PRE-KINDERGARTEN AND KINDERGARTEN ENROLLMENT
Enrollment hours are 8 a.m. – 4 p.m. at the Enrollment Center located at 6011 S 89 E Ave., Tulsa, OK.
For online registration, to https://campus.unionps.org/campus/onlinereglogin2_union.jsp
Children who are at least four years old on or before September 1, 2016, may be enrolled in the
Pre-Kindergarten program at their home school on a space-available basis. Pre-registering online does not
guarantee enrollment.
Children who are at least five years old on or before September 1, 2016, are eligible for Kindergarten.
Pre-Kindergarten and Kindergarten are full-day programs, and transportation is not provided for PreKindergarten.
The student must be a resident of Union Public Schools and living with a parent or legal guardian to
enroll. The following items must be provided to the Enrollment Center upon enrollment:
 Child’s certified birth certificate – Hospital certificates cannot be accepted.
 The following immunizations must be current to enroll in Pre-Kindergarten:
4 DPT, 3 Polio, 1 MMR, 3 Hepatitis B, 2 Hepatitis A, 1 Chickenpox
 The following immunizations must be current to enroll in Kindergarten:
5 DPT, 4 Polio, 2 MMR, 3 Hepatitis B, 2 Hepatitis A, 1 Chickenpox
 Proof of residence is required. Acceptable proofs includes two of the following, one must be from
the primary category and all proofs must be in the parent/legal guardian's name.
• The primary proof must be one of the following: Current (within 30 days) gas,
water, electric bill, or signed lease agreement/house contract.
Final bills will not be accepted as proofs of residence.
• A secondary proof of residence may be any of the following: Bank or
credit card statements, cable bill, cellular/home telephone bill, DHS letter, or pay stub.
 Parent/Legal guardian’s ID.
 Custody documentation (if applicable).

Tribal Citizenship Card (if applicable).
 Student’s social security number – Requested but not required.
Please enroll on the designated day for your assigned elementary school based on your home address,
regardless of your preference.

INSCRIPCIONES DE PRE-KINDERGARTEN Y KINDERGARTEN
Las horas de inscripciones son de 8 a.m. – 4 p.m. en el centro de Inscripciones localizado en el
6011 S 89 E Ave., Tulsa, OK

Todo estudiante que haya cumplido 4 años de edad antes o el 1ro de septiembre del 2016, será inscrito en el
Programa de Pre-Kindergarten lo cual es basado en la capacidad de espacio. Las inscripciones en línea no
garantizan un lugar en el programa. Al completar la inscripción en línea tendrán que traer la documentación al
centro de inscripciones.
Todo estudiante que haya cumplido 5 años de edad antes o el 1ro de septiembre del 2016, será inscrito en el
Programa de Kindergarten.
El programa de Pre-Kindergarten y de Kindergarten es todo el día y no existirá ningún tipo de “Transporte” para el
Pre-Kindergarten.
Los estudiantes deben residir en el Distrito de Union, y vivir con sus padres/guardianes. Para completar
las inscripciones, los padres/guardianes deben de traer los siguientes documentos:
 El acta de nacimiento – No se aceptara el certificado del hospital.
 Las siguientes vacunas son requeridas para la inscripción en el Programa de Pre-Kindergarten:
4 DPT, 3 Polio, 1 MMR, 3 Hepatitis B, 2 Hepatitis A, y la Varicela.
 Las siguientes vacunas son requeridas para la inscripción en el Programa de Kindergarten:
5 DTP, 4 Polio, 2 MMR, 3 Hepatitis B, 2 Hepatitis A, y la Varicela.
 Prueba de Domicilio son requeridas. Las pruebas aceptadas incluyen dos de las siguientes, uno deber ser
de la categoría primaria y toda prueba debe de estar al nombre de los padres o guardianes legales.
• Las prueba principal debe ser una de las siguientes: el bill más reciente (no más de 30 días) de la
electricidad, gas, o agua, el contrato de renta firmado.
Los biles finales no serán aceptados como prueba de domicilio.
• La segunda prueba puede ser una de las siguientes: estado de cuenta del banco o de tarjeta de
crédito, el bill del cable, del celular o el teléfono de casa, cartas de DHS y talón de cheque.
 Identificación de padre/guardián legal.
 Documentos de custodia legal (si es aplicable).
 Número de Seguro Social del estudiante. Es requerido pero no necesario.
Los estudiantes serán inscriptos en la escuela que le corresponde de acuerdo a su dirección, aunque esa
no sea su preferencia. Favor de inscribir a su estudiante en la fecha que le corresponde.
