Based on your feedback for enhancing the current Weekly Job Search log, a new log has been developed. This log captures the number of hours and details for each activity for the week. It also instructs the participant to summarize her/his job search efforts in a narrative format. This information will be used to initiate a more in‐depth conversation between the JFES case manager and participant regarding the results of the job search progress. A “Staff Use Only” section has also been added. Staff should record the date the activity hours were entered electronically and initial. All other forms and instructions listed below have been updated for consistency (e.g., CTWorks logo, form numbers, etc.) and have been created as pdf files. As applicable, some forms are “fillable”. JFES staff will now have the option to either manually or electronically fill in certain sections of the forms. These forms and instructions are not available in the Jobs First Employment Services (JFES) Procedures Manual. They will be located on the CTWBS Information Tab shortly. Did participant complete & submit weekly logs? If yes, skip to step #3 below. If no, continue. Yes No 2. If weekly logs are not completed correctly or some (or all) are missing, why? a. Did the participant understand what was expected of her/him? Yes No If yes, continue. If no, clarify instructions for next week. b. Does the participant need help completing them? Yes No If yes, than help her/him complete it and try to determine the cause (low literacy skills, language barrier and communicate this barrier to case manager) may have to continue to help participant complete the form. If no, continue. c. Does the participant just need more time to complete them today? Yes No If yes, allow the participant time in the office to complete the forms and review the benefits of using the forms to manage her/his time and review the benefits of noting the time spent in the activities as she/he goes instead of trying to remember what she/he did later. If no, continue. 3. Did participant do the expected job search activities and specified hours? If yes, done. If no, continue. Yes No 4. If participant did not do the expected job search activities or hours, why not? Yes No a. Did participant understand what was expected of her/him? If yes, continue. If no, review what's expected again. Is there a language or comprehension barrier? Have participant repeat back the instructions. b. Do they have good cause for not completing the assignment? Yes No If yes, And the issue is resolved, review weekly assignment again & clarify what is expected for next week. And the problem is not resolved, determine if participant will need help resolving the issue or will need more time to resolve the issue and communicate this problem to JFES staff. If no, refer for sanction and review weekly assignment again. Clarify what is expected of her/him.
