If program is longer than 180 days

For students studying in Spain whose programs are longer than 6 months (180 days) including Year,
Pre-ILP+ Year, Pre-ILP+Fall, and if student is considering extending from Fall to Spring and has a signed
DPAE form on file with UCEAP:
Required by Government:
An original medical certificate meeting the following requirements:
 It must be printed on the medical facility’s letterhead.
 It must be signed by a physician (MD or DO). Stamped signatures are not acceptable.
 It must bear the official stamp of the administering medical facility in addition to the
doctor’ssignature.
 It must be issued in the place of the student’s residence (i.e., from within the United States).
 It must contain the required text in both English and Spanish, and each version must be signed and
dated by the physician. Required text: This medical certificate attests that Mr. / Ms. [student’s name as
it appears on their passport] does not suffer from any illness that would pose a threat to public health
according to the International Health Regulations of 2005. Signature Date Este certificado médico
acredita que el Sr./Srta [student’s name as it appears on their passport] no padece ninguna de las
enfermedades que pueden tener repercusiones graves a la salud pública, en conformidad con lo
dispuesto en el Reglamento Sanitario Internacional del 2005. Firma Fecha
 The doctor must certify that the student does not suffer from any illness that would pose a threat to
public health according to WHO IHR 2005. This includes, but is not necessarily limited to: Small pox,
SARS, Human Influenza caused by a new subtype, Poliomyelitis due to wildtype poliovirus. Visit the

known risksto public health.
 Any amendment to the certificate or erasure may render it invalid.
 The certificate is valid for three months from the issue date. FYI: Students may be required to get a
medical evaluation after arrival in Spain for visa renewal.
