The goal of this work is to determine the importance of RHOA in CRC. We investigated the impact of genetic (direct sequencing) and epigenetic (CpG methylation analysis) events in the inactivation of RHOA. Isogenic cell systems with either dominant negative (DN) or constitutively active (CA) mutants of RHOA as well as shRNA knock-downs, were also generated and used to study the effects of RHOA in the proliferatory (doubling-time assay), differentiation (assessment of polarization in response to LKB1 activation), migration (wound healing assay) and anchorage independent growth (soft agar assay) capabilities of CRC cells. The “cross-talk” between RHOA and the Wnt signaling pathway was studied with luciferase-reporter plasmids for TCF4 activity and a cell model expressing an inducible DN-TCF4. RHOA expression in CRC cells showed a high variability. Nevertheless, gene mutation or promoter methylation did not account for the observed differences. Cells in which RHOA was inactivated in vitro demonstrated significantly higher migratory and anchorage independent growth capabilities. This is consistent with the observation that RHOA-DN transfectants showed significantly higher levels of Wnt activity, a hallmark of colorectal cancer. Moreover, low RHOA levels were significantly associated with poor tumor differentiation in a series of 137 colorectal cancer patients and in vitro inactivation of RHOA resulted in a significant reduction of markers of enterocytic differentiation. The protein function is unknown. Transgenic animals overexpressing KAP in kidney manifest cardiovascular disorders such as hypertension, glomerulosclerosis, glucosuria and proteinuria. Some of these characteristics fit with signs identified in the metabolic syndrome, the incidence of which is nowadays increasing. Aims: 1. KAP involvement in the metabolic syndrome in transgenic and control mice. 2. Analysis of the results obtained in murine renal cell systems with controlled expression of KAP. 3. Determination of the effects of KAP in several murine and human wt cell systems. Materials and methods: Control and transgenic mice fed on control or high fat diet. “In vivo” measurements: weekly weight of mice, monthly blood pressure, glucose and insulin tolerance tests and lipids and insulin in serum. “Post mortem” analyses: organs weight, histology of kidney, liver and adipose tissue, and expression analysis of selected genes in these tissues. “In cell” analyses: effect of KAP in kidney cells expressing KAP and in murine and human kidney and liver cells treated with prokaryotic recombinant protein in viability, toxicity, oxidative stress and lipid production/intake. Results: The transgene alone does not influence insulin resistance or the onset of obesity; however, by introducing the high-fat diet, the presence of the transgene decreases the harmful effects of this diet. The goal of this study is to find new markers of response to irinotecan for CRC patients to reduce and the risk of a treatment without clinical benefit. Three independent sensitivity tests for irinotecan on 30 CRC cell lines have been performed. Resembling the clinical situation, most of these lines had low sensitivity to irinotecan, and only a few lines showed high sensitivity. mRNA microarray data showed that aprataxin (APTX; a DNA damage repair protein) expression levels correlated with the sensitivity of these 30 CRC cell lines to the irinotecan analogue camptothecin (CPT). To investigate the causal role of Aprataxin in CPT sensitivity, APTX knock out cells showed significantly increased apoptosis levels in response to CPT compared to the APTX wild type cells. Reintroduction of APTX into APTX knock out cells completely restored the CPTresistant phenotype of the parental cells, demonstrating that Aprataxin directly regulates the sensitivity to CPT. Samples of 135 patients treated with irinotecan were used for immunohistochemical analysis of Aprataxin. 51% of these cases showed low/absent aparataxin, whereas 49% showed moderate/high levels. The median progression free survival of patients with moderate/high aprataxin levels was 5.5 months compared to those with absent/low aprataxin levels (9.2 months). The median overall survival of patients with absent/low aprataxin was 17.7 months longer than the patients with moderate/high Aprataxin levels. Objetivos: Dilucidar la estructura tridimensional de la proteína KAP mediante cristalografía y posterior difracción de rayos X. Estudios de interactómica mediante dos técnicas: Pull-down masivo y Tandem Affinity Purification (KAP), a fin de encontrar interactores específicos de KAP. Materiales y métodos: Producción y purificación de KAP: Producción de KAP recombinante utilizando el vector pET14b (sistema His-tag) y posterior purificación en columna de Ni-NTA, intercambio iónico en HPLC y gel filtración HPLC. Cristalización y análisis por rayos X: Cristalización de la proteína KAP mediante sistema de gota colgante y de forma masiva en Crystalfarm (colaboración con CICbioGUNE, Vizcaya). Difracción por rayos X en el sincrotón de Grenoble. Pull-down masivo: Fijación covalente de GST y GST-KAP recombinantes. Elución de las proteínas retenidas en las resinas a partir de extractos crudos de riñón y análisis diferencial en geles 2D de las proteínas eluidas. Identificación por MALDI-TOF de las proteínas diferenciales retenidas en la resina GST-KAP. TAP: Producción de la proteína KAP fusionada a un doble tag de purificación en células de túbulo proximal renal mediante sistema adenoviral. Realizar la doble purificación y analizar las proteínas copurificadas junto con la KAP. Resultados: Obtención de cristales útiles para realizar la difracción de rayos X. Una lista de 26 proteínas resultado del pull-down masivo. Conclusiones: Ya se ha realizado la difracción con rayos X de los cristales de KAP, en espera del análisis de los datos para obtener su estructura 3D. Introducción: La invasión miometrial del cáncer de endometrio (CE) se traduce en un mal factor pronóstico en el desarrollo de esta patología. Se inicia cuando las células tumorales adquieren fenotipo invasivo originado por la expresión de genes relacionados con la remodelación de la matriz extracelular, la motilidad celular y la inducción de la angiogénesis. En este proceso de invasión miometrial, ETV5, factor de transcripción de la familia Ets, promueve la transición epitelio-mesénquima, desregulando proteínas de adhesión celular como las inmunoglobulinas (Ig-CAMs). Las Ig-CAMs son mediadores de la adhesión célula-célula y célula-matriz, interaccionando con otras proteínas de membrana. Objetivo: Determinar el papel del factor de transcripción ETV5 en la regulación de las moléculas de adhesión IgCAMs durante la invasión miometrial del CE. Métodos: El análisis de expresión se ha realizado a partir de microarrays de cDNA, qPCR, inmunobloting e inmunofluorescencia en líneas celulares Hec-1a y su población estable con sobreexpresión de ETV5. Se han identificado las regiones promotoras a partir de Chip-on-Chip e Inmunoprecipitación de cromatina. Results: PVL rats presented high cFos expression in the hypothalamic SON, while capsaicin treated PVL (PVLC) had very low levels, behaving as sham rats. PVLC rats showed increased MAP and SMAR and decreased SMABF compared to PVL and similar to sham. PVLC showed loss of CGRP signal, while nNOS was unaffected by treatment. Expression of Sema3a, NGF/pro-NGF was increased in the SMG of PVL but not in PVLC rats. Sema3A co-localized with cholinergic pre-ganglionic axons. Conclusions: When the afferent signal of PH is abrogated, the brain nuclei are not activated, the neuromodulators are not over-expressed in SMG, and the hemodynamic circulatory alterations and sympathetic atrophy are prevented. Objectives: To investigate the role of the duration of myocardial ischemia on mitochondrial integrity and cardiomyocyte death. Methods and Results: Mitochondrial membrane potential (ΔΨm, JC-1) and MPT (calcein) were studied in cardiomyocytesfrom wild-type and cyclophilin D (CyD)-KO mice refractory to MPT, submitted to ischemia and reperfusion. Reperfusion after 15min ischemia induced a rapid recovery of m, extreme cell shortening (contracture) and mitochondrial calcein release, and CyD ablation did not affect these changes or cell death. However, when reperfusion was performed after 25min ischemia, CyD ablation improved (ΔΨm recovery and reduced calcein release and cell death (57.8±4.9% vs 77.3±4.8%, p<0.01). In a Langendorff system, CyD ablation paradoxically increased infarct size after 30min of ischemia (61.3±6.4% vs 45.3±4.0%, p=0.02) but reduced it when