When asked for names, please print clearly. This will be the way the names will appear on all certificates and official church
records. We must use your full, legal name on all Church documents./ Cuando se le pregunta por los nombres, por favor escriba
claro. Estos nombres son los que aparecerán en los certificados y los documentos ofíciales de la parroquia. Debemos usar su

The Fort Worth Diocese Sacramental Guidelines state that at least one of the parents must be Catholic. The Non-Catholic parent
must agree to raise the child(ren) in the Catholic faith. /La Guía de los Sacramentos de la Diócesis de Fort Worth establece que por
lo menos uno de los padres debe ser católico. El padre no católico debe estar de acuerdo en educar al niño/a en la fe católica.

Non- members of St. Thomas Aquinas must have a Permission letter from the pastor of the parish they are registered

Registration In The Parish/Registro En La Parroquia:
The parents must be registered members of this parish. We must have a completed Parish Registration form on file. Please call the
office and verify that we have you in our records. If you are not a member of St. Thomas Aquinas parish, special written
permission from the pastor of the parish you are registered in must be given and we must have a letter of permission in our file
before the Sacrament of Baptism will be given. This is Cannon law.
Los padres deben ser miembros registrados en la parroquia. Debemos mantener la Forma de Registro Parroquial en nuestros
archivos. Por favor llame a la oficina y verifique que tenemos sus datos en nuestros archivos. Si usted no es un miembro de la

Baptism Preparation Classes/Clases Para Preparación De Bautismo:
Parents and Godparents must attend the Baptism Preparation classes unless permission is given by the Pastor or the Director of
Religious Education. You will receive a Certificate of Completion after you have attended the Baptism classes. The classes are
good for two (2) years
Padres y padrinos deben asistir a las clases de preparación para Bautismo a no ser que haya recibido permiso especial del
Sacerdote o la Directora de Educación Religiosa. Usted recibirá un Certificado de Cumplimiento después de haber atendido las
clases. Estas clases serán validas por tres años.
Si los padres o padrinos no son feligreses de Santo Tomas de Aquino, pueden atender las clases de Preparación de Bautismo en su
parroquia. Después de atender las clases será necesario que envíen una carta a la Oficina de Santo Tomas Aquino declarando que
ya cumplieron con este requisito.
