Dear Camp Dream Street Parents:
Registration for Camp Dream Street 2015 is now underway!
This year, Camp Dream Street will take place on Monday, August 24 through Friday, August 28, 2015,
9:30 AM to 3:00 PM. Transportation to Camp will be provided at the following central locations:
Hackensack University Medical Center, Hackensack; St. Joseph’s Hospital, Paterson; and New York
Presbyterian Hospital, New York City. Other bus routes will be developed as needed.
Please complete the Family Registration Form for all your children that will attend camp this summer.
Children between the ages of 4-14 are welcomed to register. Be sure to fill out the form completely,
otherwise registration for your child will be delayed. Your signature is required at the bottom of the form.
Due to our success and growth, we are only accepting oncology patients and their siblings, who are on
active therapy or those who have completed chemotherapy treatment after August 2010. All
children, and their siblings, with chronic hematological diseases, such as sickle cell anemia or hemophilia,
are eligible to attend.
There is also a Medical Form for each child; one for the patient(s) and the other for the sibling(s) of the
patient. Please make sure that you complete the correct medical form for each child. The parent fills out
the first side making sure the authorized section is signed. Please make sure the form is completed,
including immunization records, and signed on both sides before you send it. If you require
additional forms, please call the Camp Dream Street office at 201.408.1455.
Exciting new programs are being planned in order to provide a well-rounded camp week for your
children. Please complete the FAMILY REGISTRATION FORM and return it as soon as possible. This
form must be returned by July 1, 2015. If we do not receive your Family Registration Form by July 1st,
we cannot guarantee that space will be available for your child.
The Medical Form must be completed and returned to Camp Dream Street by July 15, 2015.
NO CHILD WILL BE ALLOWED TO ATTEND CAMP WITHOUT A MEDICAL FORM SIGNED BY A
PARENT AND A PHYSICIAN.

Estimados Padres Participantes del Campamento Dream Street:
La registración para el campamento Dream Street 2015 está por comenzar. Adjunto encontrarán el
Formulario De Registración Familiar, al igual que Formularios Médicos para cada niño/a.
Favor de incluir en el Formulario De Registración Familiar a todos los niños que van a asistir al
campamento. Pueden inscribir a todos sus hijos entre 4 y 14 años de edad. Por lo tanto, deben asegurarse
de tener Formularios Médicos para cada niño. Es decir, uno para el/la paciente y uno para cada hermano/a
del paciente. Favor de asegurarse de llenar el formulario adecuado para cada niño. Si necesitan más
formularios, favor de llamar al Campamento Dream Street al 201.408.1455.
Este año el Campamento Dream Street se llevará a cabo desde el Lunes, 24 de Agosto hasta el Viernes,
28 de Agosto de 2015, 9:30 AM – 3:00 PM. El transporte en autobús será ofrecido en las siguientes
localizaciones centrales: Hackensack University Medical Center, Hackensack; St. Joseph’s Hospital,
Paterson; y New York Presbyterian Hospital, New York City. Otras paradas serán desarrolladas si es
necesario.
Nuevos programas y nuevas aventuras se están programando para proveerle a sus hijos una semana
agradable y divertida. Por favor, llenen la Registración Familiar y devuelvanla lo antes posible. Este
formulario debe ser devuelto antes del 1 de Julio de 2015. Si no recibimos los formularios de
registración antes del 1 de Julio, no le garantizamos que haya espacio para sus hijos.
Los Formularios Médicos deben ser devueltos a Camp Dream Street antes del 15 de Julio de 2015.

FORMULARIO FIRMADO POR UN DOCTOR.
