This form must be completed and returned to your student’s current school site prior to riding the bus for the 2014-15 school year. Included with this letter, you will find an important handout with information to determine if your child is eligible for bus service. If you are not certain of your walking or bus zone, please contact the school site your child will attend next year and they will be able to assist you. Please note that students on approved transfers are not eligible for bus service. If you are eligible for bus transportation and you have reviewed the student responsibilities, safety rules, and bus discipline plan, please complete the bus ridership registration form below. STUDENT RESPONSIBILITIES AND BUS SAFETY RULES: Student transportation is a privilege extended to students who qualify for transportation pursuant to state statutes and local district policy. Students are subject to the jurisdiction of school authorities while using the school transportation system, and shall at all times exhibit appropriate behavior in accordance with codes of conduct and the disciplinary policies, procedures or regulations adopted by the school board. Students who do not obey the state and local regulations governing student transportation may have their transportation privileges revoked by the school district. This also applies to transporting to and from school and field trips. Students must learn and obey the safety rules and responsibilities and use appropriate behavior while on the school bus and at the bus stop. Students must be at their regular bus stop at least five minutes prior to the scheduled stop time. The bus driver will not wait for students not at the loading area or making an effort to reach the bus. Students must wait in a safe place, clear of traffic and away from where the bus stops. Students must maintain safe and appropriate behavior when waiting for and boarding the bus. Students must obey the driver promptly. The driver/attendant is in full charge of students while they are on the bus. Students must remain seated while the bus is in motion and keep aisles and exits clear. Students must occupy the seats assigned them by the bus driver/attendant. Students are not permitted in the driver's compartment or tamper with equipment while the bus is in motion. Students cannot use profane language or obscene gestures while on the bus. Students cannot eat or drink on the bus. Students shall cooperate with the bus driver in keeping the bus clean. Students cannot open or close the bus windows without the permission of the bus driver/attendant. Students must not extend their hands, arms, heads, or objects through bus windows. Students must not throw any item inside or outside the bus while boarding, riding, or leaving the bus. Firearms, knives, explosives, breakable glass, or other dangerous objects are not permitted on the bus. Animals, except those animals authorized by an IEP or 504 Accommodation Plan, are not permitted on the bus. The use, possession, or distribution of tobacco, drugs, alcohol, or other controlled substances is prohibited on the bus. Students and parents/guardians will be held responsible for destruction to the bus, bus stop location, and/or private property. PARENT/GUARDIAN RESPONSIBILITIES: To ensure the safety of all students on a school bus, parent/guardian responsibilities include the following: Parents/guardians are responsible for ensuring that their child/ren learn and obey the school bus safety rules and use appropriate behavior required on a school bus. This includes to and from transportation and field/activity trips. Parents/guardians will ensure that their child/ren arrive at the designated bus stop five (5) minutes prior to the scheduled arrival time. Notes from parents/guardians requesting that their student be picked up or dropped off at a stop other than his/her regular stop will not be allowed unless for emergency day-care reasons. These requests must be made in writing, signed by the parent/guardian and the school site principal and received by the bus driver/transportation dept. Parents/guardians will be responsible for any observed damage by their child/ren on the bus, at the bus stop location, or to private property. Restitution may be pursued. Should a parent/guardian have a compliant regarding a disciplinary action against their child the school principal and or the Transportation Office should be notified. SCHOOL BUS DISCIPLINARY PLAN: Riding the bus is a privilege, not a right. Disobeying any of the Bus Rules, jeopardize student safety and the safety of others on the bus. It is the responsibility of the school bus driver to enforce all regulations governing the conduct of students on the school bus. The driver shall report student noncompliance with these rules to the principal/designee in accordance with local school district policy. RESPONSABILIDADES DEL ESTUDIANTE/PADRE Y FORMA DE INSCRIPCIÓN PARA SERVICIOS DE TRANSPORTE DE SFPS ¿CALIFICA MI HIJO PARA VIAJAR EN EL AUTOBÚS? CALIFICAR: - Mi hijo está en la escuela primaria y vivo más allá de 1 milla de la escuela o en un área peligrosa para caminar. - Mi hijo está en la escuela secundaria y vivo más allá de 1.5 millas de la escuela o en un área peligrosa para caminar. - Mi hijo está en la escuela preparatoria y vivo más allá de 2 millas de la escuela o en un área peligrosa para caminar. - Mi hijo tiene un IEP que declara que él/ella está autorizado para transporte (Por favor Inscríbase a través de la oficina de Educación especial) NO CALIFICA: - Mi hijo está en la escuela primaria y vivo menos de 1 milla de la escuela o no es un área peligrosa para caminar. - Mi hijo está en la escuela secundaria y vivo menos de 1,5 millas de la escuela o no es un área peligrosa para caminar. - Mi hijo está en la escuela preparatoria y vivo menos de 2 millas de la escuela o no es un área peligrosa para caminar. - Transferencia de Inter-Zona no autoriza el transporte Por favor verifique con la Secretaria de sus escuelas para los límites de ir a pie y las zonas peligrosas para caminar. Todos los estudiantes de Atalaya tendrán que registrarse para el año escolar 2013-2014. RESPONSABILIDADES DEL ESTUDIANTE Y LAS NORMAS DE SEGURIDAD DE AUTOBUSES: Transporte de los estudiantes es un privilegio otorgado a los estudiantes que reúnen los requisitos para el transporte en conformidad con las leyes estatales y políticas del distrito local. Los estudiantes están sujetos a la jurisdicción de las autoridades escolares mientras usan el sistema de transporte escolar, y deberán en todo momento demostrar un comportamiento adecuado de acuerdo a los códigos de conducta y las normas disciplinarias, procedimientos o reglamentos adoptados por el consejo escolar. Los estudiantes que no obedecen los reglamentos estatales, locales y gubernamentales que rigen el transporte de estudiantes pueden tener sus privilegios de transporte revocados por el distrito escolar. Esto se aplica para el transporte hacia la escuela/de la escuela a su casa y excursiones. Los estudiantes deben aprender y obedecer las normas de seguridad y responsabilidades y a usar el comportamiento apropiado requerido mientras que esta en el autobús escolar y en la parada de autobús. Los estudiantes deben estar en su parada de autobús regular al menos cinco minutos antes de la hora asignada. El conductor del autobús no esperará por los estudiantes en la parada o haciendo un esfuerzo para alcanzar el autobús. Estudiantes deben esperar en un lugar seguro, visible de tráfico y lejos de donde el autobús se detiene. Los estudiantes deben mantener seguro y adecuado comportamiento cuando esperan, y mientras abordan el autobús. Estudiantes deben obedecer al conductor con prontitud. El conductor/operador está a cargo completo de estudiantes mientras están en el autobús. Los estudiantes deben permanecer sentados mientras el autobús está en movimiento y mantener los pasillos y salidas libres de obstáculos. Los estudiantes deben ocupar los asientos que les asigne el conductor/operador del autobús. Los estudiantes no serán permitidos en el área del chofer, ni la manipulación del equipo del autobús mientras el autobús está en movimiento. Los estudiantes no pueden usar lenguaje profano o gestos obscenos, mientras que están en el autobús. Los estudiantes no pueden comer o beber en el autobús. Los estudiantes cooperarán con el conductor del autobús en mantener limpio el autobús. Los estudiantes no pueden abrir o cerrar las ventanas del autobús sin el permiso del conductor de autobús/asistente. Los estudiantes no deben extender sus manos, brazos, cabezas u objetos por las ventanas del autobús. Los estudiantes no deben lanzar objetos dentro o fuera del autobús mientras abordan, viajan o se bajan del autobús. Las armas de fuego, cuchillos, explosivos, vidrio rompible u otros objetos peligrosos no están permitidos en el autobús. Animales no son permitidos, excepto los animales autorizados por un IEP o Plan 504 de acomodación especial. Se prohíbe el uso, la posesión o la distribución de tabaco, drogas, alcohol o sustancias controladas en el autobús Los estudiantes y padres/tutores se hacen responsables de la destrucción maliciosa del autobús, parada de autobús, y propiedad privada. RESPONSABILIDADES DE LOS PADRES/TUTORES: Para garantizar la seguridad de todos los estudiantes en los autobuses, las responsabilidades de los padres/tutores son los siguientes: Padres/tutores son responsables de asegurar que su/s niño/s aprenda (an) a obedecer las reglas de seguridad y comportamiento en los autobuses. Esto incluye transporte de/para su casa y viajes de excursión. Los padres/tutores se asegurarán de que su/s niño/s llegue (en) a la parada de autobús designado 5 cinco minutos antes de la hora de llegada asignada. Notas de padres/tutores pidiendo que sus estudiantes sean recogidos o dejados en una parada que no sea su parada regular no podrán, al menos que sea un caso de emergencia y el niño necesite que una persona diferente lo cuide. Estas peticiones deben estar por escrito, firmadas por los padres/tutores y el Director de su escuela y recibidas por el chofer y el departamento de transporte. Los padres/tutores serán responsables por daños causados por su/s hijo/s en el autobús, lugar de parada de autobús, o la propiedad privada. La restitución puede ser perseguido. Si un padre/tutor tiene una queja acerca de alguna acción disciplinaria con respecto a su hijo, debe notificar al director de la escuela y al departamento de transporte. PLAN DISCIPLINARIO PARA EL AUTOBÚS ESCOLAR: Viajar en autobús a la escuela es un privilegio, no un derecho. Desobedecer cualquiera de las Reglas del autobús, ponen en peligro la seguridad del estudiante y la seguridad de otras personas en el autobús. Es responsabilidad del chofer del autobús escolar el hacer cumplir todas las normas que regulan la conducta de los estudiantes en el autobús escolar. El conductor deberá informar el incumplimiento de las reglas del estudiante al director de la escuela o al personal asignado de acuerdo a la política del distrito escolar local.