ischemia was prolonged to 60min (52.8±8.1% vs 87.6±3.7%, p<0.01). NMR spectroscopy in rat hearts showed a rapid recovery of phosphocreatine after 30min ischemia followed by a marked decay associated with contracture and LDH release, that were preventable with contractile blockade but not with cyclosporine A. In contrast, after 50min ischemia, phosphocreatine recovery was impaired even with contractile blockade (65.2±4% at 2 min), and cyclosporin A reduced contracture, LDH release and infarct size (52.1±4.2% vs 82.8±3.6%, p<0.01). Conclusions: The duration of ischemia critically determines the importance of MPT on reperfusion injury. Material i mètodes: Homogenados cerebrales humanos de IC, PI y CL de 3 pacientes con IS y 3 controles sanos fueron procesados en geles 2D-DIGE. 132 spots diferencialmente expresados entre zonas (= 1,5veces;p<0,05) se analizaron mediante MALDI-TOFo ESILTQ-OTMS y el algoritmo de búsqueda MASCOT. La validación se realizó mediante ELISAs con muestras sanguíneas de 70 pacientes. Los resultados se relacionaron con la situación neurológica y funcional de los pacientes (mediante las escalas NIHSS y Rankin), empleando el programa estadístico SPSS. Resultats: Se identificaron 51 proteínas, algunas de las cuáles estaban sobreexpresadas o infraexpresadas en el IC. Se seleccionaron 8 proteínas candidatas involucradas en diferentes procesos celulares, según la disponibilidad de ELISA comercial. Tres de ellas no fueron detectadas en las muestras sanguíneas. De las 5 proteínas detectables en sangre, el análisis de regresión logística determinó dos moléculas que se podían asociar independientemente con mal pronóstico de los pacientes al tercer mes tras el IS (OR 10,6;95% IC 2,06-54,85;p=0,005) o con muerte intrahospitalaria (OR 21,3;95% IC 2,1-212,5;p=0,009). Despite many years of research the prognosis of GBM patients has not improved and the median survival is 14 months. It is important to study the genesis and progression of glioma, to improve the treatment against this disease. Glioma Initiating cells (GICs), are a cellular subpopulation that share some characteristics with normal stem cells, as sustained self-renewal capacity and ability to generate secondary tumors that reproduce the characterisics and cellular diversity of the original tumor. GICs are responsible for tumor initiation, propagation, recurrence and resistance to treatments. This suggest that GICs are good therapeutic targets and understanding its biology is crucial to improve anti-cancer treatments. The standard of care for GBM patients is the combination of radiotherapy and chemotherapy but it has been recently described that GICs are resistant to radiotherapy, leading to recurrence in patients after the treatment. In our group, we have recently published that TGFβ pathway is crucial for maintaing GICs subpopulation, and the inhibition using an specific TGFβ-Receptor I inhibitor, results in a decrease in the GICs population within the tumor. The irradiation of cells at 9Gy results in an increase in CD44high/ID1 GICs subpopulation, and this increase in abolished by pre-treating the cells with TGFβRI inhibitor. We are confirming our results in an in vivo mouse xenograft mode. We are studying the molecular mechanisms of this TGFβ-mediated radioresistance in order to improve GBM treatment. Traditional methods of measurement of renal function are not sensitive enough to detect early renal dysfunction that leads to chronic allograft nephropathy (CAN) in renal transplant recipients. Therefore, the aim of this work is to identify differentially secreted proteins by the human proximal tubule-derived cell line, HK-2, under different noxious stimuli (Cyclosporine A, FK506, Rapamycin, staurosporine) that could be potentially used as biomarkers of early CAN. Our hypothesis is that these secreted proteins could be found in urine or blood from transplanted patients and allow early and specific diagnosis of this pathology. Methodology: We first established the toxic and pretoxic doses, at 24h, of each treatment by LDH assay in HK-2 cells. Pretoxic doses were chosen by ANOVA univariant statistical analysis, choosing the one before the first significant toxic dose. HK-2 cells were treated at this pretoxic dose for 24h and cell-free media were collected and concentrated. 60µg of each concentrated medium were used to perform a differential high throughput proteomic analysis obtaining proteomic maps of secreted proteins (secretomes). We determined differential spots using PDQuest software and proteins were identified by mass spectrometry. Detection of Fascin-1 in urine and plasma was done by western blot. Results and conclusions:: Fascin-1, differentially secreted by both CsA and FK506-treated cells, was increased in urine of 60% of kidney transplanted patients and not in healthy controls. Material and methods: We analyzed by gene expression microarray technology those genes whose expression was altered in an OVCA cell line with a stable down-regulation of ETV5. Through the use of IPA software, we identified those canonical pathways, biological processes and gene interaction networks that were most significantly altered. Q-RTPCR, WB, ChIP assay, ROS detection and TGFβ treatments were also performed. Results: New signaling pathways that interact with ETV5 has been unrevealed; among them the cell cycle progression and the TGFβ signaling pathway. We found that the downregulation of ETV5 reduced the expression of the oncogenic transcription factor FoxM1. Consistently, FoxM1 was over-expressed in ovarian tumor samples, and its transcriptional levels increased with ETV5 transcription. Moreover, the FoxM1 expression levels increased with tumor grade. Conclusions: The analysis of the genes and signaling pathways under the control of ETV5 in OV90 cells has unraveled new signaling pathways that interact with ETV5. The survival pathway is linked to the translocation of the transcription factor NF-κB to the nucleus and activation of survival-related genes such as FLIP or IAPs. Inhibition of survival pathways is necessary to sensitize some cell types to apoptosis induced by TNFα. It has been demonstrated that in PC-12 cells, blocking of protein synthesis with Actinomycin D or cycloheximide is sufficient to render cells sensitive to apoptosis induced by TNFα. We have previously shown that this effect is due to the downregulation of the antiapoptotic Bcl-2 family protein Bcl-xL. Here we demonstrate that TNFα triggers ERK1/2 phosphorylation in PC-12 cells. We also show that blocking of NF-kB activation inhibits ERK phosphorylation induced by TNFα and that this effect is recovered by FLIP-L overexpression. Finally we show that inhibition of the MAPK/ERK pathway renders PC-12 cells sensitive to cell death induced by TNFα. In the past decade, the number of published articles related to miRNAs in urologic oncology has increased, especially for PCa, highlighting the ongoing importance of miRNAs in this field. miRNAs have been shown to be dysregulated in the majority of solid tumors. The aim of the present investigation was to standardize the best method of miRNAs extraction and determine the most suitable endogenous gene to identify PCa specific miRNAs expression profiles in urine to a more specific diagnosis. Methods: We compared different RNA extraction methods (phenol-chloroform method and several commercial kits) using urine from patients undergoing prostate massage and analyzed the expression levels of two endogenous genes (miR-130b and RNU6) by RT-qPCR. Results: The miR-130b PCR values (Cp) obtained after the RNA extraction using the QIAmp Viral RNA mini and the miRNeasy mini kits were lower than those obtained by urine specific kits as well as the phenol-chloroform protocol. Conclusion: The commercial kits QIAmp Viral RNA mini and miRNeasy mini are probably the most appropriate methods for the miRNAs extraction in urine samples and, therefore, allow us to study a specific profile of miRNAs for early detection of PCa. Queremos determinar cómo la isquemia cerebral modifica la funcionalidad de las células endoteliales progenitoras (EPCs) y estudiar el papel de la Metaloproteinasa 9 (MMP-9) en su capacidad angiogénica. Material y métodos: Se empleó un modelo de isquemia cerebral permanente mediante oclusión de la arteria cerebral media en ratones deficientes en MMP-9 y sus correspondientes wildtype (WT) y se realizó un grupo control no isquémico. Se realizaron cultivos de EPCs para determinar el número y funcionalidad en ensayos de tubulogénesis e invasión. Resultados: El número de EPCs obtenidas era menor en animales no isquémicos deficientes