Health Insurance?
You can apply for health insurance through the Affordable Care Act (ACA). You can purchase a Qualified
Health Plan* during open enrollment periods, or if you have a qualifying event. Medicaid, Child Health Plus
and The Essential Plan** applications are accepted at any time during the year.
Here are your options:
• Call New York State of Health (the Official Health Plan Marketplace) at 855-355-5777.
• Go online: http://www.nystateofhealth.ny.gov
• For in-person assistance in your area, call Community Service Society at 888-614-5400.
Gather this information to prepare for your application:
 Name, social security number and date of birth for everyone in your tax filing household
 Immigration number for immigrant family members (e.g. Green Card number, employment
authorization number)
 Tax filing status (whether you file taxes, and if so, do you file them alone or with your spouse, and
how many dependents you claim)
 Name and address of your current employer, if this applies
 Income information (it is helpful to have last’s year tax returns for everyone in the household,
paystubs, W2 forms, or if you are self-employed, a record of last three months earnings and
expenses)
 Information about any other health insurance you or other family members may have
HSS Ambulatory Care Center participates with the following programs:

The Essential Plan: Affinity, United Healthcare and Emblem Health HIP*** For up-to-date information

HSS participates with the following NYS Health Exchange plans: Affinity Access, Emblem SelectCare,
Empire BCBS Pathway and United Healthcare Compass. For a participating physician, call Physician
Referral at 212-606-1555.
Questions about Medicaid recertification, call HRA at 718-557-1399.
For Medicaid reinstatement (if your case was closed), call New York State of Health at 855-355-5777.
* A commercial insurance plan that meets the requirements set by the Affordable Care Act.
**A health insurance program for lower-income New Yorkers who don’t qualify for Medicaid or Child Health Plus, new as of 1/2016.
***Please note that Emblem Health HIP requires an in-network authorization for all visits.
