HEALTH PLAN OF NEVADA, INC. (HPN) Selecting and Changing Your Southwest Medical Associates Primary Care Physician As an HPN member, under your HMO Tier 1 level of benefits, you will have access to Primary Care through Southwest Medical Associates (“SMA”) Physicians practicing in the SMA clinics located in our southern Nevada service area. To choose your SMA Primary Care Physician (PCP), PLEASE NOTE the following steps: Choose a specific Physician Code (the number listed next to the physician’s name) from the list of SMA Primary Care physicians included in this brochure. Physicians with 0000-0000 do not hold a panel and cannot be selected. Place the Physician Code in the appropriate space on the enrollment form. You may choose to have a different physician for each member of your family, but you must write the Physician Code by each family member’s name on the enrollment form. Female members, age 14 and over, must also select an obstetrical and gynecological (OB/GYN) provider from one of HPN’s OB/GYN physicians (through either SMA or a listed independently contracted HPN network OB/GYN). Once you choose your SMA PCP, you must seek all care from that physician. He/She will supervise all of your medical care under your HMO level of benefits. If you wish to change your PCP, you must contact the Member Services Department at (702) 242-7300 or (800) 777-1840 TTY at 711. Monday through Friday, 8 a.m. to 5 p.m. If you change your PCP, the change will be effective for treatment received by the new PCP the first day of the following month. Please review your HPN plan benefit materials for requirements regarding HMO prior authorization, covered services, limitations and exclusions. Contact Member Services to confirm your prior authorization status prior to receiving the requested or prescribed covered services. Physician Extenders (eg: P.A., P.A.C., A.P.N., P.N.P.) are licensed as General Practice providers. Although some may have additional training, the specialty they are listed under reflects the licensed specialty of their supervising physician. It’s important to know that when you enroll in an HPN medical benefit plan, covered services are provided through HPN’s delivery system, but the continued participation of any one doctor, hospital or other provider cannot be guaranteed. If specialty care is needed under your HMO tier of benefits, your SMA PCP must refer you to a designated HPN contracted specialist. Failure to follow the requirements of this HMO referral process will result in higher outof-pocket costs to you. Puede seleccionar distintos médicos para cada miembro de su familia, pero debe escribir el Código de Médico junto al nombre de cada miembro en el formulario de inscripción Las mujeres y las muchachas a partir de los 14 años también deben escoger un obstetra/ginecólogo (OB/GYN) de los médicos de OB/GYN de HPN (de SMA o de una de las redes contratadas independientes de HPN). Una vez que escoja su PCP de SMA, debe ir a ese médico siempre que necesite atención. Este médico supervisará toda la atención que reciba bajo su nivel de beneficios de HMO.
