APPLICANT ACKNOWLEDGEMENT
I hereby acknowledge that I have read, understand and agree to abide by this Independent Distributor
Agreement (front and back). I further agree that I will abide by the Policies and Procedures as stated in
this Agreement and in the Lifestyles Policies and Procedures (as currently published and as same may be
amended and/or updated from time to time in official Lifestyles publications). Further, I expressly agree
that all of the terms and conditions in the Policies and Procedures are herein incorporated by reference
and for all purposes shall constitute part of this Agreement. I agree to indemnify and hold Lifestyles
harmless against any claims, costs, damages, losses, liabilities or expenses (including attorneys fees)
arising from or connected with, directly or indirectly, any breach of this Agreement or other conduct by
me, my agent or employee. I acknowledge that if I breach the terms of this Agreement, Lifestyles shall be
entitled to damages and injunctive relief prohibiting any further violation of this Agreement.
ACUERDO DE CONFORMIDAD DEL SOLICITANTE
Por este medio reconozco haber leído el Contrato de Distribuidor Independiente, el cual entendí y convengo
en respetar (frente y reverso). Además acepto obedecer las Políticas y Procedimientos que se indican en este
Contrato y en el Políticas y Procedimientos de Lifestyles (tanto en su publicación actual, como sus enmiendas
y/o actualizaciones que aparezcan en las publicaciones oficiales de Lifestyles). Asimismo, convengo
expresamente que por medio del presente se incorporan por referencia todos los términos y condiciones
en el Políticas y Procedimientos y que para todo efecto constituyen parte de este Contrato. Convengo en
indemnizar y no hacer responsable a Lifestyles por demandas, costos, daños, pérdidas, responsabilidades o
gastos (incluyendo honorarios de abogados) que puedan surgir, directamente o indirectamente, por algún
incumplimiento de este Contrato u otra conducta mía, de mi agente o de mi empleado. Reconozco que si
violo los términos de este Contrato, Lifestyles tiene derecho al pago por daños y perjuicios, prohibiendo
cualquier violación posterior de este Contrato.

THIS AGREEMENT WITNESSETH THAT:
ESTE CONTRATO INDICA QUE:
1.All Lifestyles U.S.A. Independent Distributors are independent contractors and not employees of the Company.
2.Only authorized Lifestyles U.S.A. Independent Distributors may sell Lifestyles U.S.A. products or participate in the
Lifestyles U.S.A. Sales Program.
3. The only requirement to become a Lifestyles U.S.A. Independent Distributor is the filing of an agreement
together with payment of $40.00 USD Independent Distributor Application Fee. Lifestyles U.S.A. must receive the
fully completed signed application by the participating individual (not the corporation or business name) and
which must not have any deletions, changes or markouts. The Independent Distributor Application Fee entitles
each Independent Distributor to receive periodically the Lifestyles U.S.A. Newsletter. Any purchases of product
inventory, sales aids, literature or supplies is strictly optional.
4.In the case of a partnership or corporation, the person whose signature appears on the application form is
empowered to act for the partnership or corporation. Lifestyles U.S.A. will not grant more than one Independent
Distributorship per individual.
5.This Independent Distributors Agreement shall expire 12 months and 1 day from the date of acceptance
by Lifestyles U.S.A. The agreement may be renewed on each anniversary date for an administration fee of
50% of the current Independent Distributor Application Fee, plus any applicable taxes. Renewals must be
received within 30 days of the expiration of the agreement.
6.Lifestyles U.S.A. Independent Distributors will not make any claims, verbal or written, regarding Lifestyles
U.S.A. products other than those contained in Lifestyles U.S.A. produced or approved materials. Lifestyles
U.S.A. Independent Distributors will also agree that Lifestyles U.S.A. products are not to be sold in any retail
environment and that all advertising must be approved by Lifestyles U.S.A. according to appropriate Lifestyles
U.S.A. Policy Numbers.
7. No individual Lifestyles U.S.A. Independent Distributor may have any ownership interest, operational or
management control of, or derive any benefit directly or indirectly from, any second or subsequent Lifestyles
U.S.A. Independent Distributorship.
8.Each Independent Distributor agrees to abide by the Policies and Procedures for Lifestyles U.S.A. Independent
Distributors as currently published and as amended from time to time in the Independent Distributor Kit or
other Lifestyles U.S.A. literature. The Independent Distributor also agrees to indemnify and hold Lifestyles U.S.A.
harmless against any claims, costs, damages, losses, liabilities or expenses (including lawyer’s fees) arising
from or connected with, directly or indirectly, the Independent Distributor’s breach of this Agreement or other
conduct by the Independent Distributor or by the Independent Distributor’s spouse, agent or employee. The
Independent Distributor acknowledges that if he or she breaches this contract, Lifestyles U.S.A. shall have the
right to terminate the Independent Distributor’s position and shall be entitled to damages and injunctive relief
prohibiting any further violation of this contract.
9.The spouse of an existing Independent Distributor may only be sponsored by that existing Independent
Distributor. This does not change the policy of being sponsored in one position only. A spouse may only be
sponsored by their spouse in their existing downline.
10.These regulations are reasonably related to the laws of the jurisdiction in which the home office of the country
in which the application is submitted is located, and shall be governed in all respects thereby. The parties agree
that jurisdiction and venue shall lie within that location.
11. Georgia residents only:
A.Products, equipment and supplies shall be shipped within seven (7) days from receipt of your order.
B.A participant in this Multilevel Marketing Plan has a right to cancel at any time, regardless of reason.
Cancellation must be submitted in writing to Lifestyles U.S.A., Inc. at its principal business address.
C.In promoting Lifestyles U.S.A. products, Independent Distributors will adhere strictly to the principles and
guidelines outlined in the Independent Distributor Kit as amended from time to time, which is incorporated
herein by reference and made a part hereof.
12.After the initial order, an Independent Distributor may return Lifestyles products, including promotional materials
and sales aids, purchased within the past one year for a refund of 90% of the purchased price (less handling and
delivery charges) if the merchandise is in resalable condition, unless otherwise required by law.

