ZVK Medische uitzending Colombia Medical Transit Colombia Manda kliente Colombia pa risibí kuido médiko PAP Inleiding U wordt door uw arts of medische specialist uitgezonden voor een medische behandeling en/of onderzoek in Colombia. Op het toestemmingsformulier kunt u zien om welke behandeling u naar Colombia gaat. Wij willen u erop wijzen dat het ZVK alleen de kosten vergoedt die te maken hebben met de omschrijving op het toestemmingsformulier. Het is niet de bedoeling dat u zelf ook afspraken maakt met artsen of u laat behandelen in Colombia voor andere aandoeningen of kwalen dan omschreven op het toestemmingsformulier. Een uitzondering hierop is in geval van een spoedsituatie: dat is een situatie waarin u zo ernstig ziek wordt dat de behandeling niet kan wachten tot u weer teruggekeerd bent. Hou rekening dat de mogelijkheid bestaat dat u misschien langer moet blijven in Colombia, neem dus voldoende kleding en toiletspullen mee. NL NL 3 Colombia Colombia ligt in het noordwesten van Zuid-Amerika. Het land grenst in het noordwesten aan Panama, in het oosten aan Venezuela en Brazilië en in het noorden aan de Caribische Zee. Colombia is het vierde grootste land in Zuid-Amerika, met een bevolking van meer dan 46 miljoen inwoners. Ook is Medellin bekend vanwege het Festival van de Bloemen en het Internationale Gedichten Festival. 4 NL NL Aankomst Het is belangrijk dat uw immigratieformulier volledig ingevuld is voordat u in Medellin aankomt. Uw lokale adres in Colombia moet hierbij worden vermeld. Uw lokale adres is het adres van één van de hotels die voor u gereserveerd is in Medellin. U zult deze hotelinformatie van het ZVK ontvangen voordat u naar Colombia vertrekt. Het doel van uw bezoek aan Colombia is medische zorg zoals omschreven in de aanvraag voor uw medische uitzending. Coomeva (een verzekeringsmaatschappij in Colombia) zal voor u zorgen in Colombia. Bij aankomst op de luchthaven staat uw vervoer van de luchthaven naar het hotel al klaar. U kunt de chauffeur herkennen aan een bordje dat hij/zij bij zich hee� met het opschri� Coomeva. Uw taxichauffeur zal u naar het hotel brengen zodat u kunt inchecken. Uw transport van de luchthaven naar het hotel wordt vergoed door het ZVK. U hoe� de taxichauffeur niet te betalen voor deze dienst. Medellin Wat u over Medellin moet weten; De Gemeente Medellin hee� een oppervlakte van 382 km2 en is op één na de grootste stad in Colombia. Bevolking: Meer dan 3 miljoen inwoners. Hoogte: 1475 meter boven de zeespiegel. Temperaturen: Varieert tussen 15 tot 30 ° C. Door haar hoogte boven de zeespiegel en haar bevoorrechte locatie in het Andes gebergte hee� het klimaat in Medellin af en toe meer weg van een vochtig subtropisch klimaat dan van een tropisch klimaat. De medewerkers van Coomeva zijn uw contactpersonen tijdens uw medische uitzending naar Columbia (niet het ZVK). Coomeva werkt nauw samen met het Internationale Bureau van het ziekenhuis. 6 NL NL Dit zijn enkele belangrijke diensten van Coomeva: • Vervoer van de luchthaven naar het hotel en vice versa • Vervoer van hotel naar het ziekenhuis en vice versa • Medewerkers van Coomeva zullen u regelmatig bellen en bezoeken tijdens uw medische uitzending. • Als het geen spoeduitzending betre� dan zal het ZVK zorg dragen voor de betaling van de eerste drie dagen van de dag geldvergoeding. Indien het een spoeduitzending betre� dan zal Coomeva zorgdragen voor betaling van de eerste drie dagen. • Tijdens uw medische uitzending is Coomeva verantwoordelijk voor de betaling van de daggeldvergoeding. Deze vergoeding wordt wekelijks per cheque naar uw hotel verzonden. De Receptionist(e) zal u vertellen waar u de check kunt innen. Internationale bureaus in het ziekenhuis Alle ziekenhuizen die in dit document vermeld worden, hebben een Internationaal Bureau dat diensten aan u, onze klanten, biedt. Het internationaal Bureau werkt nauw samen met Coomeva om u beter van dienst te zijn. Het Internationaal Bureau is gevestigd in het ziekenhuis. We verwachten van de persoon die u vergezeld dat hij/zij gebruik maakt van deze mogelijkheid en dat hij/zij u, waar nodig, bijstaat tijdens uw verblijf in het ziekenhuis. Tijdens dit verblijf dient de persoon die u vergezelt zelf te zorgen voor zijn/haar maaltijden. Hiervoor kan hij/zij terecht in het ziekenhuisrestaurant, waar maaltijden tegen een goede prijs te koop zijn. Ontslag uit het ziekenhuis Als u tijdens uw medische uitzending naar Colombia opgenomen wordt in het ziekenhuis, is het uiterst belangrijk dat u het Internationaal Bureau en Coomeva informeert wanneer u uit het ziekenhuis ontslagen zult worden. Let wel dat er een administratieve procedure aan het ontslag uit het ziekenhuis voorafgaat. Als de administratieve