de MMP-9 comparado con los wild-type. Esta diferencia se mantenia en animales sometidos a 6 horas de isquemia pero los niveles de EPCs eran iguales tras 24 y 72 horas de isquemia entre animales KO y WT. Las EPCs deficientes de MMP9 tenían reducida su capacidad angiogénica en ensayos de tubulogénesis, que pudimos reproducir tratando las EPC de animales WT con un inhibidor de las MMPs. Conclusiones: El ambiente pro-angiogénico generado tras la isquemia cerebral determina el número y función de las EPCs. L’objectiu és analitzar l’eficàcia del TAR en condicions de pràctica clínica i els factors associats al fracàs terapèutic. Pacients i mètodes: Estudi prospectiu d’una cohort d’adults infectats pel VIH, que iniciaren TAR entre gener/2004 i desembre/2009 al nostre hospital. S’avaluà l’eficàcia com percentatge de pacients sense fracàs terapèutic (FT) definit com: fracàs virològic, canvi del tractament, abandonament, pèrdua seguiment, SIDA o mort. Es censuraren canvis de TAR per embaràs, interacció o simplificació. Registre dels efectes adversos que condicionaren retirada de fàrmacs. Avaluació de la supervivència lliure de FT mitjançant corba de Kaplan-Meier, i dels factors associats al FT mitjançant regressió de Cox. Resultats: 232 pacients, 75% homes; edat mitjana 33,5 anys. Transmissió heterosexual (43.5%) o homosexual (38.4%). Mitjana CD4+ 229/mm³. Mitjana ARN-VIH 4.89 log10 (43% >100.000 cop/mL). Un terç coinfectats pel VHC y/o VHB. Parelles d’anàlegs més usades, TDF+FTC/3TC (67%) o ABC+3TC (15%). Tercer fàrmac, EFV (77.22%), NVP (5.1%) o IPs (17%); 2 pacients Trizivir. Percentatge sense FT als 12, 24 i 36 mesos, 82.9% (IC95%, 78-87,8); 78.5% (73-84) i 76% (70.3-81.7), respectivament. Globalment, 57 (25%) pacients presentaren FT. Cap factor s’associà de forma independent al FT. Al final de l’estudi (mitjana de seguiment, 36,5 mesos) el 90% dels pacients tenien CV<50 cop/mL. Methods We have performed a microarray assay comparing Hec1a, a human endometrial cance r cell linegainst its stable population overexpressing ETV5. Statistical mining and literat ure were done to select candidate target genes of ETV5 suggesting a putative role in c ancer invaion. The differential expression of selected genes was validated by RTqPCR and immunobloting. All selected genes were analysed by chromatin immunoprecipitatio n (ChIP) to determine whether ETV5 interact with their promoters. At this moment, stud ies in human samples and functional analysis are being performed. Results The promoters of ten genes were selected for ChIP analysis. Among them, NID1 and N UPR1 were further validated at mRNA level. Aim: Determine the effect of psychosocial and biological factors on clinical features of IBSdiarrhoea (IBS-D). Methods: IBS-D patients and healthy volunteers (H) were included.Participants filled in 3 questionnaires to asses stress and depression levels.Anthropometric parameters and the presence of other diseases were recordered in all participants.The severity of IBS symptoms was quantified with Francis score.Jejunal biopsies were obtained to quantify mucosal microinflammation. Results: A total of 63 IBS-D patients and 48 H were included.IBS-D patients showed significantly higher levels of stress and depression (P<0.05) as well as increased bowel movements and stool consistency when compared to the H group (P<0.05).CD3+ cells, eosinophils and mast cells counts were similar in both groups. Only in the IBS-D group, gender-related differences were detected: women scored higher in the level of depression (median 13 vs. 5) and mucosal eosinophil counts were decreased (median 2 vs. 3.6 cells/hpf; P=0.028),respect to men.Interestingly, Francis score positively correlated with life stress (P<0.05) and depression levels (P<0.001). Este fenotipo se debe en gran parte a alteraciones en las vías apoptóticas, como el silenciamiento de caspasa-8, al que se atribuye la resistencia a la apoptosis inducida por distintos ligandos de muerte. Además, un análisis in silico sobre bases de datos de neuroblastoma, muestra que estos tumores expresan elevados niveles de antagonistas de receptores de muerte como LFG. Nuestros resultados muestran que reestablecer la expresión de caspasa-8 en células SH-SY5Y mediante el tratamiento con IFN-gamma no es suficiente para sensibilizarlas a la apoptosis inducida por TRAIL, aunque sí que expresen sus receptores y estos sean funcionales. Sin embargo, el tratamiento con IFN-gamma sí que restaura la sensibilidad a FasL. Además, si a su vez se disminuyen los niveles de LFG mediante el uso de RNAs de interferencia, se logra una mayor sensibilización a la apoptosis inducida por FasL. Por tanto, la expresión de LFG puede estar otorgando una ventaja a los neuroblastomas de alto riesgo al protegerlos de la apoptosis mediada por el receptor de muerte Fas. Objectius: 1) Comparar la supervivencia dels ratolins injectats. 2) Comparar la migració de les cel·lules transfectades amb GFP-ZAP-70 i amb GFP. Material i Metodes: S'utilitzaren 31 ratolins: 12 injectats amb Raji-GFP, 9 amb Raji-GFP-ZAP-70, 5 amb Ramos-GFP i 5 amb Ramos-GFP-ZAP-70. En el moment d'aparició de símptomes van ser eutanasiats, obtinguts els organs i analitzada la expressió de GFP per citometria de flux. Resultats: 1) No es van observar diferencies significatives en supervivencia. La mitjana en Raji va ser de 16 dies tant per les GFP-ZAP-70 com per les GFP (p=0.658) i per les Ramos de 42 dies per les GFP-ZAP-70 i de 38 dies per les GFP (p=0.42). 2) A la línia Raji es trobaren diferencies estadísticament significatives en la migració cap al mol d'os entre GFP-ZAP-70 i GFP (23.3% vs 2.7, p<0.001), cap a melsa (0.074% vs 0.01%, p=0.001) i timus (0.018% vs 0.0083%, p=0.04). No es trobaren diferencies en la migració amb la línia Ramos. Materials i Mètodes: D’una col·lecció pròpia de 111 soques es van seleccionar 16 clonalment diferents, vuit urinàries (U) i vuit fecals (F). Cadascun d’aquest grups, U i F, estava format per dues soques de cada un dels quatre GF, una típica (A i B1 amb pocs FV i D i B2 amb molts FV) i altra atípica (A i B1 amb molts FV i D i B2 amb pocs FV). Les determinacions dels FV i GF es van realitzar per PCR, la serotipificació per aglutinació i la letalitat intrínseca de les soques en un model murí d’inoculació subcutània. Resultats. Les soques dels GF B1, A típic i D no van causar la mort dels ratolins independentment del nombre de FV, encara que les soques típiques del GF D posseïen un elevat nombre de FV (aquest experiment es va realitzar per triplicat). Les soques atípiques del GF A, fecal i urinària, van causar la mort del 100% i del 50% dels ratolins respectivament. Les soques del GF B2 van causar la mort del 100% dels animals, encara que les atípiques tenien pocs FV. Conclusions: La letalitat va estar més en relació al GF que al nombre de FV. Expression of this fragment leads breast cancer cells to ongogenic-senescence within few days. Our work explains, at least in part, this discrepancy linking senescence to increased metastasis through cell-nonautonomous effects. By using different cell-lines we combined transcriptomic (gene arrays) and proteomic (SILAC and label-free) analysis to characterize, quantify and validate (ELISA) the specific pro-tumorigenic secretome induced by p95HER2 activity in the context of senescent MCF7-cells. Then, using p95HER2-expressing MCF10A, which don’t undergo oncogenic-senscence, we show that the -irradiation-induced senescent state greatly enhances the prosecretory effect of the oncogene. We established an in vivo model to produce senescent xenograft-tumors. We then combined it with two different in vivo models of metastasis based on in vivo bioluminescence imaging and the follow-up of the animals showed how p95HER2expressing senescent MCF7 cells can contribute cell-non-autonomously to the metastatic spread of MDA-MB-231 cells in nude mice. We are currently trying to extend our evidences to the clinic by two different means. On one side by performing correlative IHC and IF on human breast cancer samples and on the other by characterizing an HER2 positive human breast cancer we successfully transplanted into nude/NMRI mice. Material and Methods: We investigated the influence of the anatomy on TL and FL haemodynamics using invitro studies of flexible models of realistic aortic dissections, varying the tear number, size, and location. Velocity and flow patterns were analyzed at the tears and within the lumina. Results: The presence of big tears resulted in slower velocities through the tears complex flows within the FL whereas small tears resulted in higher tear velocities with a more laminar