New Member Checklist
Welcome to Prestige Health Choice. This is a list of important things to do as a new member.
Find your ID Card and Provider
Directory.
 Check the Primary Care Physician (PCP)
listed on your card.
Call for an
appointment right away.
 Call Prestige Member Services at
1-888-611-0786 to see if your current
PCP is part of Prestige if you are not
sure.
Do This if Your Current PCP Is Part of
Prestige but Not Assigned to You.
 Call Prestige Member Services at
1-888-611-0786 and ask to change your
PCP.
 If you have not seen your current PCP in
the last year schedule a checkup.
Do This if Your Current PCP Is Not Part of
Prestige.
 If your current PCP is not part of
Prestige ask the Member Services
Representative to help you with finding
a new PCP.
 After you chose your new PCP ask for
help making setting an appointment.
IMPORTANT!
You may not be able to receive care if
you go to a doctor or other health care
provider not in the Prestige network.
Lab Services
(Use Quest Diagnostics only)
 Call 1-800-377-8448 to find a location
for any lab work you need.
Call
1-888-277-8772 to make an appointment
for lab work.
Health Risk Assessment Form
 Look at the Health Risk Assessment
Form in your handbook.
 You have three ways to complete the
form.
o Complete the form from your
Handbook and mail it to Prestige.
Use the prepaid envelope provided.
o Call Prestige Member Services at
1-888-611-0786. Ask to speak to an
Outreach specialist who will record
your health information, give you
needed help, and tell you about
Prestige. You may also set a time
for someone to call you back to do
this.
o Fill out our online form at
www.prestigehealthchoice.com.
Prescription Medicines
 If you have a new doctor make a list of
all your prescription medicines. Take
this list when you see your new doctor
the first time. It is very important if
the prescriptions are from a PCP or
other doctor who is not part of Prestige
 If your doctor is in Prestige take your
new ID Card to any Prestige pharmacy
found in your Provider Directory to get
your medicine.
 If your doctor is not listed in the
Provider Directory, you may get refills
during your first thirty (30) days only.
It is important you do this for every family member who is now part of
Prestige Health Choice.

Haga esto si su Médico de Cabecera actual
participa con Prestige pero no está
asignado a usted.


Llame a Servicios a los Asociados de
Prestige al 1-888-611-0786 y diga que desea
cambiar su Médico de Cabecera.
Si no ha visto a su Médico de Cabecera
actual durante el último año haga una cita
para hacerse un chequeo.
Haga esto si su Médico de Cabecera no
participa con Prestige.



participa

Prestige
dígale
al
Representante de Servicios a los Asociados
que lo ayude a conseguir un nuevo Médico
de Cabecera.
Después que consiga su nuevo Médico de
Cabecera pida ayuda para hacer una cita.
¡IMPORTANTE!
Usted no podrá recibir atención médica si
va a un doctor o a cualquier otro tipo de
proveedor de salud que no participe en la
red de Prestige.
Mire el formulario de Evaluación de Riesgos
de Salud que se encuentra en su manual de
Asociado.
Hay tres maneras de completar el
formulario.
o Llene el formulario y envíela por correo
a Prestige. Use el sobre prepagado.
o Llame a Servicios a los Asociados de
Prestige al 1-888-611-0786, pida hablar
con un Especialista de Ayuda Social. El
Especialista grabará sus datos de salud,
le dará la ayuda que necesita y le
informará sobre Prestige.
También
puede acordar una hora en la que se le
puede llamar para hacer esto.
o Llene el formulario a través del
internet
en
la
página
web
www.prestigehealthchoice.com.
Medicinas Recetadas



Si tiene un nuevo doctor haga una lista de
todas sus medicinas recetadas. Cuando vea
a su nuevo doctor, por primera vez, lleve
la lista de sus medicinas recetadas.
Si su doctor participa con Prestige lleve su
nueva tarjeta de identificación a cualquiera
de las farmacias que aparecen en el
Directorio de Proveedores de Prestige para
obtener sus medicinas.
Si su doctor no aparece en el Directorio de
Proveedores de Prestige usted podrá
rellenar sus medicinas por treinta días
solamente.
Servicios de Laboratorio
(Utilice a Quest Diagnostics solamente)

Llame al 1-800-377-8448 para localizar el
laboratorio cercano a usted. Llame al
1-888-277-8772 para hacer una cita.
Es importante que usted haga esto para todas las personas de su familia que
son ahora Asociados de Prestige Health Choice.
