accept all of the responsibilities conferred upon me including:
 Actively participating in this program for the duration. (April 18-21st 2016: April 18-20th 9am-3pm, April 21st,
9am-5pm)
 Respecting my cohort facilitator, those around me and all program instructions.
 Following safety guidelines; utilizing tools and equipment in a responsible manner.
 Maintaining perfect attendance.
 Being on time; letting my cohort facilitator know if I will be late.
 Completing all forms accurately and honestly.
 Maintaining and cleaning the program space.
 Evaluating the program through use of surveys and/ or interviews.
 Reviewing the attendance policy.
If I have questions about my responsibilities, I will talk with my cohort facilitator so that I may complete all duties to
the best of my ability.

I understand that I may receive a $100 program completion award after the program based on attendance. I
understand that if I do not maintain 100% attendance throughout the four day span, I will NOT be eligible for this
reward and may be dismissed from the program.

 Estar a tiempo y dejar saber a mi facilitador del cohorte si voy a llegar tarde.
 Llenar y completar todos los formularios y/o solicitudes correctamente.
 Mantener limpio y en buenas condiciones el espacio y/o lugar asignado para el programa.
 Evaluar el programa mediante el uso de encuestas y/o entrevistas.
 Revisar la póliza de asistencia.
Si tengo preguntas o dudas acerca de mis responsabilidades, hablaré con mi facilitador del cohorte y trabajaré para
cumplirlas de la mejor manera posible.

Tengo entendido que puedo recibir un premio de $100 después del programa basado en mi asistencia. También
tengo entendido que si no cumplo con el requisito de 100% de asistencia durante los cuatro días, NO voy