and well defined flow profile. Material y Métodos: Se han analizado 147 tumores primarios de carcinoma de mama obtenidos del Servicio de Anatomía Patológica del HUVH desde 2008-2010. Se ha analizado la expresión de HER3 mediante IHC usando un nuevo anticuerpo generado por A. Pandiella. La expresión de membrana y citoplasma se ha evaluado por separado y se han considerado positivo los casos con un score +3. Las correlaciones con los diferentes parámetros se han realizado usando los análisis estadísticos de Chicuadrado y Kruskal-Wallis. Resultados: Hemos observado una expresión de HER3 en el 21% de los casos. Considerando la tinción de membrana, ésta se encuentra asociada al tamaño tumoral (p= 0.016) y a RE (p= 0.004), mientras que la expresión de citoplasma se encuentra asociada a RE (p= 0.005), RP (p=0.032) e inversamente a Ki67 (p= 0.001). Conclusiones: HER3 se expresa en un 21% de los tumores de mama analizados. Current methods of final diagnosis are based mainly on the pathological examination of a biopsy, which can be either obtained by aspiration or by costly and agressive hysteroscopies. Aim: Our aim is to discover protein biomarkers in the uterine fluids obtained by aspiration to detect EC in order to speed up the current protocol of diagnosis and avoid unnecessary hysteroscopies. Method: Aspirates from the endometrial cavities in the pre-operation period and normal and tumoural tissues from the endometria after hysterectomy were collected. Tissues were immediately frozen at -80°C and aspirates were centrifuged, and separated as supernatant (SN) and pellet. Two different protein extraction strategies were compared by immunoblotting and quantification. Candidate genes were validated by immunoblotting comparing normal and tumoral aspirates. Conclusion: We are able to extract protein from endometrial aspirates and we have proved that endometrial aspirates are surrogates of the molecular alterations that occur in the endometrial tissues. Etiology appears multifactorial, but a genetic contribution has been suggested. Objective: To identify CMI susceptibility genes. Patients and Methods: DNA samples from 416 CMI patients and 524 sex-matched controls were analyzed in a case-control association study evaluating the role of genes involved in paraxial mesoderm development. Morphometric analysis of PCF identified a subgroup 180 patients with small PCF (classical CMI). According to genetic coverage parameters, we selected 384 tagSNPs in 58 genes belonging to FGF8/Wnt/retinoic acid, BMP, Notch, Cdx, Hox and VEGF pathways, plus NF1 and STAT3 genes. Single marker analysis, under the additive model, was performed using the Cochran-Armitage Trend Test. Multiple marker analysis (Haploview) determined transmission disequilibrium within the LD blocks. Results: Single-marker analysis identified nominal associations for 18 SNPs located in 14 genes (P<0.05), none overcoming corrections for multiple comparisons. Five SNPs located in CDX1, FLT1, RARG displayed P-values <0.01. Multiple-marker analysis identified a risk haplotype in the CDX1 gene, P<0.005, OR = 1.36 (95% CI 1.12, 1.66). In the classical CMI subgroup, association with rs887343 in CDX1 remained significant after a 15% FDR correction (P= 4.7e-04). Conclusions: This is the first genetic association study performed in CMI. Our findings support that variants in the genes involved in basichondrocranium development may confer susceptibility to CMI, particularly those with small PCF. Comella & Stéphanie Reix Several neurodegenerative diseases are characterized by loss of neurons, mainly due by apoptosis. In Alzheimer’s disease (AD), there is wide evidence that the accumulation of Aβ deposits causes an inflammatory response in the glia cells, which secrete factors that can determine the fate of the inflammatory response, alteration of synaptic function, and neuronal viability. Death receptor (DR) signalling is highly regulated by several proteins, including intracellular antagonists such as FLIP, FAIM-L and Lifeguard (LFG). Preliminary results from our lab indicate that there is a correlation between neuronal death and the expression of these antagonists. In the transgenic mice model PS1M146LxAPP751SL there is a decrease in the levels of the mRNA for LFG and FAIM-L in the entorhinal cortex and the hippocampus CA1. The temporal pattern for the decrease of FAIM-L and LFG matches with an increase in neuronal death in these areas. This justifies the generation of new transgenic models in which these negative regulators of death are selectively expressed in neurons – by the CamKII promoter and with regulated expression – by the Tet-off System. To do this, we have generated our constructs in an IRES bicistronic vector that expresses EGFP, apart from our protein of interest. We are currently generating the animals for LFG. It has been reported to be caused by mutations in the ATP1A2, SLC2A1 and CACNA1A genes. Significant overlap exists with other paroxysmal syndromes, including early-onset familial hemiplegic migraine (FHM). Objective: To screen an AHC cohort for mutations in the FHM genes ATP1A2, SCN1A, CACNA1A and in SLC2A1 encoding the brain glucose transporter Glut1. Patients and Methods: Twelve patients from Spain and Greece fulfilling the criteria for diagnosis of classical AHC, were included. Genomic DNA was screened for mutations by automated Sanger sequencing after PCR amplification of the exons, intronic flanking regions and 3’UTR regions of all four genes. Gene dosage alterations were ruled out for the CACNA1A gene by means of MLPA and QMPSF. Results: Direct sequencing of the four genes uncovered three CACNA1A variants. The novel variant p.Glu533Lys, probably a pathogenic change, occurred in a pedigree where the proband’s younger brother presented with the phenotype of benign infantile paroxysmal torticollis. The p.Ala454Thr variant was previously reported in familial migraine and found to prevent P/Q channel modulation by SNARE proteins and lower exocytosis. The de novo p.Tyr1387Cys variant caused acute striatal necrosis in a young girl and has been reported in intermediate FHM/AHC phenotypes. No CNVs were detected in CACNA1A. Conclusions: The present results further emphasize the clinical and molecular overlap between AHC and FHM and point to a relatively high incidence of CACNA1A mutations in Southern European AHC cases. 39 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR ABSTRACTS DUE Avaluació de l’eficàcia de la intervenció d’infermeria en la millora del impacte i la qualitat de vida en el malalt diagnosticat de la Síndrome de Fatiga Crónica (SFC) 15.45 – 15.54 h ROSA BADIA ROYES Fatiga Crónica INVESTIGADOR PRINCIPAL: Jose Alegre INVESTIGADOR PARTICIPANTS EN EL PROJECTE: Montserrat LLinas (DI), Mar Sevillano (DI), Montserrat Carballo(DI), Jose Alegre Objectiu: Avaluar la eficiència d’un pla de cures d’infermeria individualitzat, dins d’un marc multidisciplinari que doni suport al malalt diagnosticat de SFC a trobar resposta adaptativa als canvis produïts per la malaltia Material i Mètode: Assaig clínic, prospectiu, aleatoritzat, controlat i paral•lel en una mostra de 55 malalts (30 grup control i 25 grup d’intervenció), diagnosticats de SFC, amb un predomini femení en un 89% de la mostra. Anàlisi de diverses variables sociodemográfiques, característiques clíniques de la malaltia i impacte d’aquesta en la qualitat de vida, a través de la complementació d’escales i qüestionaris: valoració de l’ impacte de la fatiga (escala EMIF, EVA) , qualitat de vida (SF-36),dolor (escala EVA), trastorns de la son (escala de Pittsburgh) i l’ansietat–depressió (escada HAD). Aplicació de pla de cures individualitzat amb Taxonomia NANDA-NIC-NOC amb visites trimestrals periòdiques d’infermeria al llarg d’un any al grup intervenció. Resultats: L’anàlisi de les variables va mostrar una homogeneïtat entre les dues mostres. En el grup d’intervenció s’objectiva una disminució significativa en les puntuacions de intensitat del dolor, fatiga i depressió (p<0.05). En relació als trastorns de la son els ítems com latència, durada i eficiència de la son també presenten una millora significativa (p<0.005). Al valorar els components de qualitat de vida, s’obté una millora de les dimensions de funció física, funció social, rol emocional i salut mental (p<0.01) Conclusions: L’aplicació d’un pla de cures infermer individualitzat, millora molts aspectes de la SFC, repercutint en una millora de la qualitat de vida del pacient. 