PROBATION OFFICER MUST CHECK ALL CONDITIONS THAT APPLY
1. You will participate in and complete any sex offender evaluation and recommended treatment as directed by a
Probation Officer. You may be financially responsible for all or part of the costs of such evaluation and treatment.
2. You will participate in polygraph examinations administered by a CSSD-approved, specially trained polygraph
examiner for treatment purposes and level of supervision.
3. You will have no contact with the victim(s) or victim(s') family (including, but not limited to, by means of letter,
telephone call, tape, video, email, text message or third party contact) unless approved by a Probation Officer.
Contact with the victim(s) or victim(s') family must be reported immediately to a Probation Officer.
4. You will notify your Probation Officer of any new or existing romantic or sexual relationship.
5. Your place of residence must be approved by a Probation Officer. You will not move from your place of residence
or sleep elsewhere overnight without a Probation Officer's prior knowledge and permission.
6. You will allow any Probation Officer entry into your place of residence and notify any occupant of your residence
that a Probation Officer may enter where you live.
7. All employment must be pre-approved by a Probation Officer.
8. You will provide financial and telephone records upon a Probation Officer's request.
9. You will abide by any curfew imposed by a Probation Officer.
10. You will not possess, or subscribe to, any sexually explicit or sexually stimulating material deemed inappropriate
by a Probation Officer. You will not patronize any adult book or adult video store, strip club, or adult entertainment
club or similar establishment.
11. You will not utilize telephone numbers which provide access to sexually oriented services.
12. You will not hitchhike or pick up hitchhikers.
13. You will submit to electronic monitoring as directed by a Probation Officer. You may be required to pay for the costs
of such monitoring. You will not tamper with electronic monitoring equipment and you will not remove the equipment.
14. You will participate in any other treatment program as directed by a Probation Officer.
15. If medication is required as part of your treatment program, such medication will be taken as prescribed.
16. You will abstain from the use of any alcoholic beverages and/or drugs, except as prescribed to you by a physician.
17. You are not to associate with other known sex offenders and/or convicted felons except in an approved treatment
program or with prior Probation Officer approval.
18. You will not be in the presence of minors, nor have contact in any form, direct or indirect, including, but not limited to,
personally, by means of computer, telephone, letter, tape, video, email, text message or through another person
with children under the age of sixteen without Probation Officer approval. Any contact must be reported
immediately to a Probation Officer.
19. You will not date or socialize with anybody who has children under the age of sixteen without prior approval from a
Probation Officer.
20. You are not to possess children's clothing, toys, games, etc., without prior Probation Officer approval.
21. You will not go to or loiter near school yards, parks, playgrounds, swimming pools, arcades, or any places
primarily used by children under the age of sixteen.
22. You will not possess a camera, camera phone, camcorder, videocassette, DVD or any device that can record
and/or play back visual images without Probation Officer approval.
23. You must maintain a driving log and/or activity log, as required by a Probation Officer.
24. You must agree to and sign a Computer Access Agreement, JD-CR-143, that will be provided to you by a
Probation Officer, prior to using any computer while on probation and you must submit to an examination and
search of your computer or other similar equipment to verify it is not utilized in violation of probation and/or
treatment conditions.
25. Other Probation Officer imposed conditions:

probation. I understand them, was provided a copy, and will abide by them.

EL AGENTE DE LIBERTAD A PRUEBA (ALP) DEBE MARCAR TODAS LAS CONDICIONES PERTINENTES
1. Usted deberá hacerse y completar cualquier evaluación destinada a delincuentes sexuales y se pondrá el
tratamiento recomendado según disponga el ALP. Será responsable en el plano económico de todos los costos
de esta evaluación y tratamiento o de parte de ellos.
2. Participará en exámenes poligráficos realizados por un examinador poligráfico aprobado por la CSSD y
especialmente adiestrado, para determinar los objetivos del tratamiento y el nivel de supervisión
3. No tendrá contacto con la víctima o la familia de ésta (ni siquiera por carta, por teléfono, cintas, videos, correo
electrónico, mensaje de texto, ni a través de una tercera parte) a menos que lo autorice su ALP. Todo contacto
con la víctima o la familia de ésta deberá informarse inmediatamente a su ALP.
4. Notificará a su ALP de cualquier relación sexual o amorosa nueva o ya existente.
5. Residirá en un domicilio aprobado por su ALP y no se mudará de residencia ni se quedará a dormir en otro sitio
durante la noche sin el conocimiento y la autorización previos de un ALP.
6. Permitirá la entrada en su domicilio de cualquier ALP y le avisará a todo ocupante de dicho lugar de residencia
que un ALP puede entrar donde usted vive.
7. Todo empleo deberá ser aprobado de antemano.
8. Mostrará las cuentas del teléfono y documentos de su situación económica a solicitud del ALP.
9. Deberá cumplir con el horario de estar en casa impuesto por el ALP.
10. No poseerá ni se suscribirá a ningún tipo de materiales sexualmente explícitos o estimulantes que no Considere
adecuados el ALP. No frecuentará ninguna librería ni videoteca eróticas, clubes de strip-tease, clubes con
espectáculos para adultos ni otros establecimientos similares.
11. No utilizará números de teléfono que den acceso a servicios de índole sexual.
12. No pedirá aventón ("pon") ni se lo dará a los que lo pidan.
13. Se someterá a monitoreo electrónico según estime necesario el ALP y se le puede exigir que pague el costo
del mismo. No adulterará ni se quitará el equipo de monitoreo electrónico.
14. Participará en cualquier otro programa de tratamiento según indique el ALP.
15. Si requiere medicamentos como parte de su plan de tratamiento, tomará los mismos según lo prescrito.
16. Se abstendrá de consumir todo tipo de bebidas alcohólicas o drogas, excepto si las receta el médico.
17. No podrá relacionarse con otros delincuentes sexuales conocidos o condenados por delitos graves, excepto en
un programa de tratamiento aprobado o con la autorización previa del ALP.
18. No podrá estar en presencia de menores ni tener contacto en forma alguna, directa o indirecta, personal, por
computadora, por teléfono, por carta, cinta, video, correo electrónico, mensaje de texto o por medio de otra persona,
con menores de 16 sin la aprobación del ALP. Todo contacto deberá informarse inmediatamente a su ALP
19. No saldrá ni se relacionará con ninguna persona que tenga hijos menores de 16 sin la aprobación previa de su ALP.
20. No puede poseer ropas de niño, juguetes, juegos, etc. sin la aprobación previa del ALP.
21. No acudirá ni permanecerá en las cercanías de los patios de las escuelas, parques, parques infantiles, piscinas,
salas de juegos electrónicos u otros sitios frecuentados principalmente por menores de 16.
22. No poseerá cámaras, teléfono con cámara, cámaras de video, videocasetes, DVD, ni ningún equipo que pueda
grabar o reproducir imágenes visuales sin la aprobación del ALP.
23.Tiene que llevar un diario de sus actividades y los sitios adonde va en su vehículo según requiera el ALP.
24.Tiene que aceptar y firmar el Acuerdo de Acceso a la Computadora, JD-CR-143, que le dará el ALP, antes de usar
cualquier computadora mientras se encuentre en libertad a prueba. Tiene que someterse al examen y registro de su
computadora u otro equipo similar para verificar que no se emplea en violación de las condiciones de la libertad a
prueba, el tratamiento o ambos.
