One Care Presentation Event:

Codman Square Library- Community Room, 690 Washington St, Dorchester, MA 02124
By Public Transit: From Downtown Crossing, take the Orange Line towards Forest Hills. Depart
the Orange Line at Forest Hills and take the 21 bus towards Ashmont. Depart the 21 bus at Morton
and Wildwood, and take the 26 bus towards Gallivan Blvd. Depart the 26 bus at Washington and
Welles Ave. This stop is right outside the library.
By Car:
From the North: Take Route 1 South and use the left two lanes to merge onto Route 93 South.
Take exit 11B to merge onto Granite Ave towards Massachusetts 203/Ashmont. Merge onto Granite
Ave and turn left onto Gallivan Blvd. Then turn right onto Washington St.
From the South: Take 93 North and use exit 11 to merge onto Granite Ave. Turn left onto Gallivan
Blvd, then turn right onto Washington St.
From the West: Take the Mass Pike, I-90 East and use the left two lanes to take exit 24 A-B-C for I93 toward Quincy. Merge onto I-93 South, and take Exit 11B to merge onto Granite Ave toward
Massachusetts 203/Ashmont. Then turn left onto Gallivan Blvd, and turn right onto Washington St.
Parking Options: The Codman Square Library has a free parking lot behind the library and can be
accessed from Welles Ave.

Boston Health Care for the Homeless Program (BHCHP), 780 Albany St., Boston, 02118
By Public Transit: Take the MBTA Silver Line to the Mass Ave Station. There are five regular bus
routes (routes 1, 8, 10, 47 and the Silver Line) and two cross town (CT) express bus routes (Bus CT1
and CT3) that can be used. BHCHP is located at Jean Yawkey Place on Albany Street.
By Car:
From the North: Take Route I-93 South (if coming from Route 1, follow Route 1 South (Via Mystic/
Tobin Bridge) to Route I-93 South). Take exit 18 (Mass Ave). At the traffic light, take a right onto the
access road Mass Ave. Connector. Stay in the right lane. At the end of the access road, turn right
onto Mass Ave. Take an immediate right onto Albany Street. BHCHP will be on your right.
From the South: Take Expressway North (Route 93/3) and take exit 18. At the third traffic light, take
a left onto the access road Mass Ave Connector; stay in the right lane. At the end of the access road,
turn right onto Mass Ave. Take an immediate right onto Albany Street. BHCHP will be on your right.
From the West: Take the Mass Turnpike, Route I-90 East to the end. Then take the Expressway
South (Route I-93). Take exit 18 (Mass Ave). At the traffic light, take a right onto the access road
How to Get There
Mass Ave. Connector; stay in the right lane. At the end of the access road, turn right onto Mass Ave.
and then take an immediate right onto Albany Street. BHCHP will be on your right.
Parking Options: Paid parking is available in the garage at 710 Albany Street. For the first three
hours, the rate is $7.00. For 3-5 hours, the rate goes up to $10.00.

Whittier Street Health Center, 1290 Tremont St, Roxbury, MA 02120
By Public Transit: Take the Orange Line to Roxbury Crossing. Depart the Orange Line at Roxbury
Crossing and take a left onto Tremont St. The Center is located next to the Reggie Lewis Athletic
Center. The following MBTA buses stop near the health center: routes 15, 19, 22, 43, 44, and 45.
By Car:
From the North: Take Route 1 South and use the left two lanes to merge onto I-93 South. Take exit
18 off I-93 South toward Roxbury/Andrew Sq/Mass Ave. Use any lane to turn slightly right onto Mass
Ave. Connector (signs for Mass Ave/Roxbury). Continue onto Melnea Cass Blvd, use the left two
lanes to turn onto Tremont St, and make a U-turn at Prentiss St. The health center will be on your
right.
From the South: Take I-93 N toward Boston and take exit 18 for Frontage Road toward Mass
Ave/Roxbury. Merge onto I-93 Frontage Rd, and use the left two lanes to turn left onto Mass Ave
Connector. Continue onto Melnea Cass Blvd, and use the left two lanes to turn onto Tremont St.
Make a U-turn at Prentiss St and the health center will be on your right. .
From the West: Take the Mass Pike, I-90 towards Boston. Take exit 22 toward the Prudential
Center/Copley Sq. Keep left at the fork and merge onto Huntington Ave, and use the left two lanes to
turn slightly left to stay on Huntington Ave. Then turn left onto Parker St, and another left turn onto
Prentiss St. Then turn left onto Tremont St. The health center will be on your right.
Parking Options: There is on-site parking directly next to the building at a total cost of $5.00.
