Did you know that you can now access your family`s immunization

Did you know that you can now access your family’s immunization records online from the
Washington State Department of Health??
Immunization dates are put into the Washington State Immunization Information System by each
person’s medical provider. You may print out a Certificate of Immunization Status (CIS) from this system,
sign and date it, and bring it to school when we need immunization records. We will tell you if the CIS is
complete, or if your student needs additional vaccines (vaccines given out of state will not be listed
unless a Washington State medical provider has put them into the system). To sign up and be able to
print a CIS, go to
http://www.doh.wa.gov/ForPublicHealthandHealthcareProviders/HealthcareProfessionsandFacilities/Da
taReportingandRetrieval/ImmunizationInformationSystem/ForParents and click on ‘Access Your Family’s
Immunization Information’. You will need to complete, and either mail or email their ‘Authorization to
Release Immunization Records’ to the Department of Health. Once this is received, the Department of
Health will issue you a PIN number and instructions on what to do next.
