Parent Letter Regarding Journalism’s
Annual Relationship and Sexual Health Survey

Dear VHS Parents:
I hope you are well. This letter is a follow up of my Wednesday December the 9th edulink message
regarding Journalism’s annual and anonymous Relationship and Sexual Health Survey. Please note that below
the Edulink message transcription is the form you can submit to opt your student out from participating in the
annual survey conducted by the student-run school newspaper.
If you did not receive the Edulink message, the following is what was stated:
Good evening parents! This is Carlos Cohen, principal of Ventura High School with an important message for
you! Our student newspaper, the Cougar Press, will be conducting their annual and anonymous Relationship and
Sexual Health Survey on December 16th during announcement time in second period. You may choose to view
the survey on our website beginning December 10th or view a hard copy here at VHS so that you and your student
can decide on whether to participate in the survey or not. The parent letter and student opt out form will also be
available online so you can print, sign and return to me before December the 16th if you choose the opt out
option. If it is more convenient to you please notify me by calling 641-5116, extension 1012 with your student’s
name and I will insure they have an alternative activity at the time of the survey. Your family values are
important to us, so if you have questions please feel free to call me at Ventura High School. Make it a great

Once again, you may view the survey online or drop by Ventura High School to look at the hard copy
found at the front office desk to help inform your families’ decision. If you feel they should not, please print
this page and sign below indicating you DO NOT want them to participate in the survey. You may drop the optout slip at the front office desk, fax, e-mail me at [email protected] (subject box should say
“Survey,” or call me at (805) 641-5116, Ext. 1012 and I will send a call slip to retrieve your son or daughter from
advisory class so they do not take the survey.

Informe a los Padres en Respecto a la Encuseta de Novios/as y Relaciones Sexuales
de 10 Diciembre, 2015
Estimados padres:
Como les informe el 9 de diciembre, con el mensaje de Edulink, aquí esta la nota si no desea que su alumno
participe en la encuesta anónima anual conducida y presentada por el periódico estudiantil.
Si no recibió el mensaje de Edulink, lo siguiente fue lo que se informo:
Buenas noches, padres! Habla Carlos Cohen, el Director de la prepartoria Ventura con un mensaje importante
para usted. Nuestro periódico estudiantil, Cougar Press, conducirá su encuesta anónima sobre novios y salud
sexual. Usted puede ver esta encuesta vía internet o puede pasar por la escuela si gusta de obtener una copia
de la encuesta para que pueda decidir si le gustaría que su alumno participe. Si no desea que su alumno
participe en esta encuesta, haga favor de notificarme al 641-5116, extensión 1012 y yo me asegurare de que su
alumno tenga una actividad alterna durante el tiempo de la encuesta. También haga favor de firmar la nota y
regresarla a la oficina de Ventura High School antes del 16 de Diciembre. Los resultados anónimos de la
encuesta estarán en la edición de Enero, del periódico estudiantil. Su valores familiares son muy importante
para nosotrs. Si tiene preguntas favor de llamarme aquí a Ventura High School. Gracias, tengan una buena
noche.
Usted puede revisar la encuesta via internet o puede pasar lo escuela para ver una copia y hablar con su alumno
si decide tomar o no tomar la encuesta. Si usted siente que su alumno no debería tomar la encuesta, favor de
firmar la nota y regresarla a la oficina. También puede notificarme a mi correo electrónico, mandándome un
email a [email protected] (subjecto: Encuesta). También puede llamarme a mi teléfono al (805)
641-5116 y yo me asegurare de que su alumno salga de su clase durante segundo periodo para que no tomen la
encuesta.