procedure afgerond is, zal het Internationaal Bureau en of de verplegers u informeren wanneer u het ziekenhuis kunt verlaten. Als u niet opgenomen wordt in het ziekenhuis is het heel belangrijk dat u het Internationaal Bureau en Coomeva informeert wanneer uw laatste medische afspraak met de specialist in het ziekenhuis zal plaatsvinden. Nadat u het Internationaal Bureau en Coomeva geïnformeerd hebt, zal het ZVK de handelingen verrichten in verband met uw terugreis naar huis. Slechts nadat het Internationaal Bureau en Coomeva op de hoogte zijn gesteld, kan het ZVK het proces in gang ze�en om uw terugreis naar huis te regelen. NL NL 9 Transport • Uw transport naar en vanuit uw medische instelling zal door Coomeva geregeld worden. • Ook zal uw transport van en naar de luchthaven door Coomeva geregeld worden. U hoe� de taxi-chauffeur voor deze diensten niet te betalen. Nadat de taxichauffeur u naar uw bestemming gebracht hee�, zal de taxichauffeur u vragen om een vervoersformulier voor de verrichte dienst te ondertekenen. • Andere vervoersbehoe�en zijn voor uw eigen rekening. • Als voorzorgmaatregel raden wij aan, als u gebruik maakt van een taxi voor privé doeleinden, dat u slechts taxi’s neemt die door de hotelmedewerkers aanbevolen worden of taxi’s die bekend zijn bij Coomeva. • Bij de receptie van het hotel kunnen zij een taxi voor u regelen. Let wel op dat u in dit geval, zelfs als u gebruik maakt van een taxi die bij Coomeva bekend staat, u voor deze dienst moet betalen. Vergezellen tijdens uw medische uitzending Indien u recht hee� op medische begeleiding dan ligt de verantwoordelijkheid om iemand te vinden die u zal vergezellen bij u, de client. Het ZVK is niet verantwoordelijk voor het vinden van iemand die met u mee zal reizen tijdens uw medische uitzending. Wij verwachten van de persoon die u vergezelt dat hij/zij u tijdens uw medische uitzending onder andere bijstaat bij de volgende zaken: • De cliënt bijstaan bij vervoer en verplaatsingen • De cliënt bijstaan bij het maken van nieuwe afspraken • De cliënt bijstaan bij het invullen van formulieren (op de luchthaven/in het ziekenhuis) • Contactpersoon voor Coomeva en het Internationaal Bureau van het ziekenhuis • De cliënt bijstaan bij het voorbereiden/kopen van maaltijden of bij het halen van boodschappen • De cliënt bijstaan bij het wassen van kleren. • De begeleider hee� geen recht om zich op kosten van ZVK medisch te laten onderzoeken tijdens een medische uitzending. Indien dit toch gebeurt, dan komen de kosten uiteraard voor rekening van de begeleider zelf. Als een begeleider ziek wordt tijdens een medische uitzending, dan worden deze kosten natuurlijk wel vergoed. De bovengenoemde punten zijn voorbeelden en wij verwachten dat de persoon die u vergezelt u, de cliënt, bijstaat als het nodig/relevant is. Indien nodig verwachten we dat de persoon die u vergezelt een maand bij u blij� in Colombia. De nadruk ligt hierbij op ‘indien nodig’. Na een periode 10 NL NL van 1 maand kunt u de persoon die u vergezelt vervangen. De kosten voor deze vervanging komen voor rekening van het ZVK. In het geval dat de persoon die u vergezelt korter dan een maand blij�, is het ZVK niet verantwoordelijk voor de kosten die komen kijken bij het vervangen van de persoon die u vergezelt. In sommige gevallen, a�ankelijk van uw diagnose en behandeling, kan dit ernstige gevolgen hebben voor het succes van uw medische verwijzing. Om die reden is het heel belangrijk dat de cliënt een verantwoordelijke persoon uitzoekt die hem kan vergezellen tijdens de medische uitzending. In sommige gevallen, wanneer we bericht van Coomeva of de client ontvangen dat een begeleider zich niet verantwoordelijk gedraagt, zal ZVK zorgen dat deze persoon bij toekomstige medische uitzendingen niet meer als begeleider kan worden ingezet. Hotels A�ankelijk van het ziekenhuis dat u zult bezoeken voor de medische zorg en de beschikbaarheid van de hotels, zult u in één van de vijf hotels in Medellin verblijven. Een ZVK medewerker zal u voor uw vertrek hierover informeren. Het ontbijt is inbegrepen in uw reservering. Alle vijf hotels hebben een gratis Wi-Fi Internetverbinding. U kunt bij de receptie een gebruikersnaam en wachtwoord aanvragen. Telecommunicatie in Colombia Colombia hee� als landcode 0057. Houd er rekening mee dat mobiel bellen bij sommige prepaid providers niet mogelijk is in Colombia. Houd er ook rekening mee dat roaming met uw mobiele telefoon erg duur is. Als u ervoor kiest om een locale SIM kaart voor uw mobiele telefoon te kopen in Colombia, wees voorzichtig als u deze in uw Smartphone gebruikt. Een aantal cliënten van ons hee� problemen gehad met hun Smartphones toen ze een SIM kaart uit Colombia in gebruik namen. Wij raden