EGINNING ON AUGUST 1, 2014 if you are enrolled in a Mohawk Industries Medical Plan you
will have a new allowance for CT Scans. If your doctor recommends a CT Scan, you can
ensure the cost is below the allowance by calling Your Choice Advocate at
1-855-733-7295 or visiting www.mycigna.com. Once you have met your deductible, Cigna will
pay 80% in-network up to the allowed amount, and then you will be responsible for the
remaining cost (this remaining cost will not apply to your annual deductible or out-of-pocket
maximum). This allowance is based on the national average cost for CT Scans.

vez que usted alcance su deducible, Cigna pagará el 80% por el procedimiento en la red hasta el
