DATA PROTECTION FORM
AUTHORIZATION TO HANDLE PERSONAL DATA AND INFORMATION
BFIS informs you that it has electronic files as well as documents and audiovisual records that contain personal data and information. They
are all part of the ADMINISTRATION’S GENERAL DATABASE.
The school’s database has been created to handle all the information needed to administer and handle files and to provide information to or
contact individuals and entities in accordance with applicable legislation.
The school has public domains, spaces, or platforms of information distribution -web, blogs, magazine and other publications – where
images, recordings, documents or personal information may be published for purposes regarding the school’s activities.
The recipients of the information are the departments, offices and domains under BFIS management as well as some collaborating entities.
The data will only be transmitted/transferred if necessary to fulfill the commitment the school has acquired with the undersigned, for reasons
of force majeure, or when required by the authorities or applicable legislation.
The school has security cameras that have been installed for security reasons.
Should you not agree to give the school the data it requests, the school shall not be able to handle the files, and this could condition your
participation in certain activities, hinder the school from properly handling the files, and most importantly, prevent the school from properly
complying with the contract.
You will be able to exercise your right to consult, exclude, oppose, access, rectify and cancel the information at all times pursuant to Act
15/1.999 of the 13th of December and as specified in RD 1720/2007 of the 21st of December.

In accordance with the information provided heretofore,
I give the school my CONSENT AND AUTHORIZATION to handle both my information and data and that of my child/ren as described
above, as long as such information and data is handled to manage the services that BFIS provides me with. For this reason, I also agree
that BFIS transfers said information and data to public and private entities for the purpose described as long as it is done pursuant to Law
15/1999 of December 13th, Law 34/2002 of July 11th, and Royal Decree 1720/2007 of the December 21st, notwithstanding my right to
revoke said consent should I so desire.

AUTORIZACION PARA EL TRATAMIENTO DE LA INFORMACION Y DATOS PERSONALES
El colegio Benjamin Franklin International School (BFIS) le informa de que dispone de un fichero informático, un archivo documental, y
otro audiovisual, que contienen información y datos personales. Todos forman parte de la base de datos general de administración de
BFIS.
La base de datos de BFIS se ha creado y se gestiona con el fin de tratar toda la información necesaria para administrar los expedientes
y para informar a las personas y a las entidades pertinentes en la forma establecida por la ley.
BFIS dispone de espacios de difusión pública de información-web, blogs, revista y otras publicaciones, en las que se pueden publicar
imágenes, sonido, trabajos, o datos personales en el ámbito de la actividad de BFIS.
Los destinatarios de la información son los departamentos, oficinas y ámbitos gestionados por BFIS, así como algunas empresas
colaboradoras. Sólo se cederán aquellos datos que sean necesarios para dar cumplimiento al compromiso de BFIS con el firmante, por
causa de fuerza mayor, o cuando lo requiera una autoridad, de acuerdo con las leyes establecidas.
BFIS tiene instaladas (o instalará) cámaras de video vigilancia por motivos de seguridad.
La negativa a dar los datos que pedimos, tendrá como consecuencia, que no podamos gestionar los expedientes, y, por tanto, puede
condicionar su participación en algunas actividades, el tratamiento de su información, y, sobre todo, el cumplimiento del contrato.
En todo caso, tiene derecho a ejercer los derechos de consulta, exclusión, oposición, acceso, rectificación y cancelación en el ámbito
reconocido por la ley Orgánica 15/1.999 de 13 de diciembre, mediante el procedimiento establecido en el RD 1720/2007 de 21 de
diciembre. El responsable de la base de datos es BFIS. Para ejercer sus derechos reconocidos por ley y para cualquier aclaración,
puede dirigirse a Lola Cortés a través de correo electrónico [email protected] o llamando al 93 434 23 80 ext. 357.
De acuerdo con la información proporcionada,
CONSIENTO Y AUTORIZO a BFIS para que trate mi información y la de mi/s hijo/s y mis datos y los de mi/s hijo/s de acuerdo con la
información recibida, siempre que el tratamiento tenga como finalidad la gestión de los servicios que BFIS me presta. Por este motivo,
también doy mi consentimiento a BFIS para que ceda mi información a las entidades públicas y privadas adecuadas para las finalidades
descritas y siempre que se haga de acuerdo con lo dispuesto en la Ley 15/1999 de 13 de diciembre, la Ley 34/2002 de 11 de julio y el
Real Decreto 1720/2007 de 21 de diciembre, sin perjuicio del derecho a revocar el consentimiento que otorgo ahora, cuando lo
considere oportuno.