40 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR Presión positiva continua en la vía aérea con válvula de Boussignac como apoyo en el destete de la ventilación de alta frecuencia oscilatoria. A propósito de un caso. 15.54 – 16.03 h MERCEDES GARRIDO MARTÍN Recerca en cures de salut INVESTIGADOR PRINCIPAL: Mercedes Garrido Martín INVESTIGADOR PARTICIPANTS EN EL PROJECTE: Lidia Pereda Llop, Mercedes Garrido Martín Introducción: La presión positiva continua en la vía aérea (CPAP) es un sistema de ventilación no invasiva y no mecánica que produce un despliegue y reclutamiento de las unidades alveolares parcial o totalmente colapsadas, con una mejora de la compliance, augmento de la capacidad residual funcional pulmonar y como consecuencia la mejora del intercambio gaseoso a nivel alveolar. Además hemodinámicamente, disminuye la precarga y la postcarga (aumentando el gasto cardiaco en pacientes con insuficiencia cardiaca congestiva sin apenas afectar el gasto cardiaco en pacientes sanos). Objetivo: Mostrar los beneficios. Describir los cuidados de enfermería de la CPAP de Boussignac en relación a un paciente con insuficiencia respiratoria ingresado en nuestra unidad. Metodología: Análisis de un caso. Revisión de protocolos de H.U.V., consulta bibliográfica en base de datos (pubMed) y reuniones con personal experto. Resultados y cuidados enfermería: - Explicación de un caso clínico Beneficios de la CPAP de Boussignac: es un sistema fácil de manejar, confortable y reduce los días de ingreso hospitalario, conectando la válvula y la nariz artificial a la cánula traqueal facilita el destete. Disminuye las intubaciones 41 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR orotraqueales y sus complicaciones asociadas. Permite aspirar secreciones y realizar fibrobroncoscopias con aspiración sin modificar la presión final en la vía aérea a pesar del paso de la sonda a través de ella. - Cuidados de enfermería: • Antes de ser colocada la CPAPde Boussignac: explicar al paciente la técnica y la importancia de su colaboración; monitorización de la frecuencia cardiaca y respiratoria, presión arterial, saturación de oxígeno. Colocar al paciente en posición de Fowler. Realizar una gasometría arterial según indicación. • Una vez colocada la CPAP: mantener al paciente vigilado de forma continua (nivel de consciencia, uso musculatura accesoria, frecuencia respiratoria, auscultación, frecuencia y ritmo cardíaco, presión arterial, saturación de oxígeno, diuresis). Realizar una gasometría arterial a los 60 minutos. Controlar la presión del manómetro así como el grado de fuga (evitar oscilaciones de presión mayores a 1 cm H2O tanto en inspiración como espiración). Es necesario un sistema de humidificación activo o en su defecto realizar periodos de descanso, las nebulizaciones se realizaran con el dispositivo diseñado conectándolo al caudalímetro de aire medicinal con una presión no mayor de 6 cm H2O. • Colaboración en los criterios de retirada: progresiva, con periodos de desconexión. Control disnea, uso musculatura accesoria y parámetros gasométricos. Conclusiones: Tras el uso prolongado de la VAFO en un paciente con severa insuficiencia respiratoria, el uso de la CPAP de Boussignac durante 3 días permitió una rápida mejoría de los parámetros clínicos y gasométricos del paciente, pudiendo progresar en el destete de forma precoz y eficaz. La integración de este sistema requiere profesionales con práctica en su manejo y cuidado. ¿Es necesario un cribado nutricional en la planta de oncología de un hospital de tercer nivel? 16.03 – 16.12 h MIGUEL GIRIBÉS VEIGA Unitat de Suport Nutricional INVESTIGADOR PRINCIPAL: Rosa Burgos Peláez INVESTIGADOR PARTICIPANTS EN EL PROJECTE: Ana Sancho, Miguel Giribés, Hegoi Segurola, Victoria Avilés, Gemma Simats, Guillermo Cárdenas, Jose Antonio López, Belen Sarto, Raquel Lorite 42 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR Introducción: La desnutrición hospitalaria afecta a más del 30 % de los pacientes que ingresan en el hospital y esta prevalencia es aún mayor en el paciente oncológico. No obstante el escenario de los paciente ingresados en la planta de oncología ha cambiado, ya que la mayoría de los estudios y tratamientos son ambulatorios y en la actualidad un alto porcentaje de los ingresos son por complicaciones. Por tanto, la detección precoz del riesgo de desnutrición es imprescindible para un correcto abordaje nutricional. Objetivos: Evaluar la detección de desnutrición/riesgo de desnutrición empleando distintos métodos de cribado (Nutritional Risk Screening 2002 (NRS-2002), Malnutrition Screening Tool (MST), Índice de riesgo nutricional (IRN)) y compararla con la Valoración Global Subjetiva (VGS). Material y métodos: Estudio prospectivo observacional de pacientes consecutivos ingresados en el servicio de oncología de un hospital de 3º nivel asistencial. A todos ellos se les realizó en las primeras 48 del ingreso los 3 cribados nutricionales: NRS-2002, MST e IRN. Posteriormente a todos ellos se les realizó una valoración nutricional completa utilizando la VGS. Resultados: Se incluyeron 41 mujeres y 69 hombres (n=110), con una edad media de 64.3 años (r.19-91), todos ellos reclutados en los 3 meses que duró el estudio. Los diagnósticos fueron: 32% neoplasia de pulmón, 14% neoplasia genitourinaria, 14% neoplasia de mama, 11% neoplasia colo-rectal, 11% neoplasia aparato digestivo superior, 6% neoplasia ginecológica, 4% neoplasia cerebrales, 3% neoplasia cabeza y cuello, 3% tumoración neuroendocrina y el 4% otros. Los resultados obtenidos se representan el la siguiente tabla Sin riesgo de desnutrición Desnutrición/Riesgo Sensibilidad NRS-2002 15 95 86% MST 18 92 83% IRN 4 106 96% VGS 0 110 Conclusión: Si bien el IRN es más sensible en la detección de riesgo de desnutrición, no existen diferencias significativas a la hora de comparar los tres métodos de cribado con la VGS. Se confirma que la gran mayoría de los pacientes ingresados en servicios oncológicos presentan riesgo nutricional evaluados por los diferentes métodos de cribado. Por tanto estos paciente requieren de una valoración nutricional completa. En estos paciente el cribado nutricional podría obviarse y debería realizarse directamente una valoración nutricional. Probablemente el cribado nutricional cobre más sentido en los paciente oncológicos que acuden al hospital de día o en las consultas externas. 43 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR Evaluación de la fibrosis hepática mediante elastografía de transición 16.12 – 16.21 h LAURA MILLÁN SEGOVIA Joan Genescà INVESTIGADOR PRINCIPAL: Marisol Ruiz-Meana INVESTIGADOR PARTICIPANTS EN EL PROJECTE: Antonio González, Salvador Augustin, Laia Badia Objetivos: La elastografía hepática transitoria (ET, Fibroscan®) es una técnica no invasiva que permite la estimación del grado fibrosis hepática (FH) mediante la medición de la rigidez hepática. Los objetivos fueron evaluar la tasa de éxito de la ET y conocer los factores relacionados con la FH. Material y Métodos: Se evaluó la elasticidad (kPa) en 240 pacientes ambulatorios con enfermedad hepática crónica asintomática mediante ET. Se recogieron datos epidemiológicos y analíticos. Se clasificaron según la calidad de la elastografía: óptima, subóptima y no válida y el grado de fibrosis: sin/poca (<7kPa), significativa (≥ 7< 12kPa) y avanzada/cirrosis (≥ 12kPa). Resultados: El 53,8% eran mujeres, edad media 58,7±13 años, el 83,3% presentaban infección por VHC. El 84,2% de las determinaciones fueron óptimas, 7,5% subóptimas y 8,3% no válidas. La baja calidad de la ET se relacionó de forma significativa con el IMC (p<0,001) y la edad (p=0,02). El 39,5% de los pacientes no tenían fibrosis (5,3±0,9kPa), el 34,1% significativa (8,9±1,3kPa) y el 26,4% avanzada (23,5±12,6kPa). En el análisis multivariado la rigidez hepática se correlacionó directamente con la edad, IMC y niveles de ALT e indirectamente con las plaquetas (p<0,01). Un IMC elevado (26±3), edad avanzada (63,5±10), sexo masculino (59%), plaquetopenia (130774±76500) y ALT elevada (115±108) se relacionaron significativamente con la fibrosis avanzada (p<0,05). Conclusiones: La rigidez hepática depende de la edad y de la obesidad, además de parámetros de función y actividad hepática. La razón principal para no poder interpretar la rigidez hepática es la obesidad. 