manera empleados de la Compañía.
2.Únicamente los Distribuidores Independientes autorizados por Lifestyles U.S.A. pueden vender los productos de
Lifestyles U.S.A. o participar en el Programa de ventas de Lifestyles U.S.A.
3.El único requisito para convertirse en Distribuidor Independiente de Lifestyles U.S.A. es llenar el contrato y enviarlo
con un pago de $40.00 USD que es la Cuota de solicitud para Distribuidor Independiente. Lifestyles U.S.A. debe
recibir la solicitud llenada que deberá estar firmada por el individuo participante (no el nombre de la corporación
o negocio) y no debe tener borrones, cambios o tachaduras. La Cuota de solicitud de Distribuidor Independiente
le da derecho a cada Distribuidor Independiente a recibir periódicamente la Revista de Lifestyles U.S.A. La compra
de producto para inventario, material de venta, literatura o suministro es estrictamente opcional.
4.En el caso de una sociedad o corporación, la persona cuya firma aparezca en el Formulario de solicitud está
facultado a proceder en nombre de la sociedad o corporación. Lifestyles U.S.A. no concederá más de una
Distribución Independiente por individuo.
5. Este Contrato de Distribuidor Independiente habrá de expirar en 12 meses y 1 día a partir de la fecha de
Aceptación de Lifestyles U.S.A. El contrato podrá ser renovado en cada fecha de aniversario a cambio de una
cuota administrativa equivalente al 50% de la Cuota de Solicitud para Distribuidor Independiente actual, más los
impuestos que apliquen. Las renovaciones deben recibirse a los 30 días del vencimiento del contrato.
6.Los Distribuidores Independientes de Lifestyles U.S.A. no harán afirmaciones, verbales o escritas, respecto a los
productos de Lifestyles U.S.A. excepto por lo que indican los materiales producidos o aprobados por Lifestyles
U.S.A. Los Distribuidores Independientes de Lifestyles U.S.A. también convienen en no vender los productos de
Lifestyles U.S.A. en locales al menudeo y que toda publicidad deberá ser aprobada por Lifestyles U.S.A. conforme
a las politicas de Lifestyles U.S.A.
7.Ningún Distribuidor Independiente de Lifestyles U.S.A. individual podrá tener intereses de propiedad, control
operacional o administrativo ni podrá beneficiarse directa o indirectamente de una segunda Distribución
Independiente de Lifestyles U.S.A. o subsiguiente.
8.Los Distribuidores Independientes convienen en obedecer las Políticas y Procedimientos para los Distribuidores
Independientes de Lifestyles U.S.A. según lo publicado actualmente y según sean cambiadas en el Paquete de
Distribuidor Independiente u otra literatura de Lifestyles U.S.A. El Distribuidor Independiente también conviene
indemnizar y mantener a Lifestyles U.S.A. libre de cualquier responsabilidad contra demandas, costos, daños,
pérdidas, responsabilidades legales o gastos (incluyendo honorarios de abogados) que puedan surgir, directa
o indirectamente, en conexión con la violación por parte de un Distribuidor Independiente de este Contrato o
alguna otra conducta por parte del Distribuidor Independiente o del cónyuge, agente o empleado del mismo.
El Distribuidor Independiente acepta que si viola este contrato, Lifestyles U.S.A. tendrá derecho a terminar la
posición de Distribuidor Independiente y tendrá derecho a daños y desagravios por mandato judicial que le
prohíba cualquier violación posterior de este contrato
9.El cónyuge de un Distribuidor Independiente solamente podrá ser auspiciado por dicho Distribuidor
Independiente. Con esto no se cambia la política de poder ser auspiciado solamente en una posición. Un cónyuge
solamente podrá ser auspiciado por su cónyuge en su propia línea de auspiciados.
10.Este reglamento se relaciona razonablemente con las leyes de jurisdicción en la que se encuentra el Ministerio del
Interior del país en donde se entregó la solicitud y estará gobernada por la misma. Las partes convienen en que
la jurisdicción y sede estará ubicada dentro de esa ubicación.
11. Solamente para residentes de Georgia:
A. Los productos, equipos y suministros deberán enviarse a los siete (7) días de recibir el pedido.
B.Un participante en este Plan de Mercadeo Multinivel tiene derecho a cancelar en cualquier momento, por
la razón que fuere. La cancelación debe presentarse por escrito con Lifestyles U.S.A., Inc. en su domicilio
social principal.
C.Al promover los productos de Lifestyles U.S.A., los Distribuidores Independientes obedecerán estrictamente
los principios y lineamientos incluidos en el Paquete de Distribuidor Independiente y sus cambios, que se
incorporan aquí por referencia y, por lo tanto, son parte de éste.
12.Después del pedido inicial, el Distribuidor Independiente puede devolver los productos de Lifestyles, incluyendo
materiales promocionales y apoyos de venta, comprados durante el año pasado para recibir un reembolso de
90% del precio de compra (menos cargos por manejo y envío) si la mercancía está en condición revendible, a
menos de la ley indique lo contrario.

same may be amended from time to time) for business purposes. I herby acknowledge that I have the right of access
to my Applicant Information to request updates and amendments.

promotional efforts.

of personal information. I further covenant that I will treat all Lifestyles’ Independent Distributor Data Confidentially
and will not discuss nor disclose such confidential information with a third party.
