Traiga el dossier cuando venga a la consulta preoperatoria de
ANESTESIA.

Próximamente usted acudirá a una visita con el anestesiólogo/a
cuyo fin es planificar la anestesia más adecuada para su patología o
estudio diagnóstico.
Para facilitar la evaluación de su estado actual de salud le pedimos
que complete el siguiente cuestionario. Se trata de unas preguntas
sencillas con respuestas tipo SI / NO que debería marcar con un
círculo sobre la que crea más adecuada. En el caso que no sepa
responder, no se preocupe, ya que lo podrá completar en la visita
preoperatoria.
