20th Annual North Florida Hispanic Festival 2013
The North Florida Hispanic Association will be hosting its Annual Hispanic Festival on
October 5th, 2013 on the grounds of St. Louis Church located at 3640 Fred George Rd.,
Tallahassee, Florida. Last year’s attendance was tremendous and with the anticipated
advertising we should expect even a bigger crowd attending this year’s festival. We
hope that you will be able to participate as a vendor at the festival.
An application, insurance form and vendor information have been included with this
letter and we are accepting vendor applications. We are offering an early registration
rate of $90.00 if you submit your application and money order prior to September 10.
Applications received after September 10, will require the higher rate of $100.00. We
look forward to hearing from you soon. If you have any questions, please contact me at

La Asociación Hispana del Norte de la Florida será de nuevo anfitrión del FESTIVAL
HISPANO que se celebrará en Octubre 5, 2013 en el terreno de la Iglesia San Louis,
que se encuentra en el 3640 Fred George Road, Tallahasse, FL. La asistencia el año
pasado fue muy numerosa y esperamos que con la campaña publicitaria que tenemos
planeada, asista al Festival una concurrencia aún mayor que la del año pasado.
Confiamos en que podamos contar con usted de nuevo este año con un puesto
representativo de nuestras tradiciones hispanas.
Estamos ofreciendo un precio de $90.00 en el pago de su solicitud si es recibida antes
del 10 de septiembre. Para las solicitudes recibidas después de esta fecha el precio sera
de $100.00. Esperamos tener noticias de usted a la mayor brevedad posible. Favor

WAIVER FORM
The North Florida Hispanic Association does NOT provide liability insurance for the protection of
persons, vendors, organizations, spectators or others who participate in the Hispanic Festival events at
St. Louis Church.
LIABILITY DISCLAIMER AND HOLD HARMLESS AGREEMENT
In consideration of participating in the HISPANIC FESTIVAL at St. Louis Church, I
____________________________________________, an authorized representative of
____________________________________________, do hereby agree to defend, indemnify and
hold, The North Florida Hispanic Association, Inc., St. Louis Catholic Church, Florida Event Planning
& Meeting Service, its sponsors, and Leon County as harmless from and against any claim, demand,
suit, loss, causes of action, damages, liabilities, obligations, costs, expenses, and judgments (including
without limitation, death, and damages to property) caused by participants acts or omissions or failure
to abide by the rules and conditions contained herein.
This release extends to, applies to, covers and includes all known, unknown, disclosed, unforeseen,
unanticipated, and unsuspected injuries, damages, losses or liabilities, and the consequences thereof,
notwithstanding the provisions of any state, federal, local or territorial law or statute to the contrary.
Such provisions are hereby expressly waived.
It is further understood and agreed that my participation in the HISPANIC FESTIVAL at St. Louis
Church is not to be construed as an admission of liability or the assumption of any responsibility by
THE NORTH FLORIDA HISPANIC ASSOCIATION, INC. for the personal acts or omissions by me
or those of my organization.
I hereby agree on behalf of my heirs, executors, administrators, and assigns to indemnify THE
NORTH FLORIDA HISPANIC ASSOCIATION, INC. and its members and officers jointly and
severally and hold harmless from and against any and all actions, claims, demands and liabilities, loss
damages and expense of whatever kind or nature, including attorney fees, which may at any time be
incurred by reason of my participation in the HISPANIC FESTIVAL.
