NOTICE OF CHALLENGE OF APPLICATION
Notice is hereby given that your application for voter registration has been challenged pursuant
to Sec. 13.074 Texas Election Code. The ground(s) for challenge is/are:

_____ You are under 17 years, 10 months of age.
_____ You have been convicted of a felony.
_____ You have been determined by a final judgment of a court exercising probate jurisdiction
to be totally mentally incapacitated or partially mentally incapacitated without the right to
vote.

_____ Your application was submitted in the following unauthorized manner: __________
You are entitled to a hearing on this challenge by submitting a written, signed request for a
hearing to me not later than the 10th day after the date of the challenge. I have enclosed a form
that you may use to request a hearing. If you request a hearing, I will schedule a hearing to be
held not later than the 10th day after your request is filed, or a later date if you so request. You
may appear personally at the hearing or you may submit an affidavit that satisfies the grounds of
the challenge before the hearing begins.

Instruction:
If the person required to sign this document cannot sign his/her name because of physical
disability or illiteracy, he/she must affix his/her mark to the document or paper, and a witness
must attest the mark. If the person cannot make his/her mark, the witness must state that fact on
the document or paper.
Instruction to Applicant:
The request for hearing must be submitted to the voter registrar not later than the 10th day after
the date of the challenge which is noted on the form “Notice of Challenge of Application”. The
voter registrar will mail you a written notice of the date, hour, and place set for the hearing. The
hearing will be held not later than the 10th day after the date the hearing request is filed or made
or at a later date per your request.

Instrucción:
Si la persona requerida para firmar este documento no puede firmar su nombre a causa de una
incapacidad física o analfabetismo, él/ella deberá añadir su marca al documento o papel, y un
testigo deberá atestiguar la marca. Si la persona no puede hacer su marca, el testigo deberá
declarar esto sobre el documento o papel.
Instrucciones Para el Suplicante:
Se deberá presentar la petición para la audiencia al registrador de votantes no más tarde del
décimo día después de la fecha de la recusación que aparece en la forma “Aviso de
Impugnación de Una Solicitud”. El registrador de votantes le enviará por correo un aviso
escrito señalando la fecha, la hora y el lugar designado para la audiencia. La audiencia se
llevará a cabo no más tarde del décimo día después de la fecha en que la misma se registre o se
haga o en una fecha subsecuente que sea a petición del suplicante.
