Eligibility
Parents/Guardians of age-eligible children must
register online: an email address is required and ability
to scan and upload required documents. Child must be
four on or before September 1st of the current school
year and reside in Florida.

Proof of the Eligible Child Age
Parents/Guardians must provide ONE of the following:
 Certified birth record of certificate
 A passport or a certificate of arrival in the US
showing age
 A valid military ID card
 Certified copy of baptism or religious record
with affidavit, or
 A current shot/immunization indicating the date
of birth, signed by a public health officer or
practicing physician

Proof of Residency in the State of Florida
Parents/Guardians must provide ONE of the following
with name and current address on it:





Florida drivers License
Florida identification card
Current utility bill (within 6 months)
Current pay stub (within 6 months)
Most recent property tax assessment showing
homestead exemption
 Current residential rental agreement or current
receipt from rental payment
 Latest military order showing the child’s parent is a service member in the United States
Armed Forces and is assigned to duty in Florida when the child attends the VPK Program

Elegibilidad
Los padres/guardianes de niños de edad elegible tienen que registrarse vía internet: se necesita un
correo electrónico y capacidad de escanear los siguientes documentos requeridos. Para ser elegible el
niño deber de tener cuatro años en o antes del 1 de
septiembre del año escolar corriente y residir en
Florida.
Prueba de la Edad Del Niño Elegible
Los padres/guardianes deben proporcionar UNO de los
siguientes:
 Certificado de nacimiento gubernamental
 Un pasaporte o un certificado de llegada a los
Estados Unidos que muestre la edad
 Tarjeta de identificación válida como dependiente militar
 Copia certificada de bautismo o registro religioso con declaración jurada
 Registro actualizado de inmunización/vacunas
firmado por funcionario de salud pública o un
médico autorizado
Prueba de Residencia en el Estado de la Florida
Los padres/guardianes deben proporcionar UNO de
los siguientes:
 Licencia de conducir de la Florida
 Tarjeta de identificación de la Florida
 Factura reciente de servicios públicos (no
mas de 6 meses)
 Talonario actual de empleo (no mas de 6 meses)
 Valoración de impuestos de propiedad actual
que muestra exención de vivienda
 Contrato de alquiler o Contrato de arrendamiento o recibo reciente de renta residencial
 Orden militar actual que muestre que el padre
del niño es un miembro de las Fuerzas Armadas de los Estados Unidos y es asignado en
Florida cuando el niño asista al programa de
VPK
Después de aprobación, un Certificado de Elegibilidad
será proveído para entregar al Proveedor de VPK de
