Year One and Year Two – For 7th – 12th Graders
What Needs To Be Turned In For Each Teen by Thursday, September 22nd, 2016

Completed Medical Release Slip
•
$ 80 annual fee per teen. Late fee of additional $20 per teen if total payment is
not received by December 15th, 2016. No waiver and no exceptions.

For Year One Teen – Copy of Baptismal Certificate
For Year One Teens needing Baptism – Copy of Birth Certificate
•
For Year Two Teen – certificate of successful completion of Year One process
Meeting Dates and Times
•
Year One Meetings are on Tuesdays from 4:00 – 5:30 p.m. in the Parish Office.
•
Year Two Meetings are on Wednesdays from 3:00 – 4:30 p.m. in the Parish
Office.
Requirements
•
Teen needing Baptism must successfully pass Year One to receive Baptism.
•
Teen must successfully pass both Year One and Year Two to receive the
Sacrament of First Communion. First Reconciliation is received during Year Two.
•
Teen must go to Mass each week, go forward to receive a blessing each week,
and have a Mass Card signed by the priest each week. Mass cards must be
brought to class each week to be checked.
•
Teen may not miss more than four classes per year, and every missed class
has a written make-up that must be completed in a timely manner.

Primero y Segundo Año – Jóvenes en el grado escolar 7 – 12
Cada Adolescente necesita entregar lo siguiente, antes de el Jueves, Septiembre

La forma de la información médica completa
•
$ 80 dólares por cada joven por cada año. Se cobrara una cuota de $20 dólares
si no se recibe el pago total antes de Diciembre 15, 2016. Sin excepciones.

Primer Año – Copia del Acta de Bautizo
Primer Año – Copia del act de nacimiento del alumno/a si necesita bautisarse
•
Segundo Año – Certificado de haber completado y pasado el Primer Año
Fechas y Hora de Clases
•
Primer Año - Clases los Martes de 4:00 – 5:30 p.m. en la oficina parroquial.
•
Segundo Año – Clases los Miércoles de 3:00 – 4:30 p.m. en la oficina parroquial.
Requisitos
•
Los jóvenes que necesitan bautisarse tienen que pasar y aprobar el Primer año
para recibir el bautismo.
•
Los jóvenes necesitas pasar y aprobar el primero y segundo año de formación
para poder recibir sus sacramentos.
•
Los jóvenes necesitan ir a Misa todas las semanas y acercarse a recibir la
bendición y llevar su tarjeta de Misa para pedirle al Sacerdote que se las firme
cada semana. Las tarjetas se tinene que llevar a clase cada semana para
verificar
la asistencia a misa.
•
No se puede faltar más de cuatro clases por año. Cada clase que falten tiene
trabajo que necesitan completar y entregar. Completar y entregar en la siguiente
clase.