44 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR Factores involucrados en la alteración del sueño en usuarios de una unidad de quemados 16.21 – 16.30 h YOLANDA PARRILLA MONTERO Recerca en cures de salut INVESTIGADOR PRINCIPAL: Blanca Vilanova Draper INVESTIGADOR PARTICIPANTS EN EL PROJECTE: Yolanda Parrilla Montero, Blanca Vilanova Draper Objetivo Principal: Detectar si los usuarios mantienen un buen descanso nocturno. Detectar factores involucrados en la alteración del sueño. Objetivos Específicos: Detectar si existe alguna relación entre la alteración del patrón del sueño y la edad o el sexo. Observar si existe relación o causa de insomnio entre los pacientes que duermen con la puerta abierta o cerrada. Material y métodos: Estudio observacional transversal descriptivo. Instrumento de medida: escala elaborada por la investigadora adaptada a la escala estandarizada Likert. Población: pacientes ingresados en la unidad de quemados durante el periodo de estudio. Se obtuvo la muestra mediante muestreo no probabilístico accidental.Tras aplicar los criterios de inclusión-exclusión. 51 sujetos a estudio. Resultados: Representación población a estudio: variable sexo, 63% hombres frente al 37% mujeres. Edad media 47-56 años. Duermen mal 65% frente a un 35% que manifiestan dormir bién. La actividad Sanitaria 87% y la estimulación ambiental 78%, son las causas de insomnio. Relación causa efecto puerta abierta/cerrada-dormir bien/mal 76% puerta abierta, 24% puerta cerrada. Conclusiones: Algunas de las actuaciones de enfermería para conseguir unas condiciones ambientales y personales que ayuden a dormir deberían dirigirse hacia: disponer de un entorno agradable, proporcionar silencio, anticiparse a las necesidades, prescindir de procedimientos no esenciales durante la noche, recomendar métodos para conseguir la relajación total y mantener los hábitos de la persona que preceden al sueño. 45 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR Estudio de prevalencia de hipertensión arterial, diabetes y dislipemia en pacientes adultos con enfermedad renal crónica 16.30 – 16.39 h JESÚS ANDRÉS CARBÓ Unitat d’Hemodiàlisi INVESTIGADOR PRINCIPAL: Jesús Andrés Pérez Carbó INVESTIGADOR PARTICIPANTS EN EL PROJECTE: Jesús Andrés Pérez Carbó, Eva Castillo Hernández, Maria Mercè Piñol Fernández, Carol Veronica Veiras Silveira, Maria José ludeña Garcia Estudio de prevalencia de hipertensión arterial, diabetes y dislipemia en pacientes adultos con enfermedad renal crónica Objetivos: Analizar la prevalencia de hipertesión arterial, diabetes y dislipemia. Determinar el número de factores de riesgo que padecen los pacientes del estudio. Material y método: Estudio descriptivo, longitudinal de cohorte. Se trata de un muestreo no probabilístico de conveniencia. Sólo se han estudiado aquellos pacientes adultos con insuficiencia renal crónica hospitalizados en la unidad de nefrología del Hospital General de la Vall d’Hebrón, durante los meses de junio y julio del 2011. Resultados: De los 30 pacientes estudiados, el 73,33% presenta hipertensión arterial, el 53,33% diabetes y el 36,66% dislipemia . El 20% presenta 3 factores de riesgo asociados, el 33,33% 2 y el 46’66% presenta 1 factor asociado a la insuficiencia renal crónica. Conclusiones: Todos los pacientes estudiados presentan alguno de los factores asociados a la insuficiencia renal crónica. Determinar si se consigue el peso seco del paciente. Valorar si existen complicaciones interdiálisis. Material y método: Se trata de un estudio descriptivo, longitudinal de cohorte. Los sujetos de estudio, han sido todos aquellos pacientes adultos,con insuficiencia renal crónica de 3 o más meses de evolución en tratamiento sustitutivo renal de hemodiálisis ≥ a 4h con serologías positivas, dializados durante el mes de agosto de 2011,en la unidad de hemodiálisis del Hospital General de la Vall d’Hebrón. Resultados: El 25% de la muestra presenta un peso medio prediálisis entre 1kg a 1,5kg. El 50% presenta un peso medio prediálisis entre 1,6kg a 2,5kg.El 17% presenta un peso medio prediálisis entre 2,6kg a 3,5kg. El 8% presenta un peso mayor a 3,5kg. Conclusiones: Según este estudio, podemos afirmar que la mayoría de los pacientes con enfermedad renal crónica dializados en la sala B de la unidad de hemodiálisis del Hospital General de la Vall d’Hebrón, presentan un adecuado peso interdiálisis, lo que facilita poder obtener el peso seco del paciente al finalizar la sesión. 47 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR Intervención nutricional en pacientes con Enfermedad Inflamatoria Intestinal (EII) en brote agudo 16.48 – 16.57 h ANA SANCHO GOMEZ Unitat de Suport Nutricional INVESTIGADOR PRINCIPAL: Rosa Burgos Peláez INVESTIGADOR PARTICIPANTS EN EL PROJECTE: Cleofé Perez-Portabella, M. Segurola, Belen Sarto,Raquel Lorite, Jose Antonio López , Ana Sancho, Victoria Avilés, Guillermo Cárdenas, Rosa Burgos Introducción: Los pacientes con EII presentan elevado riesgo de malnutrición durante el brote agudo de la enfermedad. Es necesaria la intervención nutricional con el objetivo de evitar la malnutrición y sus consecuencias negativas en la evolución clínica de la enfermedad. En esta situación, la nutrición enteral (NE) es el tratamiento nutricional de elección. Objetivos: Valorar la evolución del estado nutricional al ingreso y al alta hospitalaria, tras la intervención nutricional con un protocolo de nutrición enteral vía oral. Material y métodos: Pacientes ingresados en hospital de tercer nivel por brote de EII. Cálculo necesidades energéticas (30/kg/día) y proteicas al ingreso (1,2g/kg/día). Valoración nutricional ingreso/alta según índice de riesgo nutricional (IRN), albúmina, %pérdida de peso (%PP), %peso habitual (%PH) , proteínas totales e índice de masa corporal (IMC). Marcadores inflamatorios: PCR/ VSG. La nutrición enteral oral con fórmula completa hipercalórica e hiperproteica sin fibra ni lactosa se inició el primer día del ingreso junto con dieta líquida complementaria (caldos e infusiones), siguiendo una pauta progresiva con objetivo de alcanzar los requerimientos nutricionales al tercer día (30% de las necesidades el primer día, 65% segundo día y 100% tercer día.) Tras la remisión del brote agudo, se retiró la nutrición enteral oral progresivamente hasta cubrir los requerimientos mediante dieta oral completa (Dieta sin residuos y sin lactosa) Estudio estadístico: SPSS V.16 Resultados: 31 pacientes (16 colitis ulcerosa; 15 Crohn); 20 mujeres; Edad=40±14,1; Estancia media= 13días±7. Material y Métodos: Pacientes consecutivos con función hepática conservada asintomáticos clasificados en 3 grupos: plaquetas ≥150000 y ecografía normal (Grupo A), plaquetas <150000 y ecografía normal (Grupo B) y plaquetas <150000 y ecografía anormal con esplenomegalia leve (13-15 cm) y/o hígado nodular. Los pacientes con una rigidez hepática ≥14 kPa fueron evaluados por gastroscopia y se midió el gradiente de presión venosa hepática (GPVH). Resultados: Se evaluaron 232 pacientes (56% mujeres, edad media 57±14, 84,5% hepatitis C): 70% grupo A, 20% grupo B y 10% grupo C. No se obtuvo valor de elastografía en 19 (8%). Se detectó una rigidez ≥14 kPa en 50 pacientes (23%): 9% grupo A, 44% grupo B y 78% grupo C (p<0,001). Se practicó gastroscopia en 39 pacientes: el 82% no tenían varices esofágicas y el 18% restante varices pequeñas (grado 1); 5 de los 7 pacientes con grado 1 eran del grupo C. El GPVH se obtuvo en 34 pacientes: el 91% tenían hipertensión portal (GPVH >5 mmHg) y el 65% hipertensión portal significativa (GPVH ≥10 mmHg). Todos los pacientes de los grupos B y C tenían hipertensión portal. Los recientes desarrollos técnicos han resultado en una disminución del riesgo, proponiendo incluso dicho procedimiento en régimen ambulatorio. El objetivo del estudio fue evaluar la incidencia y el momento de presentación de la hipotensión en pacientes sometidos a SC y su correlación con la aparición de complicaciones postoperatorias. Material y Métodos: Estudio retrospectivo en 49 pacientes monitorizados durante 12 horas después del procedimiento. Se registró la incidencia de hipotensión (TAS<90mmhg) y la necesidad de vasopresores. Los pacientes de dividieron en 2 grupos de acuerdo con el momento de presentación de la hipotensión (≤6 hs o >6 hs) evaluando la aparición de complicaciones cerebrovasculares durante su estancia hospitalaria realizando seguimiento al mes, 6 y 12 meses. Para el análisis estadístico se utilizó χ2 resultados en media±DE. Resultados: 18 pacientes (36,8%) presentaron hipotensión, requiriendo drogas vasoactivas en el 64,3% de ellos. La hipotensión se presentó de manera precoz (< 6 hs) en 14 pacientes (77,8%) y tardía en 4 pacientes (22,2%). No hubo diferencias en la incidencia de complicaciones cerebrovasculares. Al año del SC, se observó trombosis (>60%) en 12% de los pacientes y la mortalidad fue del 2%. Sin embargo, en un 40% de los casos resulta inefectiva. El mecanismo por el que resulta inefectiva no es conocido. Objetivo: Analizar las diferencias en el contenido de factores trombóticos