Rooted in Relationships Initiative Authorization for Sharing Information: Center‐Based Child Care Provider Dear Child Care Provider, The Rooted in Relationships Initiative has partnered with the University of Nebraska Medical Center/Munroe‐Meyer Institute Interdisciplinary Center for Program Evaluation (ICPE) to conduct an evaluation of the Initiative. As part of the evaluation, you will be asked to complete several surveys about the training you have received and a satisfaction survey about your experiences working with a Pyramid Coach. A trained, outside observer will also do two observations of your classroom using an evaluation tool. Findings from the observations will be shared with you. Your name, observation results and survey information will be entered into and stored in a secure electronic database. A summary of the findings of this evaluation will be shared with Rooted in Relationships Initiative staff, community partners and funders. Your name and individual classroom data and survey responses will be shared only with you and your Pyramid coach. You are not required to participate in these assessments. If you decide not to participate, it will not affect your standing in the Rooted in Relationships Initiative in any way. You can choose to withdraw from the evaluation at any time.
