1. Print the Nomination Petition form on 8½ X 11 inch paper.
2. The second page (instructions for circulator) must be copied
on the reverse side of the nomination petition.
Instructions to print the PDF document:
NOTE: The following PDF document represents what the Nomination Petition form should look
like. Errors can be made when printing the document, so it is important to make sure you
follow certain guidelines when printing.
1. Click “print” icon from the Adobe Product, not your browser.
2. Check the printer “properties”
3. Paper size - Select “letter” size paper (8.5 x 11)
4. Orientation - Select “landscape”
5. Make sure that “fit to page” is NOT checked
6. When you have printed the petition, make sure that it complies with the form of
petition requirements found in the statute below (verify that line spacing is 1/2”).
16-315. Form of petitions; registration of circulators
A. The nomination petitions shall be in substantially the following form:
1. Petitions shall be on paper eleven inches wide and eight and one-half inches long.
2. Petitions shall be headed by a caption stating the purpose of the petition, followed by the body of the petition stating the intent of the
petitioners.
3. There shall be ten lines spaced one-half of an inch apart and consecutively numbered one through ten.
4. The signature portion of the petition shall be divided into columns headed by the following titles:
(a) Signature.
(b) Printed name.
(c) Actual residence address, description of place of residence or Arizona post office box address, city or town.
(d) Date of signing.
5. A photograph of the candidate may appear on the nomination petition.
B. The following shall appear on the petition:
Instructions for Circulators
1. All petitions shall be signed by circulator.
2. Circulator is not required to be a resident of this state but otherwise must be qualified to register to vote in this state and, if not a
resident of this state, shall register as a circulator with the secretary of state.
3. Circulator's name shall be typed or printed under the circulator's signature.
4. Circulator's actual residence address or, if no street address, a description of residence location shall be included on the petition.
C. The secretary of state shall prepare sample nomination petition forms and distribute such forms to all election officers.
D. Circulators who are not residents of this state must be registered as circulators with the secretary of state before circulating petitions.
The secretary of state shall provide for a method of receiving service of process for those petition circulators who register pursuant to this
subsection. The secretary of state shall establish in the instructions and procedures manual issued pursuant to section 16-452 a procedure
for registering circulators and receiving service of process.

16-315. Forma de las peticiones; registro de las personas circulando las peticiones
A. Las peticiones de nominación deberán hacerse en substancialmente la siguiente forma:
1. Las peticiones deberán imprimirse en papel de once pulgadas de ancho por ocho y media pulgadas de largo.
2. Las peticiones deberán contener un encabezado con el título declarando el propósito de la petición, seguido por el cuerpo de la petición
estipulando el intento de los peticionarios.
3. Deberá haber diez líneas espaciadas por media pulgada y numeradas consecutivamente del uno al diez.
4. La porción de la firma de la petición deberá estar dividida en columnas con los siguientes encabezados:
(a) Firma.
(b) Nombre en letra de molde.
(c) Domicilio de la residencia actual, descripción del lugar de residencia o domicilio de apartado postal en Arizona, ciudad o poblado.
(d) Fecha de la firma.
5. Una fotografía del/la candidato/a puede aparecer en la petición de nominación.
B. Lo siguiente deberá aparecer en la petición:

1. Todas las peticiones deberán ser firmadas por la persona circulándolas.
2. No se requiere que la persona circulando las peticiones sea residente de este estado pero de otra forma debe estar calificada para votar
en este estado, y si no es residente de este estado, deberá registrarse como persona circulando peticiones con la Secretaría de Estado.
3. El nombre de la persona circulando la petición deberá estar impreso o escrito en letra de molde bajo la firma de dicha persona.
4. Se deberá incluir en la petición el domicilio residencial actual de la persona circulándola, si no hay calles en el domicilio, se deberá incluir
en la petición una descripción de la ubicación de la residencia.
C. La Secretaría de Estado deberá preparar formas muestra de la petición de nominación y distribuir dichas formas a todos los oficiales
electorales.
D. Las personas circulando las peticiones que no sean residentes de este estado deberán registrarse como personas circulando peticiones
con la Secretaría de Estado antes de circular las peticiones. La Secretaría de Estado deberá proveer un método de recibir servicio de
proceso para dichas personas circulado peticiones que se registren en conformidad con esta subsección. La Secretaría de Estado deberá
establecer en el manual de instrucciones y procedimientos emitido en conformidad con la sección 16-452 un procedimiento para registrar a
las personas circulando peticiones y un proceso de servicio de recepción.