y fibrinolíticos en el trombo de pacientes resistentes a la lisis, respecto al trombo de pacientes sensibles. Métodos y Resultados: Se incluyeron 20 pacientes con un primer IAMEST sometidos a ICP. De éstos, 10 fueron sometidos a ICP primaria y 10 a ICP de rescate por ineficacia de la fibrinólisis. De todos los pacientes, se obtuvo un trombo coronario mediante un sistema de aspiración Pronto V3®. Se realizó un análisis inmunohistoquímico del trombo utilizando diferentes anticuerpos monoclonales. Además se determinaron los niveles de fibrina, CD34, Factor VonWillebrand, inhibidor plasmina activada, P-selectina, activador de la plasmina tisular por inmunofluorescencia. No hubo diferencias en la edad, sexo, factores de riesgo cardiovascular, tiempo desde inicio de síntomas-balón, localización del infarto ni número de vasos afectados. Los pacientes sometidos a ICP de rescate mostraron niveles significativamente superiores de fibrina, P-selectina y FVW que los pacientes sometidos a ICP primaria. We examined whether guiding fluid therapy by an algorithm based on optimizing the global end-diastolic volume index (GEDI) and the extravascular lung water index (EVLWI) could reduce fluid balance and the need of mechanical ventilation in surgical patients with septic shock. Methods: Forty-two surgical patients were included prospectively and compared with a control group. In the goal-directed therapy (GDT) group fluid management was guided during the first 48 hours after ICU admission by an algorithm based on GEDI ( 700ml/m2 or 700mL/m2) and EVLWI (<10mL/kg or 10mL/kg); the control group was treated based on central venous pressure, mean arterial pressure and clinical evaluation. Results: The two groups did not differ significantly in demographic or severity of illness and organ dysfunction. Patients of the GDT group received less intravenous fluids and fluid balance at 48 hours after admission was lower (1911±1449 mL vs. 2690±1697, mean ± SD; p=0.03). Duration of mechanical ventilation was shorter in the GDT group (96 [120] vs. 188[255] hrs., median [IR]; p=0.001). No differences were found in ICU stay, hospital stay and neither in hospital mortality between groups. Se sabe muy poco del papel del citoesqueleto de microtúbulos (MTs) en estas circunstancias, pero se ha descrito que es degradado en fases tempranas de isquemia. Objetivo: Iniciar la caracterización de la posible implicación de los microtúbulos en el daño por reperfusión en un modelo de cardiomiocito aislado. Material y Métodos: Cardiomiocitos aislados de rata. Técnicas de imagen e inmunocitoquímica para valorar cambios de la red de MTs en un modelo de hipoxia. Medidas de tracción celular mediante microscopía de tracción y detección de cambios moleculares por micro-espectroscopía Raman. Resultados: Cardiomiocitos expuestos a hipoxia desarrollan rigor y disrupción severa de la red de MTs, la ruptura es parcialmente inhibida en presencia de Taxol, tratamiento con el que la tasa de hipercontractura disminuye un 20%. Los espectros Raman sugieren que el grado de polimerización de la red de MTs modifica significativamente la interacción general inter-proteinas y/o de éstas con el medio. Se estudiaron siete aislados de E. cloacae (UCI/área General y Traumatología) y siete de K. pneumoniae (Unidad de Lesionados Medulares). Se identificaron los genes de resistencia por PCR y secuenciación. La clonalidad de las cepas se estudió mediante XbaI-PFGE y MLST. Los plásmidos se caracterizaron por métodos de PCR-based replicon typing, S1-PFGE, Southern blot y conjugación. El tipado plasmídico se realizó mediante el método de doble locus y de secuenciación de la relaxasa MOB. En todas las cepas se detectaron los genes blaVIM-1, aac(6’)-Ib, qnrA y blaCTX-M9(excepto en una). Los aislados de E. cloacae se distribuyeron en tres grupos clonales presentando por PFGE menos de 70% de similitud entre ellos, mientras que todos los aislados de K. pneumoniae pertenecieron a un único grupo clonal determinado por MLST i.e ST252. Todos los aislados contenían un plásmido conjugativo IncHI2 MOBH11 portador de todos los genes de resistencia detectados. Estos plásmidos pertenecían al mismo grupo ST1-IncHI2. En conclusión, este estudio apoya otros trabajos realizados en España indicando un riesgo de endemia de cepas productoras de VIM-1. El análisis plásmidico apoya la hipótesis de la transmisión horizontal entre diferentes cepas y especies del plásmido IncHI2 portador de los genes de resistencia. Esos datos reflejan como la transmisión horizontal de un plásmido simultánea a la difusión clonal de ciertas cepas puede potenciar la difusión epidémica de un gen de resistencia. Epithelial-mesenchymal-transition (EMT) was characterized on ETV5 overexpressing cells using microarray data, expression assays, chromatin immunoprecipitation, focal adhesion turnover analysis, adhesion, migration and invasion assays. In human samples, ETV5 and LPP were examined at mRNA and protein level. Regarding LPP, we correlate ETV5 and LPP expression and localization in EC samples and in vitro, LPP relocalizes to focal adhesions upon ETV5 overexpression, which we associate with a probable function of LPP as an extracellular sensor to support ETV5 promoted invasion. Conclusions: We have widely characterized the role of ETV5 on EMT during the initial steps of tumor invasion in EC. In addition, we have identified LPP as a novel protein involved in the EC invasive process correlating with ETV5 expression. Material y métodos: Para inducir la EAE, se inmunizaron ratones de cepas resistentes y susceptibles a desarrollar la enfermedad con PLP139-151. Los animales se sacrificaron a día 12 post-inmunización. Se obtuvo suero, líquido cefalorraquídeo (LCR) y esplenocitos para estudiar la respuesta inmune humoral y celular. Resultados: No se hallaron diferencias en las poblaciones de células B y T reguladoras. El porcentaje de células B T-dependientes (B220+CD5-) (media±SEM: 51,4±7,3 vs. 31,0±6,1; p=0,05), células T naïve (CD4+CD25-) (29,3±4,4 vs. 15,1±1,1; p=0,05) y células T activadas (CD4+CD25+) (3,4±0,7 vs. 1,9±0,1; p=0,05) era superior en la cepa susceptible, en la que también se observó un aumento de la producción de IL-17 (123,6±21,7 vs. 7,0±7,0 pg/ml; p=0,046). Al estudiar la respuesta humoral se observaron niveles más elevados de anticuerpos anti-PLP139-151 IgM e IgG así como presencia de bandas oligoclonales de IgG e IgM en LCR en los ratones susceptibles respecto a los resistentes. Respecto a la capacidad de proliferación de las células T, en ambas cepas la proliferación frente a PHA fue similar (índice de estimulación: 11,4±2,0 vs. 15,8±4,8; p=0,275); sin embargo, solo los esplenocitos de los ratones susceptibles proliferaron frente a PLP139-151 (índice de estimulación: 6,4±1,9 vs. 1,1±0,1; p=0,05). Conclusiones: La presentación adecuada del péptido PLP139-151 induce una respuesta específica tanto humoral como celular, ambas necesarias para el desarrollo de la EAE. Además, los anticuerpos anti-PLP podrían tener un papel crucial en la inducción de EAE en este modelo. 58 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR Targeted Drug Delivery Systems against Breast Cancer Stem Cells – A Proof-of-Concept 9.51 – 10.00 h PETRA GENER CIBBIM - Nanomedicine Drug Delivery and Targeting INVESTIGADOR PRINCIPAL: Simo Schwartz Navarro INVESTIGADOR PARTICIPANTS EN EL PROJECTE: Simo Schwartz Navarro Specific delivery of chemotherapeutic compounds to solid tumors and metastatic foci by nanotechnology based drug delivery systems (DDS) has great potential to reduce the toxicity and adverse effects related to most anticancer drugs used today, reducing the exposure of normal cells and tissues to cytotoxic drugs. In many solid tumors, resistance to therapy and metastatic disease seem to be sustained by the presence within the tumors of cancer stem cells (CSC), capable of regenerating a tumor after chemotherapy and/or radiation treatment. Hence, in order to develop superior chemotherapy treatments, it is necessary to gain a greater understanding of the unique biological properties of cancer stem cells (CSC). Breast CSC correspond to a small fraction of cells within the tumor that express stem cell markers on their surface (CD44+/CD24-/low/lin-), which provide useful targets for new DDS. We provide here a novel strategy for optimising the yield of CSC from a heterogeneous population of breast cancer cell line. A vector expressing the reporter gene tdTomato under the control of the CSC specific promoter was used to separate CSC from MCF7 cell line by FACS. Separated cells were further probed for expression of stem cell