Important News about All Kids

Reminder: Coverage for children in All Kids Premium Levels 3 - 8 will end at midnight
on June 30, 2012
On July 1, 2011, State law required the All Kids program to have an income limit. Starting
July 1 2011, children were no longer able to enroll in All Kids Premium Levels 3 through 8.
Children already enrolled in Premium Levels 3-8 on July 1, 2011 were able to remain in the
program until June 30, 2012 as long as they met other requirements.
As of the date of this notice, you had a child or children enrolled in All Kids Premium Levels
3-8 who could lose coverage after June 30th because of this change in State law. If your
income or family size has changed, your child(ren) may remain eligible for a different All
Kids program.

If your gross monthly income is at or under the amounts shown below for your family
size, please contact All Kids Customer Service toll-free at 1-877-805-5312 (TTY 1-877204-1012) to see if your children could remain eligible for a different State medical
program.

your children may qualify for other insurance.
