Please inform your Screening Technician if you have:
 Traveled outside of the USA or Canada in the past 3 years.
For example, a trip to Mexico or a cruise to the Caribbean.
Please note: Travel must be carefully reviewed for donation quality and safety. This may take a
few extra minutes – we appreciate your patience.
 Spent time that adds up to three months or more in the United
Kingdom (U.K.) between 1980 and 1996 or lived anywhere in
Europe since 1980. Please refer to the Variant Creutzfeldt-Jakob
Disease (vCJD) Countries of Risk document to view a list of the
countries in the U.K. and Europe.
 Been a member of the U.S. military, a civilian military employee, or
a dependent of a member of the U.S. military and:
 You were in Belgium, the Netherlands, or Germany from 1980 through 1990
 You were in Spain (including the Canary Islands and Spanish North African
territories), Portugal (including the Azores), Turkey, Italy or Greece from 1980
through 1996
 Traveled to Africa
Donors with a history of Ebola virus infection or disease should not donate blood
or blood components.
