you are interested, please fill out, detach and return in an envelope marked “PTA – Class Parent Committee” no
later than Friday, September 30, 2016. Two volunteers per class will be chosen.

students together for family-oriented programs and events. Membership dues are $15 for a family and $10.00 for
individual. **This year’s special bonus: receive ONE FREE 25oz Hampton Street School Stainless Steel Water
Bottle, which will be sent home once membership is processed. ** (ONE bottle per household. Additional bottles
may be purchased separately). Classes with 100% participation will receive a prize! With your help the PTA is
able to provide our students with activities and events, like the Welcome Back Picnic, Gingerbread U and Field
Day, just to name a few. Please return dues in an envelope marked “PTA MEMBERSHIP”
