To receive help, You need to complete the following steps:

appointment:

2) Bring the following documents to your appointment:

Number of people being enrolled (whole or partial family), including
the birth date of each person.
Social Security numbers for each family member.
Home Zip Code and proof of address for each member or the family.
(Mail, ID, School correspondence)
Most Recent Income Tax Filing, including dependent tax information
and head of household status (If any) or a Recent Pay Stub.
Legal immigration such as your immigration number. (Residency Card
or Certificate of Naturalization)
Information about your status as a member of federally recognized
tribe (If Applicable).
Don’t Forget:
If you have health insurance that offers the minimum essential benefits
through your employer, or use Medicare or Medi-cal, you should
continue to use those benefits. If you have insurance through your job,
but your spouse or children are not covered through your employer’s
plan, you may be eligible to enroll in Covered California to get
insurance for the rest of your family.

la familia (Tarjeta de Residencia y/o Certificado de Naturalización)
6. Información de su estado como miembro de una tribu de Nativos
Americanos (Si aplica).
No se olvide:
Si usted tiene seguro de salud que ofrece los beneficios esenciales a través
de su empleador, o tiene Medical o Medicare debe continuar usando esos
beneficios. Si usted tiene seguro de salud a través de su empleador pero su
esposo (a) no están cubiertos en el mismo plan usted puede ser elegible
para inscribirse en Covered California y adquirir seguro de salud para el
resto de su familia.