u aan om een gewone telefoon te gebruiken voor de Colombiaanse SIM kaart en niet een Smart-phone. Belt u met uw mobiele telefoon met een locale SIM kaart naar een van de eilanden van Caribische Nederland, dan kan de landcode van de eilanden van Caribisch Nederland varieren. Dit is a�ankelijk van het lokale telefoonbedrijf. Medewerkers van Coomeva of het Internationaal Bureau kunnen u vertellen welke landcode u moet toetsen als u naar de eilanden van Caribisch Nederland wilt bellen. U kunt er uiteraard ook voor kiezen om de hoteltelefoon te gebruiken om de nodige telefoontjes te plegen. A�er the administrative process is complete the international office and/or the nurses will inform you when you may leave the hospital. If you are not admi�ed to the hospital, it is very important that you inform the International Office and Coomeva when would be your last medical appointment with the specialist at the hospital. A�er informing the International Office and Coomeva, ZVK would start with the needed arrangements for you to return home. Only a�er the International Office and Coomeva are informed, can ZVK proceed with your arrangements to return to the Dutch Caribbean. 34 ENG Transportation • • • • • Your transportation to and from your medical appointments will be ar ranged for you by Coomeva. Also your transportation to and from the airport will be arranged for you by Coomeva. You do not need to pay the taxi driver for these services. A�er taking you to your destination, the taxi driver will ask you to sign a trans port sheet for the service he rendered.. Other transportation needs are for your own expense. As a precaution we suggest, when using a taxi for your personal need, you only use taxis recommended by the hotel or taxi’s that you are familiar with from Coomeva. The front desk at the hotel can assist you in arranging a taxi. Please take note that in this case, even if you use a Coomeva taxi driver that you will need to pay for this service. Accompany during your medical referral If you are entitled to an accompany, the responsibility of finding a responsible accompany is for you, the client. ZVK is not responsible for finding accompany for you to travel with you during your medical transit. We expect from an accompany that he/she assists you during your medical referral with among other things, the following: • • • • • • • Assist client with their mobility; Assist client with planning of new appointments; Assist client with filling out forms (at airport/hospital); Contact person for Coomeva and the International Office of the hospital; Assist client with preparation/purchasing of meals or purchasing of groceries; Assist client with laundry The accompany has no right to medical examination at the expense of ZVK during a medical transit. If it does occur, then of course, the costs will be for the accompany itself. If an accompany becomes ill during a medical transit, then of course ZVK will reimburse the costs. The abovementioned are examples and we expect accompany to assist the client where needed/if relevant. If needed, we expect your accompany to stay with you one month in Colombia. The emphasis here is if needed. A�er a period of 1 month you can change your ENG 35 accompany. The costs for this change will be for ZVK. If your accompany stays shorter than one month, then ZVK is not responsible for the costs related to change your accompany. In some cases, depending on your diagnose and treatment, this can have serious consequences for the success of your medical referral. Therefore it very important that you the client choose a responsible person for your accompany during your medical referral abroad. In some cases where we receive word from Coomeva or from you the client, that an accompany is not responsible, ZVK will prevent this accompany from being an accompany during future medical referral for clients. Hotels Depending on the hospital you visit for Medical a�ention and the availability of the hotels, you will stay in one of five hotels in Medellin. A ZVK worker will inform you on this before your departure. Breakfast is included in your reservations. All five hotels have a complimentary Wi-Fi internet service. You can ask for a username and password at the reception. Telecommunication in Colombia The country code for Columbia is 0057. Please take note that with your mobile phone some prepaid providers may not work in Colombia. Also keep in mind that roaming with your mobile phone is very expensive. If you choose to purchase a local SIM card in Columbia for your mobile phone, be cautious in doing this for your Smartphone. We have had some clients who have had some problems with their smart phones while using a SIM card from Columbia.
