Registration will be completed online. If you do not have
internet access, you can register at the main office of your
student’s school.

*Payment online will need to be completed by credit card or
check.
Cash Payments will need to be made at your school’s main
office. If paying with cash, and you are free/reduced, you
will need to come to the District Office and pick-up a copy
of your free/reduced status letter from Paul Guymon.
On the back of this flier you will find the Income Eligibility
Guidelines for Free/Reduced Lunch. Logan Afterschool
Club will be using the same Income Eligibility Guidelines in
determining ASC Session Fees. If your student qualities for
free lunch, your student(s) is eligible to attend ASC free of
charge.
If you qualify for reduced lunch, your session fees are as
follows:
 $15 - One (1) Student
 $30 - Two (2) Students
 $45 - Three (3) Students
 $60 - Four or more (4+) Students
If you do not qualify for free or reduced lunch, then your
session fees are as follows:
 $50 - One (1) Student
 $100 - Two (2) Students
 $150 - Three (3) Students
 $200 - Four or more (4+) Students
Please have the following information ready when
registering:


Name of your student's teacher
Full names of two (2) emergency contacts, as well
as their phone numbers
 Names of your child's medications, if they take any
on a regular basis
