Los siguientes cargos y gastos serán cargados a la cuenta del cliente en relación con la compra y venta de los contratos de Opciones 1. UNA COMISIÓN DE $75.00 POR CONTRATO DE OPCIÓN 2. UNA COMISIÓN DE $25 A LA PIC POR CONTRATO DE OPCIÓN. TODAS LAS COMISIONES Y HONORARIOS SERÁN COBRADAS ÚNICAMENTE AL MOMENTO DE LA INICIACIÓN DEL CONTRATO. EL CLIENTE RECONOCE Y ESTA DE ACUERDO CON ESTOS COSTOS Y CARGOS. CUALQUIER DISPUTA O PREGUNTA CONCERNIENTE A ESTOS COSTOS Y CARGOS DEBE DE DIRIGIRSE A LA ATENCIÓN DEL IB Y/O PIC POR ESCRITO DENTRO DE LOS DOS (2) DÍAS LABORALES POSTERIORESALAENTREGAAL CLIENTE DE CUALQUIER DECLARACIÓN, INCLUSIVE LOS REFERENTES A CUALQUIERA DE ESTOS CARGOS Y COSTOS.LA OMISIÓN DEL CLIENTE EN DAR NOTIFICACIÓN AL IB/PIC SE JUZGARA COMO LA RATIFICACIÓN DE DICHOS COSTOS Y CARGOS. __________________________________________ Firma de Cliente ________________ Fecha __________________________________________ Firma del Cliente ________________ Fecha 10
