It’s important to ask questions and get information from your healthcare providers, so you can work with them to stay healthy. GETTING READY FOR YOUR APPOINTMENT Always be prepared when you go in for a visit. Ask yourself, what are my health goals and what symptoms or conditions do I want to discuss with my provider? Use the form that you received with this flyer to write your questions down before your appointment. SET EXPECTATIONS Use the form we’ve included with this flyer to take notes before, during and after your appointment. On the back, you'll see more questions that you may want to ask your provider. Think about what you want to get out of your visit. Let your provider know right away what you expect, and ask your provider what their expectations are of you so that you can take an active role in your healthcare. BRING A FRIEND It can be helpful to take a family member or friend with you when you go to your office visit. They can help remind you about things you planned to ask and remember what your provider says. ASK FOR AN INTERPRETER If you need an interpreter to help you communicate with your provider, let the office staff know when you schedule your appointment. Summarize what your provider said to make sure you understand by saying: “Thank you for providing all of that information. Now, let me see if I understand it. I heard you say . . .” Use the form to take notes.
