Holiday Feast Drawing: Official Rules
1. PRIZE DRAWING. Enter to win a Butterball gift check (Retail value $100.00) by obtaining a quote for automobile
insurance from any Affirmative Retail, Inc. (“Sponsor”) subsidiary, including A-Affordable Insurance Agency, Inc.,
InsureOne Independent Insurance Agency, LLC, Driver’s Choice Insurance Services, LLC, and USAgencies Management
Services, Inc. Contest begins on November 8, 2010 and ends on December 17, 2010. Two prizes will be awarded in each
of Sponsor’s retail districts, for a total of forty-two (42) prizes awarded. Drawings will be held on November 22, 2010
and December 17, 2010 in each of Sponsor’s retail districts. On each drawing date, one winner will be selected at
random from all eligible entries received from November 8, 2010 through the close of business on the day prior to the
drawing in the retail district in which the drawing is held. Odds of winning depend on the number of eligible entries
received in the applicable retail district. Winners will be notified within 48 hours of drawing by phone or email.
2. HOW TO ENTER.
NO PURCHASE IS NECESSARY TO WIN. Holiday Feast Drawing entrants must obtain a quote for
automobile insurance from Sponsor either: (i) in person at one of Sponsor’s retail locations; (ii) by telephone; or
(iii) online through Sponsor’s website (where available). There is no cost to obtain the quote and there is no obligation
to purchase insurance. Purchasing will not increase your odds of winning.
Must be eighteen (18) years or older to enter. Limit one (1) entry per person. Persons

responsibility. No transfer, assignment, cash redemption or substitution of prizes except by Sponsor due to prize
unavailability and then for a prize of equal or greater value. Certain other restrictions may apply. Void where
prohibited. Offer available only to residents of AL, IA, IL, IN, KS, LA, MO, SC, TX, and WI.
4. GENERAL RELEASE AND CONDITIONS OF PARTICIPATION. ALL PRIZES ARE PROVIDED "AS IS" WITHOUT WARRANTY
OF ANY KIND, EITHER EXPRESS OR IMPLIED, AND THE SPONSOR HEREBY DISCLAIMS ALL SUCH WARRANTIES, INCLUDING
BUT NOT LIMITED TO, THE IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE AND/OR
NON-INFRINGEMENT. By participating in the Grand Prize Drawing and redeeming a prize, the winner hereby releases
and agrees to hold the Sponsor harmless from and against any and all costs, injuries, losses or damages of any kind,
including, without limitation, death and bodily injury, due in whole or in part, directly or indirectly, to participation in
any prize-related activities, or the receipt, use or misuse of any prize.
5. DATA COLLECTION. Sponsor collects personal information from you when you enter the Drawing. The information
collected is subject to the Affirmative Insurance Holdings, Inc. Privacy Policy, available at

Sorteo Del Día De Fiesta Del Banquete: Reglas Oficiales
1. SORTEO. Entre para ganar un cheque del regalo de Butterball (valor al por menor $100.00) obteniendo una
cotización para seguro de automóvil de cualquier subsidiario de Affirmative Retail, Inc. (“Patrocinador"),
incluyendo A-Affordable Insurance Agency, Inc., InsureOne Independent Insurance Agency, LLC, Driver’s Choice
Insurance Services, LLC, y USAgencies Management Services, Inc. El concurso comienza el 8 de noviembre de
2010 y termina el 17 de diciembre de 2010. Dos premios se otorgarán en cada uno de los distritos comerciales

son elegibles. El ganador deberá recoger el premio en persona en uno de los distritos comerciales del
Patrocinador. El ganador deberá firmar un Anuncio y Consentimiento que permitirá que el Patrocinador para
tomar y utilizar su fotografía para hacer publicidad como condición para recibir el premio. Los impuestos
federales, estatales y locales son responsabilidad del ganador único. Ninguna transferencia, cesión, reembolso
en efectivo ni sustituir los premios, excepto por el Patrocinador debido a la falta de disponibilidad del premio y