markers to ensure specific CSC enrichment and cultured in vitro. The in vivo tumour forming capacity of the isolated cells is currently under investigation. Materiales y Métodos: Corazones aislados de rata fueron perfundidos en un sistema Langendorff y sometidos a 40 minutos de isquemia y diferentes periodos de reperfusión con o sin postcondicionamiento isquémico (6 ciclos de 10 segundos de isquemia y 10 de reperfusión) o tratamiento farmacológico. Muestras de estos corazones se analizaron para detectar producción de peroxinitrito, el acoplamiento de la eNOS y la fosforilación de eNOS y Akt por Western blot, la concentración de BH4 por espectroscopía en ultravioleta y la liberación de nitritos en efluente coronario (medida indirecta de NO). Resultados: Los corazones postcondicionados mostraron una menor producción de peroxinitrito que los corazones control, asociado a una mayor detección de BH4 en citosol, una reducción del monómero de eNOS (eNOS desacoplada) y un aumento en la liberación de nitritos en los primeros minutos de la reperfusión. Sin embargo, no presentaron un aumento en la fosforilación de Akt y eNOS respecto a los controles en ninguno de los tiempos de reperfusión estudiados. Comella FAS and its physiological ligand, FASL, regulate apoptosis of unwanted or dangerous cells, functioning as a guardian against autoimmunity and cancer development. Distinct cell types differ in the mechanisms by which the 'death receptor' FAS triggers their apoptosis. In type I cells, such as HEK293 cells, activation of 'effector caspases' by FAS-induced activation of caspase-8 suffices for cell killing, whereas in type II cells, such as rat PC12 cells, caspase cascade amplification through caspase-8-mediated activation of the pro-apoptotic BCL-2 family member BID (BH3 interacting domain death agonist) is essential. Previous studies have shown that XIAP (X-chromosome linked inhibitor of apoptosis protein) is the critical discriminator between type I and type II apoptosis signalling (Jost et al, 2009). Upon Fas treatment, XIAP is downregulated in Type I cells and upregulated in Type II cells. We have previously demonstrated that the long form of Fas Apoptosis Inhibitory Molecule (FAIM-L), a death receptor antagonist whose expression is specific of the nervous system partially protects cells from Fasinduced apoptosis (Segura et al., J. Neurosci. 2007). Here we demonstrate that FAIM-L protects only Type II but not Type I cells from Fas-induced apoptosis. PC12 cells overexpressing both FAIM-L and XIAP are more resistant to apoptotic cell death induced by Fas than cells overexpressing only FAIM-L or XIAP. Reduction of endogenous expression by siRNA shows that the protective effect of FAIM-L requires the expression of XIAP. Comella Apoptosis is involved in physiological as well as in pathological situations. It is mainly regulated by caspases, which can be activated by the intrinsic pathway, involving mitochondria, and the extrinsic pathway, initiated by the activation of Death Receptors (DRs). Among the DRs involved in apoptosis, Fas plays an essential role in the regulation of cell death in the nervous system. Apoptosis induced by DRs can be regulated at the receptor level in part by a family of inhibitors called Fas-apoptotic inhibitory molecules (FAIM). One of them, lifeguard (LFG), was identified for its ability to protect from Fas-mediated cell death but not by Tumor Necrosis Factor Receptor1 (TNF-R1). High levels of LFG are detected in the hippocampus and the cerebellum, and its expression is up-regulated during mouse postnatal development. The overexpression of LFG in cortical neurons prevents caspase-dependent cell death triggered by the activation of Fas. Moreover, the down-regulation of endogenous LFG by small interfering RNA leads to a sensitization of both cerebellar granule cells and cortical neurons to Fas-mediated cell death. Furthermore, we have begun the identification of LFG-interacting proteins that could shed light on the anti-apoptotic roles of LFG, by means of an immunoprecipitation strategy based on the transient transfection and over-expression of a Flag–tagged LFG construct. As the prostate epithelial cells and their secreted products can be detected in the urine, their use as a proximal-fluid to detect PCa is very attractive. The main aim of this work is to provide a non-invasive early diagnosis of PCa. Methods: Transcriptomic; We tested whether 3 putative PCa biomarkers (PSMA;PSGR;PCA3) could be detected by RTqPCR in 154 post-prostate massage urine (PMU) sediments. Proteomic; We used a combination of proteomic technologies, in 30 post-PMU supernatants to identify differentially expressed proteins for PCa. The identified proteins were qualified using selected-reaction-monitoring (SRM) assays on an independent cohort of 50 PMU. This candidate biomarker and other interesting secreted-proteins were verified using SRM-assay on a total cohort of 107 PMU. 63 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR Results: For the transcriptomic approach we demonstrated that using a multiplex model, we achieved an area under the curve (AUC) for the multi receiver operating characteristic (mROC) curve =0.82 (96% sensitivity and 50% specificity). For the proteomic approach, 24 potential-biomarkers were identified. Qualification of the candidates was then undertaken using an SRM-assay. By statistical analysis 5 proteins were selected that together shown an AUC=0.93 (95% sensitivity and 78% specificity). Conclusions: Taken together, these results provide a strategy for the development of a more accurate model for PCa diagnosis. In the future, a urine-based diagnostic test for PCa could be used to determine better which patients should undergo biopsy. Related publications: 1. Cyclophilins also act as the intracellular receptors of cyclosporine A, a complex that inhibits the activation of the calcineurin/nuclear factor of activated T-cells signaling pathway, which contributes to the immunosuppressive effects of this drug. Cyclophilin B (CypB) differs from other cyclophilin family members for the presence of a C-terminal endoplasmic reticulum retention sequence, enabling secretion of CypB into the extracellular milieu. The presence of CypB in serum and breast milk suggested the involvement of CypB as an intercellular messenger. CypB is also required during the replication of the HCV and may contribute to the pathogenesis of human breast cancer. We have previously described CypB interaction with the Na/K-ATPase pump in human kidney, which is crucial for pump location and activity. In the present work we show that CypB downregulation by siRNA induced cultured proximal tubular cells (HK-2) to differentiate to an epithelial phenotype, as monitored by enzymatic activity and the expression of epithelial markers. In order to elucidate the involvement of CypB in the maintenance of the epithelial phenotype, we have studied the CypB interactome and analyzed the response of siRNA CypB cells to TGFβ. Prostate tumor overexpressed‐1 (PTOV1) , discovered by our group in a screening for genes differentially expressed in prostate cancer, is also overexpressed in high grade malignant bladder, breast, and colon carcinomas. PTOV1 overexpression induces proliferation, migration and invasion in vitro. To uncover its mechanisms of action, we studied interactions with known cellular proteins and signalling pathways. The results suggest a dual function for PTOV1: nuclear PTOV1 is repressing Notch signaling, cytoplasmic PTOV1 promotes c‐Jun translation. Results: hPTOV1 can restore the Notch gain‐of‐function phenotype and enhances the loss‐offunction in vitro and in the Drosophila model. PTOV1 binds to 40S ribosomes regulating protein translation and invasion. To identify mRNAs translationally regulated 66 PROGRAMA – 5a JORNADA CIENTÍFICA VHIR by PTOV1, total and polysomal‐bound mRNA populations from cells overexpressing HA‐PTOV1 and controls were analyzed by microarrays. 1044 mRNAs, showing significant longer 5’UTR, were enriched in polysomes overexpressing PTOV1. Pathway and genesets enrichment analyses suggest that these mRNAs are regulators of cell differentiation, cell surface, endosomes and biosynthetic processes. Conclusions: PTOV1 impacts on tumor progression by downregulating Notch and promoting recruitment and translation of mRNAs specifically involved in regulating cell differentiation and biosynthetic processes. These novel functions of PTOV1 may explain the contribution of its dysregulated expression to cancer aggressiveness, suggesting its usefulness as potential therapeutic target in advanced disease. 67
