Dear IWS Children’s Clinic Patient and Family,
Big changes are occurring in health care! And we want you to stay with us!
By now you should have received an enrollment packet from the Illinois Department
of HealthCare and Family Services requiring you to enroll in a health plan.
If you want to continue to see our doctors at the IWS Children’s Clinic you will need to
sign up with either of the following plans:



Family Health Network  is our preferred provider because we feel it
has many benefits for you (like free transportation to medical
appointments).
Blue Cross Community Family Health Plan
Loyola Family Care
You will need to include in a Preferred Care Provider (PCP) number on the enrollment
form, our physician’s PCP numbers are as follows:
