SimplyEngaged is a personal health and wellness program which allows you to earn rewards
when you complete these health and wellness actions.
It’s easy to start earning rewards1
Access the Reward Program Overview through Rally™ when you log in to myuhc.com for
specific details regarding your wellness incentive program.
Earn a Reward

Participate in a biometric health screening2 and get a $75 reward
 Participate in a confidential event through any of the following convenient options: Worksite
event (if employer has elected); Health Provider Form; or Lab Screening
 Learn more about your important health numbers: total cholesterol, blood pressure and
Body Mass Index (BMI).

Complete an online health survey2 through Rally when you log in to myuhc.com® within
90 days of the start of the program and get a $25 reward
 Answer all of the survey questions to personalize your overall experience.
 Complete the survey and receive your results as a “Rally Age” – an indicator of how
your health age compares with your actual age.

Get a $20 reward each month that you visit a participating fitness center2 at least
12 times per month
 Register at a participating fitness center or YMCA® through the Health and Wellness
tab on myuhc.com.
 You must present your fitness ID card each time you go to the gym.

Get rewarded for going to the gym
Going to the gym isn’t always easy. To help you stay motivated, you will get a reward1 for
visiting a participating fitness center at least 12 times a month.

Many fitness centers offer passes that let you try a facility before joining.
How do I enroll?
1. Log in to myuhc.com
2. Click on the Health & Wellness tab to go to RallyTM
3. Access the Reward Program Overview
4. Search for a participating fitness center. (Availability varies by fitness center.)
5. Select your facility from the search results. If you don’t see your preferred facility available,
a referral process exists. Call 1-855-215-0230 if you have questions about your rewards.
6. Follow the instructions to enroll in the fitness reimbursement program.
7. Show your fitness center membership card each time you visit the facility.
8. Earn a reward when you visit the fitness center at least 12 times in a month.

Wellness Coaching
Health coaches may help you change your
behavior and improve your health
Once you’ve received your biometric screening results and completed the health survey, you’ll have
some suggested goals that may help improve your overall health and wellness. To help meet your goals,
you can take part in online health actions (also called Missions) or telephone-based coaching.
Complete 3 Missions and get rewarded1
 You can access your individual online Missions through the Rally™ experience when you
log in to myuhc.com®.
 Missions are interactive action plans focused on your physical activity (Move), nutrition
(Eat), well-being (Feel) and condition-related (Care).
 Log in often to track your progress. Missions take at least 4 weeks to complete.
 Once you complete 3 Missions, you earn a reward!
Complete a telephone-based coaching program and get rewarded1
 You can call the wellness coach at 1-800-478-1057 to begin working on your personal health
improvement plan.

our coach will work with you to track your progress and help you reach your goals. Allow an
average of 2-5 months to complete the program.

2016 SimplyEngaged Incentives for SAISD
By participating in the wellness incentive program, you and your covered spouse can earn up to $200 in rewards for
taking steps to understand and improve your health and well-being. Pick and choose any combination of the activities
listed below to total a maximum $200 reward per covered employee and their covered spouse. If you and your spouse
are covered, that is a total of $400! All activities and incentives must be earned before the deadline of 10/31/16.
