• Any breached skin (cuts, dermatitis or
abrasion) should be covered with a
waterproof film dressing. Staff with
dermatitis should report evaluation as per
local procedures.

to 4.6 x 106 per cm. Given the current state of health care, the government will desire to curb the infection rate
for economic purposes and the benefit of the patient. Perhaps the most cost-effective means to curb the rate of
infection is to evaluate and focus on the preoperative scrubbing techniques that we employ prior to entering
the operative theater suite.

Surgical Scrub Checklist
Here is a checklist to help guarantee processes are in place (or strategies for improvement) to ensure patient
safety from the time that patients enter a health care facility to the time they leave.
1. Provide ongoing education for all staff members including competencies. Education
can be undertaken in many different ways including live in-services, Webinars,
audio-conferences or online training. Health care Facility determined
competencies/assessments for clinical staff should include a skills checklist
addressing hand hygiene knowledge and practices. Education should include all
clinical staff and those providing direct patient care.
2. Provide staff with The World Health Organization's "My 5 Moments of Hand
Hygiene" which defines the key moments when health care workers should
perform hand hygiene. This includes the following:
• Before touching a patient
• Before Clean/aseptic procedures (such as anesthesia services)
• After body fluid exposure/risk
• After touching a patient
• After touching patient surroundings
3. Make hand hygiene easy by providing adequate opportunities for hand washing
and use of alcohol-based hand sanitizers. Sinks should be placed in strategic

locations and supplies such as soap and paper towels should be provided at all
times. Alcohol hand sanitizer products should be user friendly, available at all
times and strategically placed throughout the facility, particularly in clinical areas
and high-demand locations.
4. Introduce hand-hygiene monitoring programs. Tools for monitoring hand-hygiene
compliance are available in various forms and include the use of "secret
shoppers," i.e., a covert observation whereby no one but the person appointing
the observer and the observer him/herself knows who is conducting the
monitoring, or an overt observation whereby all staff members are aware of the
person conducting the observation.
5. Holding staff accountable by providing feedback from monitoring programs. Staff
should be informed of the findings from ongoing monitoring programs and should
be provided the opportunity to improve practice by providing resources,
educational opportunities, and one-on-one consultation.
