Pisgah Forest Elementary Chorus Contract for Participation 2015­2016 I have high expectations for our group(s), and would like for the child and his/her main caregiver to review this information carefully together. Initial each line to show you both understand and agree. Child / Parent initials _____ _____ I will commit to making sure I/my child attends as many rehearsals as possible. I agree to try not to schedule doctor and/or dentist appointments, etc. on chorus rehearsal days (but if something unavoidable comes up, please alert Mrs. Smith by email about your child’s absence [email protected] ) I understand that my child may be removed from chorus if he/she misses more than 3 rehearsals. _____ _____ I will commit to picking my child up on time each day chorus meets (chorus ends at 4:00 p.m.). I understand that my child may be removed from chorus if he/she is picked up more than 15 minutes late more than twice. _____ _____ I will help provide my child with a 3­ring binder and pencil used only for chorus and will help make sure he/she has it for every chorus rehearsal. _____ _____ I will encourage my child(ren) to maintain exemplary behavior during rehearsals and performances. I understand he/she may get a formal “warning” for misbehavior by Mrs. Smith and could be removed from chorus after 2 warnings ( I will send an email or speak to the parent directly if a warning is issued). _____ _____ I/My child will support Mrs. Smith in her decisions that best suits the needs of the chorus as well as individual students. I/My child understand that Mrs. Smith’s decisions are deliberate and relevant, and will first assume that a decision was made in good faith. If I have or my child has a concern about a decision Mrs. Smith makes, I agree to schedule an appointment with her to speak privately. _____ _____ I will encourage my child(ren) to practice chorus music outside of rehearsals to ensure success. As the student, I will practice my music outside of rehearsals. _____ _____ I/My child will make sure to attend the performances with the proper clothing and arrive on­time and ready to perform! _____ _____ I will help and encourage my child to remain in chorus for at least one semester to help teach this “long­term” goal. As the student, I agree to stay in chorus at least through the last scheduled performance.
